Predictive value of myocardial injury markers and NT ⁃ proBNP for clinical outcomes in elderly patients with COVID ⁃19

Objective To unveil the prognostic effect of myocardial injury markers and NT⁃proBNP on patients aged ≥75 years with coronavirus disease⁃19(COVID⁃19).Methods A total of 141 consecutive patients aged ≥75 years who were admitted to the hospital between 12th and 19th February 2020. In⁃hospital death,clinical characteristics and myocardial injury markers and NT ⁃ proBNP on admission were obtained from medical records. The association of myocardial markers evaluated by LDH,CK,CK ⁃MB,AST,ALT,hs ⁃TnI,and NT ⁃ proBNP were collected. Results 18(12.8%)patients died,including 11(61.1%)male and 7(38.9%)female. Patients with elevated level of LDH,hs⁃TnI,and NT⁃proBNP had significantly higher risk of death than those in discharged group(P < 0.05). ROC curve analysis showed that the area under curves of NT⁃proBNP was the highest(0.94), followed by hs⁃TnI(0.88). When the cutoff value of NT⁃proBNP was 1 025 ng/L,the positive predictive value was 94.40% ,the negative predictive value was 89.40%. Conclusions Myocardial injury and elevated NT ⁃proBNP levels are common in elderly patients with COVID⁃19,

Objective To investigate the factors affecting the positive duration of SARS⁃CoV⁃2 antibody in patients with COVID ⁃ 19.Methods A retrospective analysis was made among 43 patients with COVID ⁃ 19 who were followed up for more than 6 months in our hospital from January 21 to October 25,2020. The clinical charac⁃ teristics were compared between those patients with antibody negative test results and those with persistent positive test results after 6 months of follow⁃up. Results After follow⁃up of over 6 months,both SARS⁃CoV⁃2 IgM and IgG tested negative in 10 patients,taking up 25.6%. The patients tested persistently positive were characterized by old age,lower lymphocyte count and higher incidence rate of fever(P < 0.05). Multivariate regression analysis showed that lower lymphocyte count and fever were independently related to the persistent antibody positive test resultsfor more than 6 months. Objective To investigate the behavioral characteristics of emergency patients in departments of neurology at the peak of epidemic ,Covid ⁃19. Methods Comparisons were made between the emergency patients in the department of neurology of our hospital in 2020 and those in 2018 and 2019 in terms of gender,age and main symptoms. Then the patients in 2020 were evaluated for post⁃traumatic stress disorder(PTSD). Results In 2020,the proportion of female patients in the epidemic peak was significantly decreased ,with statistically significant difference(= 0.003). There was significant difference in the proportion of patients with acute stroke and somatization(< 0.001). There was no significant difference in the incidence of PTSD among different gender,age and symptom classification(= 0.388,0.909 and 0.224,respectively). Conclusion At the peak of Covid⁃19,the number of women and patients with somatization symptoms was relatively reduced. Therefore,more attention should be paid to the women and patients with somatization symptoms for their health the subsequent epidemics.

Objective To explore psychological resilience and influencing factors in the frontline medical staff during the fight against novel coronavirus pneumonia. Methods The medical staff worked on the frontline of the anti⁃epidemic period from January to April 2020 were selected as study objects. The general information questionnaire psychological resilience scale,post⁃traumatic stress disorder scale,and social support scale were used to conduct a questionnaire survey on posttraumatic stress disorder,psychological resilience,and social support levels in the frontline medical staff during the fight against the epidemic. Single⁃ and multiple⁃factor Logistic regression was applied to analyze the influencing factors of psychological resilience. Results The scores of psychological resil⁃ ience,posttraumatic stress disorder and social support level of in the front⁃line medical staff were(63.01 ± 7.49), 31.59 ± 9.01),and(39.32 ± 5.94)points,respectively. Disinfection protection(OR = 5.115),prevention and control training(OR = 3.503),stress response(OR = 0.889),and social support(OR = 2.173)were independent factors affecting psychological resilience in the front⁃line medical staff. Conclusions The scores of psychological resilience in the anti⁃epidemic front⁃line medical staff are lower than those at the average level. Psychological resil⁃ience is closely related to stress response and social support.

Congenital heart disease(CHD)is the most common congenital malformation and surgery is the main treatment. Due to the complexity of CHD,the individual curative effect and prognosis vary widely. Preoperative risk prediction can guide the timely targeted measures to avoid the occurrence of perioperative adverse events,which greatly improve the prognosis and quality of life of patients. Surgical risk assessment has always been emphasized in cardiac surgery. In recent years,the role of fundus blood vessels in the diagnosis and treatment of cardiovascular diseases has become increasingly prominent. This article aims to discuss the fundus changes of patients with CHD and the research progress of its surgical risk prediction model,and to provide related information in the prognosis evaluation of congenital heart surgery.

Objective To explore the role of glycoprotein Serglycin in chemoresistance and metastasis of epithelial ovarian cancer. Methods The stable interference cell line was established from epithelial ovarian cancer SKOV3 cell line by interfering with Serglycin expression using shRNA technology,and Western blot technology and qRT ⁃PCR were employed to verify knockdown efficiency. MTS method was used to detect the sensitivity of ovarian cancer cell line with stable knockdown of Serglycin to the chemotherapeutic drug cisplatin. Transwell cell was used to detect the invasion and metastatic ability of ovarian cancer cell lines with stable knockdown of Serglycin;Western blot was used to detect the effect of Serglycin on EMT ⁃ related molecules. In addition,online prognosis analysis software(kmplot)was used to analyze the correlation between Serglycin and the prognosis of patients with epithelial ovarian cancer. Results Knockdown of Serglycin cells significantly enhanced the sensitivity of epithelial ovarian cancer cells to the chemotherapy drug cisplatin and significantly inhibited the invasion and metastasis ability of ep⁃ ithelial ovarian cancer cells compared with the control group. The knockdown promoted the EMT changes of epithelial ovarian cancer cells via ZEB2. The overall survival of the patients with low expression of Serglycin was significantly longer than those with high expression(HR = 1.26,P<0.001). Conclusion Serglycin promotes EMT changes of ovarian cancer cells via ZEB2,and then promotes chemoresistance and invasion and metastasis of ovarian cancer cells.

Objective Traditional Chinese medicine chloroquine(CQ)has been used as one of the main drugs for rheumatoid arthritis treatment,but its mechanism of action is still unclear. This study investigated the inhibitory effect of CQ on the activation of fibroblast⁃like synoviocyte(FLS)in rheumatoid arthritis and its molecular mechanism. Methods MH7A cells were pre⁃stimulated by TNF⁃α to establish a FLS activation model for rheuma⁃ toid arthritis. The effects of CQ on MH7A cell proliferation,pro⁃inflammatory cytokines IL⁃6 and CXCL10,and invasion proteins MMP⁃2 and MMP⁃9 were detected. Then,TRAF3 expression and autophagy in MH7A cells were evaluated after CQ treatment. Afterward,TRAF3 was down⁃regulated by siRNA and autophagy was induced by rapamycin to examine the role of TRAF3 in CQ ⁃induced inhibition of FLS activation and its mechanism. Results CQ effectively inhibited the proliferation of MH7A cells ,decreased the secretion of IL⁃6 and CXCL10 ,and reduced MMP⁃2 and MMP⁃9 expression. CQ did not up⁃regulate TRAF3 expression,but increased TRAF3 protein content. And the TRAF3 mediated the CQ⁃induced inhibition on FLS activation. CQ inhibited autophagy,while rapamycin reduced TRAF3 protein levels and antagonized the inhibition of chloroquine on FLS activation. Conclusion CQ can increase the TRAF3 protein in FLS by inhibiting autophagic degradation,and inhibit the abnormal activation of FLS in rheumatoid arthritis via TRAF3.

Objective To explore the function of Dock2 in the host′s adaptive immunity against C. roden⁃ tium infection,and study the relationship between the changes of lymphocytes and bacterial clearance in the late stage of C. rodentium infection. Methods WT and Dock2⁃/⁃mice were intragastrically infected with C. rodentium to observe the infection within 21 days. ELISA was used to detected IFN⁃γ,TNF⁃α,KC,MCP⁃1 and MIP⁃2 in the sera of mice during 21 days,the protein expression levels of IL⁃6 and IL⁃17,and the levels of specific antibodies IgM,IgG,IgG1,IgG2b,IgG2c and IgG3;flow cytometry was used to detect T lymphocytes and thymocytes. The T lymphocytes isolated from WT and Dock2-/- were injected into the TCRa-/- mice by tail veins,and the levels of IgG,IgG1,IgG2b,IgG2c,IgG3,IgA and IgM were detected by ELISA. Sera of WT mice at D0 and D21 after infection were injected into the tail veins of Dock2-/- mice followed by infection,and the survival rates were recorded. Results:After C.rodentium infection,Dock2-/- mice had much more production of pro⁃ inflammatory cytokines and macrophage/neutrophil infiltration than WT mice. The mechanism of bacterial clearance disorder caused by Dock2 deletion after C. rodentium infection may be associated with the T⁃cell and B⁃cell deficiency,and reduced serum antigen⁃specific IgG antibody in Dock2-/- mice. Consistently ,T cell transfer and serum transfer has inhibitory effects on C.rodentium⁃induced colitis. Conclusion Dock2 was required for host bacterial clearance after C.roden⁃tium infection.

Objective To investigate the osteogenic differences of the Beagle canine mandible after implantation of the pure titanium implants treated by sandblasting and acid etching. Methods Four Beagle dogs were implanted with the pure titanium implants after sandblasting and acid etching. The Beagle dogs were sacrificed at 2 weeks and 4 weeks after implantation,and the specimens were observed radiologically and histologically. Results All the wounds in the four Beagle dogs were healed in the first stage without implant loosening. Histological and radiological observation showed that new bone formation on the surface of the implant could be observed. Contact and distance osteogenesis were observed. Contact osteogenesis is related to the biological characteristics of different bone regions of the mandible,and the area close to cortical bone has a good effect of contact osteogenesis. Contacting with cortical bone was not obvious in areas larger than 600 μm. Conclusion Contact osteogenesis was observed on the surface of pure titanium implants with acid etching and sandblasting,and contact osteogenesis was related to the biological characteristics of different bone regions of the mandible,and more new bone was generated in the area close to cortical bone.

Objective To investigate the effect of miR ⁃ 191 on endometrial cancer cells and its mecha⁃ nism. Methods RT⁃PCR method was used to detect the expression of miR⁃191 and TET1 in endometrial cancer tissues,adjacent tissues and endometrial cancer cells,and the changes of TET1 mRNA were detected after transfection of miR ⁃ 191 inhibitor. MTT and Transwell method were used to detect cell proliferation and invasion ability. Western blot method was used to detect cell TET1 protein expression. The dual luciferase report analyzed the relationship between miRNA ⁃191 and TET1. Results The expression of miR ⁃191 in endometrial cancer tissues was higher than that in adjacent tissues,but the expression of TET1 was lower. Inhibition of miR⁃191 could significantly inhibit the proliferation and invasion of Ishikawa cells and up ⁃ regulate the expression level of TET1 (P<0.05). Dual luciferase report showed that TET1 was a potential target gene of miR⁃191,and miR⁃191 had a significant regulatory effect on the expression of TET1 gene. Conclusion miR ⁃191 may inhibit the proliferation and invasion of endometrial cancer cells by targeting down⁃regulation of TET1 expression.

Objective To investigate whether resveratrol regulates apoptosis of HCT 116 cells through SATB2 and its mechanism. Methods HCT 116 cells were treated with resveratrol(Res)at different concentrations for 24 h,and the optimal dose of Res was determined to be 8.33 μg/mL. HCT 116 cells were divided into control group,si⁃SATB2 group,si⁃SATB2+ Res group,empty plasmid group,SATB2 overexpression group,Res group and SATB2 overexpression + Res group. After treatment,total protein was extracted from the cells,and the protein expressions of SATB2,Bax,Bcl2,Caspase⁃3,9 and Cyt⁃C were detected by Western Blot. Results (1)Resvera⁃ trol up ⁃ regulated the expression of SATB2 protein in HCT116 cells.(2)Knocking down of SATB2 gene incveased the Bcl⁃2/Bax ratio and down⁃reglated the expression of CYT⁃C ,Caspase⁃3 and Caspase⁃9 proteins,and Res reversed these effects.(3)Overexpression of SATB2 reduced the Bcl⁃2/Bax ratio in HCT 116 cells and down⁃regulate the expression of Cyt⁃C,Caspase⁃3 and Caspase⁃9 proteins,while Res enforced these process. Conclusion Inhi⁃ bition of SATB2 reduced the level of apoptosis in HCT 116 cells,and Res could promote the expression of apoptotic protein in HCT 116 cells by up⁃regulating SATB2.

Objective To investigate the efficacy and safety of endoscopic bilateral thyroidectomy using the gasless subclavian approach space. Methods Retrospective analysis of 39 patients who underwent bilateral thyroid surgery in the First Affiliated Hospital of Bengbu Medical College from October 2019 to September 2020 was performed,including 19 cases of subclavian incision and 20 cases of traditional open surgery. Analysis of the differences in total operative time,estimated intraoperative blood loss,central lymph node dissection in malignant patients,postoperative pain visual analogue scale(VAS),postoperative hospitalization time,postoperative compli⁃ cations,and cosmetic satisfaction at 3 months after surgery. Results The average operation time of the endoscopic group was longer 1than that of the traditional open group(P < 0.05),the pain score 3 days after operation was higher than that of the traditional open group(P < 0.05),and the postoperative cosmetic satisfaction was signifi⁃ cantly higher than that of the traditional open group(P < 0.05). There was no significant difference in intraopera⁃ tive blood loss,postoperative hospital stay,number of central lymph node dissection,postoperative complications and pain score 3 months after operation between the two groups(all P > 0.05). Conclusion Endoscopic bilateral thyroidectomy using the gasless subclavian approach is suitable for patients with high cosmetic requirements,and patients′ satisfaction with incision cosmetic is high.

Analysis of early efficacy of medial opening ⁃ wedge vs. lateral closed ⁃ wedge high tibial osteotomy in the treatment of medial compartment knee osteoarthritis

Objective To compare the clinical efficacy of medial opening⁃wedge with lateral closed⁃wedge high tibial osteotomy for medial compartment osteoarthrosis of the knee. Methods A retrospective analysis was performed on 48 patients with medial compartment osteoarthritis of the knee treated from January 2014 to December 2018,including 25 receiving OWHTO and 23 receiving CWHTO. The hip,knee and ankle angles(HKA)and proximal medial tibia angles(MPTA)were measured during regular postoperative follow ⁃ up,and pain and knee function were assessed by visual analogue scale(VAS)and knee HSS scores. Results There were no statistical significant differences in preoperative MPTA,HKA,HSS and VAS scores between OWHTO and CWHTO groups (P > 0.05). At the last follow⁃up,the MPTA angle was(90.47 ± 1.15)° in OWHTO group and(90.95 ± 1.18)° in CWHTO group,and the HKA angle was(180.25 ± 1.55)° and(180.91 ± 2.03)°,respectively. There were no statistical significances between the groups(P > 0.05),but there were significant differences from the preoperative comparison(P < 0.05). At the last follow ⁃ up,the HSS score was(84.60 ± 2.29)points in OWHTO group and (85.30 ± 1.55)points in CWHTO group. There was no statistical significance(P > 0.05)between the two groups but as compared with that before surgery,there was significant difference(P < 0.05). Conclusions OWHTO and CWHTO have a better clinical efficacy for medial compartment osteoarthritis of the knee,and there is no signifi⁃ cant difference in the early efficacy between the two procedures.

Objective To investigate the prognostic value of diffusion⁃weighted(DW)MRI combine tumor volume for the treatment response of nasopharynx in nasopharyngeal carcinoma(NPC). Methods 94 patientswith NPC were enrolled ,ADC value and tumor volume of nasopharynx was recorded before treatment. Patients were divided into complete remission(CR)group and non CR group according to treatment response,the predic⁃ tive ability of tumor volume and ADC value and their combination was analyzed. Results The pretreated value of ADC,gross tumor volume of nasopharynx(GTVnx)and GTVnx with retropharyngeal lymph nodes(GTVnx+RLN showed statistical difference between CR group and non CR group. AUC of ADC,GTVnx and GTVnx+RLN were 0.656,0.678 and 0.699 respectively. AUC reached 0.772 when we combined ADC value with GTVnx+RLN. We further incorporated ADC value and GTVnx + RLN by constructing nomogram,C ⁃index were 0.770. Conclusion The combination of pretreated ADC value and GTVnx+RLN can be used to predict the treatment response in NPC.

Objective To detect the expression level of the NLRP3 inflammasome and the associated cytokines of syphilis serofast state,and to investigate the effect of NLRP3 inflammasome in the formation of syphilis serofast. Methods In a case⁃control study of 50 syphilis serofast patients and 50 healthy subjects,qRT⁃PCR was used to detect the expression of NLRP3 inflammasome and associated cytokines mRNA in peripheral blood mononu⁃ clear cells(PBMCs)of patients with syphilis serofast state and healthy subjects ,and to detect the levels of cytokines IL⁃1β and IL⁃18 in the serum by ELISA. Results The expressions of NLRP3,ASC,Caspase⁃1,IL⁃1β and IL⁃18 mRNA in PBMCs of patients with syphilis serofast state were significantly lower than those of healthy subjects(all P < 0.05),and the contents of IL⁃1β and IL⁃18 in serum of syphilis serofast state patients were also lower than those of healthy controls(both P < 0.05). Conclusion The expression of NLRP3 inflammasome and downstream cytokines decreases in patients of syphilis serofast ,and suggests that it maybe related to the natu⁃ral immunity abnormality of these patients.

Objective To explore application value of three ⁃ dimensional reconstruction technology in thoracoscopic radical resection for lung cancer in the elderly patients with lung cancer and tuberculosis. Methods A retrospective analysis was performed on 80 elderly patients with lung cancer and tuberculosis who had undergone thoracoscopic radical resection for lung cancer in our hospital from August 2015 to October 2019. Forty ⁃one patients receiving surgical procedures based on three⁃dimensional reconstruction and virtual surgery were assigned to group A,while another thirty⁃nine patients undergoing routine surgery were assigned to group B. The surgical situation was compared between the two groups. Results The proportions of lobular sign,spinous process sign vascular convergence sign,short burr sign and pleural depression sign displayed by three⁃dimensional reconstruction imaging were greater than those by axial imaging(P < 0.05). The accuracy,sensitivity and specificity of three⁃ dimensional reconstruction for determining lymph node metastasis were 87.80%,93.10% and 75.00%,which were higher than those of axial imaging(70.00%,75.00%,and 58.33%). The surgical duration and postoperative hospital stay were shorter,intraoperative blood loss and postoperative drainage volume were lower,and number of intraoper⁃ ative resected lymph nodes was larger(P < 0.05),and the incidence of complications was lower in group A than in group B(9.76% vs. 26.83%,P < 0.05). Conclusions Application of three⁃dimensional reconstruction technology in thoracoscopic radical resection for lung cancer in the elderly patients with lung cancer and tuberculosis can effectively provide lesion information,which is beneficial to shortening surgical duration,reducing intraoperative blood loss,and lowering occurrence of postoperative complications.

Objective To explore the abnormal expression of CD4+ T cells and NK cell perforin(PRF)to evaluate the immune status and condition of children with hand ⁃foot ⁃mouth disease(HFMD),and to analyze its guiding value for clinical prognosis. Methods A total of 143 children with HFMD admitted to our hospital from January 2017 to June 2020 were selected and divided into normal group(= 67),severe group(= 45)and critical group(= 31). Another 35 children with normal findings in physical examination were selected as the control group. T lymphocyte subsets(CD3+T,CD4+T,CD8+T),monocytes(CD14+ ),NK cell PRF,and granzyme B(GrzB),B cell subsets(CD19 + ),immunoglobulins(IgA,IgG,IgM)and complement(C3,C4)levels were compared. The expression of CD4+ T cells,NK cells PRF and the severity of the disease(PCIS score,PRISM III score)as well as the prognostic relationship were analyzed. Results The numbers of CD3 +T,CD4 +T,CD14 + CD19+B,and NK cells in the severe group,critical group,and normal group were lower than those in the control group. The expressions of NK cell PRF and GrzB were higher than those in the control group where the severe group and the critical group had more obvious changes than the normal group(< 0.05). The levels of IgA,IgG,IgM and complement C3 and C4 in each group have no significant difference(> 0.05);Pearson correlation analysis showed that CD4+ T cells were positively correlated with PCIS score(= 0.665,< 0.05),and negatively correlated with PRISMⅢscore(= -0.757,< 0.05),PRF was negatively correlated with PCIS score(= -0.615,< 0.05), and positively correlated with PRISMⅢ score(= 0.596,< 0.05). CD4+ T cells in children with good prognosis were higher than those with poor prognosis,and PRF of NK cells was lower than those with poor prognosis(< 0.05). Conclusion Children with hand,foot and mouth disease had obvious low expression of CD4+ T cells and high expression of NK cells PRF,both of which could be used to assess the cellular immune status of severely ill children and are related to the severity of the disease and poor prognosis.

Objective To evaluate the expression of tissue factor(TF)on platelet surface during prepara⁃ tion and storage of platelet concentrates(PC)for transfusional use. Methods Platelet TFs in healthy donor whole blood and PC were measured by flow cytometry on the day of preparation and up to 4 days of storage with classical platelet activation markers,i.e.,fibrinogen,P ⁃selectin,and glycoprotein GPIIb(CD41)analyzed as well. The data were analyzed according to blood ABO group. Results TFs were detected on whole blood platelets,with the highest detection ratein O donors. Compared to the whole blood,the detected TFs were higher on the platelet surface,even escalating with the process of storage. The rise in TF levels were positively correlated with the elevations of the other platelet markers. Conclusion Platelet surface TF is contained in PC. Further studies are needed to investigate a possible correlation between the TF levels and the hemostatic response of platelet transfusion recipient.

Objective To explore the clinical efficacy of montelukast in the treatment of patients with elevated serum LTC4 and FeNO⁃positive cough variant asthma as well as its prognosis. Methods A total of 120 patients with cough variant asthma(CVA)were selected from the outpatient department of the respiratory department of our hospital and divided into groups A(increased serum LTC4,FeNO negative group),B(increased serum LTC4 FeNO positive group),C(low serum LTC4,FeNO positive group),and D(low serum LTC4,FeNO negative group),30 cases in each group. Another 30 healthy people were selected as the control group. All groups A,B,C and D received oral treatment of 10 mg montelukast once a night. After 1 month and 3 months of treatment,the efficacy of severe cough symptom score,LTC4,PEF and FeNO levels were evaluated respectively,and the patients were followed up for 3 months after the treatment. Follow ⁃ up visits were conducted for 3 months after the end of treatment. Results In group A,the cough symptoms were relieved after 1 month and 3 months of treatment,and the serum LTC4 level and PEF mutation rate were significantly reduced(P < 0.05). The cough symptoms in group B were relieved after 1 month and 3 months of treatment,and the serum LTC4 level,FeNO level,and PEF mutation rate were all reduced,the difference was statistically significant(P < 0.05);there was no statistical difference in the efficacy between the two groups(P > 0.05). The cough symptoms of the subjects in group C were slightly relieved after 1 month of treatment,FeNO levels and the PEF variation rate was slightly reduced,without statistically signifi⁃ cant difference(P > 0.05). After 1 month of treatment,the cough symptoms in group D were slightly relieved and the PEF variation insignificantly reduced(P > 0.05). Conclusion Montelukast has a good clinical effect in the treatment of adult CVA patients with elevated serum LTC4,but it is poor clinically for treating adult CVA patients with low serum LTC4. During the effective treatment period of 3 months,the longer the treatment duration is,the better the effect.

Objective To investigate the frozen⁃thawed embryo transfer(F⁃ET)outcomes of the patients with repeated embryo implantation failure(RIF)after intrauterine injection of dexamethasone sodium phosphate (DexSPI). Methods RIF patients who received F⁃ET again in our hospital from June 2018 to October 2020 were assigned into DexSPI group and control,and the former received intrauterine injection of dexamethasone sodium phosphate on the day of endometrial transformation,while the later did not receive intrauterine injection on the same time. The clinical outcomes were analyzed and compared between two groups. Results The clinical pregnancy rate and embryo implantation rate in DexSPI group were significantly higher than those in the control group(clini⁃ cal pregnancy rate 47.62% vs. 26.51%,embryo implantation rate 38.46% vs. 21.24%)(P < 0.05),no significant difference in early abortion rate. Conclusion Intrauterine injection of dexamethasone sodium phosphate may im⁃ prove the F⁃ET outcomes of the patients with RIF.

Objective To investigate the clinical value of combined detection of alpha⁃fetoprotein(AFP), alpha⁃fetoprotein alloplasm⁃3 ratio(AFP⁃L3%)and Golgi somal glycoprotein 73(GP73)in primary liver cancer. Methods The levels and positive rates of AFP,AFP⁃L3% and GP73 in serum of patients with chronic hepatitis liver cirrhosis and primary liver cancer were detected,and the data were compared. Results AFP levels in chronic hepatitis group,cirrhosis group and primary liver cancer group were(97.1 ± 88.2)ng/mL,(208.2 ± 204.1)ng/mL 284.5 ± 275.2)ng/mL respectively,which were significantly higher than those in the control group. AFP positive rate in primary liver cancer group was 53.3%,which was significantly different from the other three groups. The level of AFP ⁃ L3% and the positive rate of AFP ⁃ L3% in primary liver cancer group were(32.8±21.3)% and 75.0%,respectively,the level of which were significantly higher than the other three groups. The levels of GP73 in chronic hepatitis,cirrhosis and primary liver cancer groups were significantly higher than those in the control group. The levels of GP73 in cirrhosis and primary liver cancer groups were 234.3 ± 91.8 ng/mL and 224.3 ± 83.7 ng/mL,respectively. There was no significant difference between the two groups. The positive rate of GP73 was the highest in the cirrhosis group(71.9%). The sensitivity and specificity of the combined detection of the three indexes were 91.10% and 97.2%,respectively,and the AUC was 0.942,which was higher than that of the single detection index. Conclusion AFP⁃L3% and GP73 should be monitored at the same time when AFP is used as an auxiliary diagnostic index of primary liver cancer. GP73 is of high value in the diagnosis of liver cirrhosis and primary liver cancer. If the three tests are carried out at the same time,the sensitivity rate and accuracy of chronic hepatitis cirrhosis and primary liver cancer can be improved. Thus,the value of early diagnosis of primary liver cancer can be improved,in the prediction of early primary liver cancer has important significance.

Objective To observe the thickness changes of ganglion cell complex in macular area of patients with diabetes,and to analyze its significance in the diagnosis of early diabetic retinopathy. Methods Eighty ⁃three patients(83 eyes)with diabetes were retrospectively analyzed,among whom 50 patients(50 eyes without diabetic retinopathy(NDR)were assigned to NDR group,and the other 33 cases(33 eyes)with mild to moderate non⁃proliferative diabetic retinopathy(NPDR)were assigned to NPDR group. A total of 50 healthy people (50 eyes)were selected as the control group,and the thickness of GCC was detected by optical coherence tomogra⁃ phy(OCT),and the data were analyzed. Results Compared with the control group,the thickness of GCC of S3 N3,S6 and N6 sectors in NDR group were thinner(t = 0.049,0.044,0.040 and 0.049,P < 0.05),while those of S3,N3,S6,N6 and I6 sectors in NPDR group were thinner(t = 0.015,0.016,0.043,0.037,0.042,P < 0.05), and the difference was statistically significant(P < 0.05). Conclusion The thickness of GCC in patients with NDR and NPDR were significantly thinner than those of the healthy people. Early retinopathy in diabetic patients could be found through OCT detection.

Objective To understand the characteristics of HIV infection among young blood donors aged 18 to 25 in this area,It provides a basis for the development of recruitment strategies for blood donors and reduces the risk of HIV transmission through blood transfusion. Methods A total of 2 973 685 samples of unpaid blood donors were tested for HIV by two ELISA reagents and nucleic acid reagents from two different manufacturers and the third party agencies(guangzhou golden field medical inspection center)confirmed by western blot method for HIV positive people. The characteristics of HIV infection among young blood donors aged 18 ~ 25 years were analyzed statistically. Results Among the 2 973 685 blood donors,736 cases were confirmed to be anti ⁃HIV positive. Among the 1 561 660 young blood donors aged 18⁃25,289 were confirmed anti⁃HIV cases,accounting for 39.27% of the total confirmed cases. Among 289 HIV ⁃confirmed young blood donors,males were significantly higher than females(P < 0.01). Other distribution characteristics were as follows:218 patients(75.45%)donated blood for the first time,and 4.15%(4.15%)donated blood for more than 3 times. Street and work groups accounted for 84.43% and 15.56%,respectively;There were 286 cases(98.96%)who donated whole blood,and the other 3 cases were component blood donation.35.99% of the city′ s household registration and 36.68 of the provinces Students and non⁃school blood donors accounted for 24.57% and 60.9%,respectively. Conclusion Among blood donors aged 18⁃25 years in Guangzhou,the risk of HIV infection among male blood donors is much higher than that of female blood donors. Fixed blood donors,group blood donors and school students are the groups with low risk of HIV infection;in the recruitment of blood donation in this region,we should strengthen the health consulta⁃tion on male homosexuality and HIV publicity among young people to ensure the safety of blood for clinical use.

1,25⁃(OH)2D3,the active form of Vit D,plays an important role in airway inflammation and remodeling.due to its anti⁃inflammatory and antifibrosis effects.1,25 ⁃(OH)2D3 plays an anti⁃inflammatory role in airway by reducing inflammatory factors,inducing the release of antimicrobial peptides and binding to Vit D recep⁃ tor.At the same time 1,25⁃(OH)2D3 can reduce airway epithelial fibrosis and affect the proliferation and secretion of airway cells such as airway smooth muscle cells,airway epithelial cells and airway fibroblasts. In addition,1,25 ⁃(OH)2D3 decreased the expression of TGF⁃β1 and IL⁃17A. This article reviews the research of 1,25⁃(OH)2D3 in airway inflammation and airway remodeling.

Extracellular vesicle(EVs)is a kind of vesicle⁃like substance with lipid bilayer structure released by cells into the extracellular environment. Myeloma(MM)is a malignant hematological tumor characterized by abnormal proliferation of clonal plasma cells in bone marrow. As a medium of signal communication between cells,EVs interacts between MM cells and bone marrow microenvironment,which promotes the occurrence and development of the disease. This paper introduces the biological characteristics of EVs and its important effects on immunosuppression,distant metastasis,bone destruction and drug tolerance in patients with MM,and focuses on the latest progress of EVs as a new liquid biopsy marker in the diagnosis and prognosis monitoring of MM,which provides a theoretical basis for further study of EVs in MM.

Colorectal cancer is one of the most common gastrointestinal malignancies. With the develop⁃ ment of diagnosis and treatment,the mortality has decreased,but the prognosis is still poor. At present,chemo⁃ therapy drugs used to treat colorectal cancer generally inhibit tumor growth by inducing cell death However,cell death resistance is the main reason for the unsuccessful treatment and recurrence of gastrointestinal tumors,and most tumor cells are drug⁃resistant due to the dysregulation of apoptotic mechanism. New cell death modes such as necroptosis may offer the possibility for colorectal cancer treatment. Necroptosis is a caspase ⁃independent cellular pattern,usually characterized by necrotic morphology,mainly regulated by RIPK 1 and RIPK 3,and performed by MLKL. Currently,researches on targeted therapy for colorectal cancer based on necroptosis are ongoing,suggest⁃ ing that necroptosis will provide a new strategy for the treatment of colorectal cancer. This review focuses on necrop⁃ tosis and discusses its mechanism of action in the development and treatment of colorectal cancer.
