Pancreatic cancer is a highly malignant tumor of the digestive system,especially metastatic pancreatic cancer is characterized by short course of disease,rapid progression and high mortality. Although gem⁃ citabine has become the standard treatment for advanced pancreatic cancer,the therapeutic effect still falls short of the expected survival. Therefore,a variety of treatments such as radiotherapy and chemotherapy,immunotherapy, targeted therapy and nanotechnology have been gradually developed in clinic,however,the effect of single drug therapy for metastatic pancreatic cancer is not satisfactory at present. Researchers are considering the combination therapy to explore new treatment schemes,and there are many clinical and basic studies of combined immunotherapy for metastatic pancreatic cancer. However,the treatment in this field is still controversial. This article reviews the research progress of combined immunotherapy for metastatic pancreatic cancer in recent years.

Overexpressed NKx2.5 genes in mesenchymal stem cells improve myocardial function in patients with myo⁃ cardial infarction via enhancing SDF⁃1/CXCR4⁃axis homing

Objective To investigate the effect of the overexpressed NKx2.5 genes in bone marrow mesen⁃ chymal stem cells(BMSCs)on cardiac function after myocardial infarction by enhancing the SDF⁃1/CXCR4 axis to promote BMSC homing. Methods Lentivirus was used to overexpress NKx2.5 in BMSCs(BMSCNKx2.5 ). The empty body group,BMSC group,and mouse cardiomyocyte group were used as the control groups. The expressions of SDF⁃1,CXCR4 and NKx2.5 in each group were detected by RT⁃PCR,and the expression of transcription factors NKx2.5,SDF⁃1 and CXCR4 proteins was detected by western⁃blot. BMSCNKx2.5 ,BMSC,and BMSCempty body were injected into the mouse myocardial infarction model through the tail vein. The BMSCNKx2.5 +AMD3100 group was set up. The untreated myocardial infarction group,sham operation group,and normal mouse group were used as the control groups. Changes in the cardiac function of mice with myocardial infarction were detected via Color Dop⁃ pler ultrasound. The expression of NKx2.5,SDF ⁃ 1 and CXCR4 proteins was detected by western ⁃ blot. Results NKx2.5,SDF ⁃1 and CXCR4 in the BMSCNKx2.5 group had the highest expression(P < 0.05). In vivo experiments confirmed that the BMSCNKx2.5 group exhibited the most significant improvement in cardiac function after myocardial infarction(P < 0.05). The expression of transcription factors NKx2.5,SDF⁃1 and CXCR4 in BMSCNKx2.5 group was the highest(P < 0.05). However,when the SDF⁃1⁃CXCR4 axis inhibitor AMD3100 was added,the improvement in cardiac function was insignificant. Conclusion BMSCNKx2.5 can improve myocardial function after myocardial infarction by enhancing the SDF⁃1/CXCR4 axis to promote BMSC homing.

Objective Discuss the clinical value of the automatic dropping in chromosome preparation by comparing the differences between the automatic and manual dropping of amniotic fluid cell chromosomes. Methods The amniotic fluid cells were harvested routinely and automatically. Under the same settings,the amniotic fluid cells were dropped by the metaphase chromosome auto ⁃ spreader and the manual method,respectively. The two methods were compared in terms of mitotic index,ratio of chromosome dispersion area,effective karyotype rate and chromosome band level. Results A total of 47 amniotic fluid specimens were included and dropped into 376 slides. 1066 mitotic phases were analyzed effectively,of which 650 were obtained by metaphase spreader and 416 were obtained manually. As a result,the mitotic index and the quality of the karyotype harvested by the automatic dropping were statistically significantly higher than that of the manual dropping(P < 0.0001). Meanwhile,the degree of chromosome dispersion obtained by the two methods was equivalent,without statistically significant differ⁃ ences between the methods. As for the efficiency,the time by the manual method was significantly shorter than the automatic dropping(P < 0.01). Conclusion The automatic dropping for chromosome preparation is better in quality and stability than the manual method,and thus it is worthy of clinical application.

Objective lncRNA KCNQ1OT1 targets miR⁃129⁃5p to regulate inflammation and apoptosis in a Parkinson disease(PD)cell model. Methods Human SK⁃N⁃SH cells were cultured in vitro,and SK⁃N⁃SH was treated with 1,2,4 mmol/L MPP+ to construct a PD cell injury model. Transfect si⁃NC,si⁃lncRNA KCNQ1OT1 miR⁃NC,miR⁃129⁃5p,si⁃lncRNA KCNQ1OT1 and anti⁃miR⁃NC,si⁃lncRNA KCNQ1OT1 and anti⁃miR⁃129⁃5p into MPP+treated SK,respectively⁃N⁃SH constructs PD cells,denoted as si⁃NC+MPP+group,si⁃lncRNA KCNQ1OT1+ MPP+group,miR ⁃NC+MPP+group,miR ⁃129⁃5p+MPP+group,si ⁃lncRNA KCNQ1OT1+anti ⁃miR ⁃NC+MPP+ group,si⁃lncRNA KCNQ1OT1+anti⁃miR⁃129⁃5p+MPP+group. Results Compared with the NC group,the apoptosis rate,Cleaved⁃caspase⁃3 protein expression,TNF⁃α,IL⁃6,and IFN⁃γ levels in the 1,2 4 mmol/L MPP+ group were significantly increased(P < 0.05). Compared with the NC group,the expression level of lncRNA KCNQ1OT1 in the 1,2,4 mmol/L MPP+ group was significantly increased,and the expression level of miR⁃129⁃5p was significantly decreased(P < 0.05). Compared with the si⁃NC+MPP+ group,the apoptotic rate,Cleaved ⁃caspase ⁃3 protein expression,TNF ⁃α,IL ⁃6,and IFN ⁃ γ levels in the si ⁃lncRNA KCNQ1OT1+MPP+ group were significantly reduced(P < 0.05). Compared with the miR⁃NC+MPP+group,the apoptosis rate,Cleaved⁃ caspase⁃3 protein expression,TNF⁃α,IL⁃6,and IFN⁃γ levels in the miR⁃129⁃5p+MPP+group were significantly reduced(P < 0.05). Compared with the si⁃lncRNA KCNQ1OT1+anti⁃miR⁃NC+MPP+group ,the si⁃lncRNA KCNQ1OT1+anti⁃miR⁃129⁃5p+MPP+group cell apoptosis rate,Cleaved⁃caspase⁃3 protein expression,TNF⁃α,IL⁃ 6. The level of IFN ⁃ γ increased significantly(P < 0.05). Conclusion lncRNA KCNQ1OT1 down ⁃ regulates the expression of miR⁃129⁃5p to inhibit inflammation and apoptosis in PD cell models.

Objective To study the effect of endogenous aromatic ligand ITE(2⁃(1′H⁃indole⁃3′⁃carbonyl)⁃ thiazole ⁃ 4 ⁃ carboxylic acid methylester)on the biological behavior and its mechanism of cervical cancer cells. Methods The expression and localization of AhR in cervical cancer and normal cervical tissues were detected by immunohistochemistry. MTS assay detected cell proliferation and Transwell assay detected cell migration. The qRT⁃ PCR and Western Blot assays were used to detect relative expression in the mRNA and protein of AhR and its downstream genes CYP1A1,CYP1B1. RNA⁃seq and bioinformatics were used to screen genes related to AhR that affect cervical cancer cell proliferation and migration,as well as related signaling pathways. The ability of cervical cancer cells to proliferate and migrate was observed after siRNA knockdown of PKN1. KEGG was used to analyze the enriched pathways related to proliferation and migration,and Western blot was used to detect protein expres⁃ sion in the PI3K⁃AKT pathway after ITE treatment of cervical cancer cells. Results The expression of AhR was higher in cervical cancer tissues than in normal cervical tissues. ITE activated AhR and downstream genes CYP1A1 and CYP1B1 and inhibited the proliferation and migration of cervical cancer cells SiHa and CaSki in a dose⁃ and time⁃dependent manner(P < 0.05). RNA⁃seq technique and bioinformatics screening showed that the expression of PKN1,which is closely related to the proliferation and migration functions of cervical cancer,was down⁃regulatedafter ITE treatment(P < 0.05),and Western blot results showed that PKN1 protein expression was significantly reduced in SiHa and CaSki cervical cancer cell lines after ITE treatmentr(P < 0.05). Proliferation and migration of SiHa and CaSki cells were significantly reduced by knocking down cellular PKN1 expression(P < 0.05). KEGG signal pathway enrichment analysis revealed that the differential expression gene signal pathway was primarily concentrated in the PI3K⁃AKT signal pathway. PI3K⁃AKT pathway inhibitor LY294002 blocked ITE⁃induced in⁃ crease in P⁃AKT levels(P < 0.05),and can reverse the inhibitory effect of ITE on the proliferation and migration of cervical cancer cells(P < 0.05). Conclusion Endogenous aromatic hydrocarbons containing ITE can inhibit the proliferation and invasion of cervical cancer cells,which may be related to PKN1 downregulation and PI3K ⁃ AKT activation.

Effects of dexmedetomidine on oxidative stress and inflammation levels in prostaglandin induced asphyxiat⁃ ed offspring by activating PI3K/AKT signaling pathway and its role in brain protection

Objective To investigate the effects of dexmedetomidine on oxidative stress and inflammation in prostaglandin induced asphyxiated offspring by activating PI3K/AKT signal pathway and explore the mechanism of brain protection as well. Methods Pregnant rats were randomly grouped into model group,dexmedetomidine low⁃dose group,dexmedetomidine high⁃dose group,and dexmedetomidine + LY294002 group,the pregnant rats in the control group gave birth naturally,while the pregnant rats in the model group and the drug administration group were used to construct the asphyxia model of labor. After grouping,12 offspring rats were selected from each group to detect their learning and memory abilities,hippocampal neuron damage and apoptosis,levels of superoxide dismutase(SOD),malondialdehyde(MDA),cyclooxygenase⁃2(COX⁃2)and interleukin⁃6(IL⁃6)in the brain tissue,expression of autophagy and PI3K/AKT pathway⁃related proteins in brain tissue. Results Compared with the control group,the hippocampal neurons of offspring rats in the model group were severely damaged. The apoptosis rate,levels of MDA,COX⁃2 and IL⁃6 in brain tissue,and expression levels of LC3⁃II/LC3⁃I,Beclin⁃1 and P62 were obviously increased(P < 0.05),residence time in the target quadrant,times of crossing the original platform number of neurons,and levels of SOD and p⁃PI3K/PI3K and p⁃Akt/Akt in the brain tissue were obviously decreased(P < 0.05). Compared with the model group,the damage of hippocampal neurons in the dexmedetomidine groups were alleviated. The apoptosis rate,levels of MDA,COX⁃2 and IL⁃6 in brain tissue,and expression levels of LC3⁃ Ⅱ/LC3⁃Ⅰ,Beclin ⁃1 and P62 were all decreased(P < 0.05),residence time in the target quadrant,times of crossing the original platform,number of neurons,and levels of SOD and p⁃PI3K/PI3K and p⁃Akt/Akt in brain tissue were all increased(P < 0.05),and high ⁃ dose dexmedetomidine had an even stronger effect. In addition compared with the high⁃dose dexmedetomidine group,the trend of various indexes in dexmedetomidine + LY294002 group was reversed. Conclusion Dexmedetomidine can reduce prostaglandin⁃induced oxidative stress and inflam⁃ mation,inhibit autophagy,reduce hippocampal neuron damage and apoptosis,and improve learning and memory functions of labor asphyxia neonatal offspring rats by activating PI3K/AKT signaling,and thus it plays a role in brain protection.

Objective To investigate the effect of adductor canal block(ACB)combined with patient⁃con⁃ trolled intravenous analgesia(PCIA)on postoperative depression in patients undergoing total knee arthroplasty. Methods Eighty patients undergoing total knee replacement were randomly divided into ACB plus PCIA group (Group A)and PCIA group(group B),with 40 cases in each group. Group B were given PCIA after operation while group A was given ACB before operation additionally. The Visual Analogue Scale(VAS)score at resting and during movement were recorded at 2 h(T1),6 h(T2),12 h(T3),24 h(T4),72 h(T5)after operation. The use of remedial analgesics after operation and the incidence of nausea and vomiting were recorded. The hospital anxiety and depression scale(HADS)score was recorded before operation,1 day and 3 days after operation. Results The VAS scores at resting and during movement from T1 to T4 in group A were significantly lower than those in group B(P < 0.05). In group A,the use of remedial analgesics and the incidence of nausea and vomiting after operation were significantly reduced(P < 0.05). In group A,the HADS score 1 day after operation and the propor⁃ tion of patients with significant depressive symptoms(HADS⁃D ≥ 8)were significantly lower than those in group B (P < 0.05). There was significant positive correlation between HADS score 1 day after operation and VAS score at resting and during movement from T1 to T4(P < 0.05). There was significant positive correlation between HADS score 3 days after operation and VAS score at resting and during movement from T1 to T3(P < 0.05). Conclusion ACB combined with PCIA is more effective than PCIA alone after total knee arthroplasty,and can reduce the occur⁃ rence of adverse emotions and promote rapid recovery of patients.

Clinical application of 3D printing model for percutaneous transhepatic one⁃step biliary fistulation and rig⁃ id choledochoscopy in patients with intrahepatic cholangiolithiasis

Objective To evaluate the efficacy and safety of a 3D printing model combined with percuta⁃ neous transhepatic one⁃step biliary fistula(PTOBF)and rigid choledochoscopy in the treatment of intrahepatic duct stones in patients with type I bile duct classification. Methods Clinical data of 40 patients with intrahepatic bile duct type I classification were analyzed. In experimental group,17 patients were treated with a 3D printing model combined with PTOBF and rigid choledochoscopy;23 patients in control group underwent simple PTOBF and rigid choledochoscopy. Seven indicators including primary operation time,primary removal rate,incidence of multi ⁃ phase lithotomy,final removal rate,amount of surgical bleeding,channel size,and complications in the two groups were observed and compared. Results The primary and final clearance rates of patients in the experimental group were higher than those in the control group(P < 0.05). There were lower incidence of multi⁃phase lithotomy reduced time of primary operation,less amount of bleeding and main postoperative complications in the experimental group,when compared those in the control group,showing statistical differences(P < 0.05). There was no signifi⁃ cant difference in channel size between the two groups(P > 0.05). Conclusion There are shortened operation time,high stone removal rate and few complications with a 3D printing model combined with PTOBF and rigid choledochoscopy in the treatment of intrahepatic duct stones,which is safe and worth to be popularized in clinical practice.

Objective To study the activation patterns of the cerebral cortex under different accommoda⁃ tive loads of the human ciliary muscles ,the relevant functional brain networks and the effective connections between these brain regions. Methods Thirty⁃three normal subjects were recruited to perform different accommo⁃ dative tasks under MRI. The acquired data from MRI scanning were processed using SPM12 and Group Ⅰ CATv4.0a software packages on MATLAB platform. One⁃sample t⁃test was used to analyze the normal brain activation pattern diagrams. Using the Informax algorithm,the task state data was decomposed into independent spatial components followed by extracting the corresponding components in two dimensions of time and space and analyzing the corre⁃ sponding functional network. Based on dynamic causal modelling,the effective connections between these areas were analyzed. The voxel level threshold P < 0.001 and the FWE cluster level P ≤ 0.05 were used as multiple com⁃ parison correction methods. Results The brain BOLD signal under ⁃3D and ⁃6D accommodative stimulation indi⁃ cated that the neural signals controlling the ciliary muscle were involved in a wide range of cerebral cortex,includ⁃ ing the occipital lobe,temporal lobe,cerebellum,limbic lobe,thalamus,parietal lobe and frontal lobe,and the area and intensity of brain activation under different accommodative loads varied with the different accommodative loads. Based on independent component analysis,task⁃state⁃related activated brain regions in the spatial and tempo⁃ ral dimensions mainly included visual networks,cerebellar networks,and attention networks. Based on DCM,the analysis of the effective connection between these areas discovered two neural circuits involving in the ocular accom⁃ modation,one the circuit of occipital lobe⁃cerebellum⁃parietal lobe and the other the circuit of occipital lobe⁃cere⁃ bellum⁃frontal lobe,with the thalamus acting as the mediator of information transmission. Conclusion Brain pro⁃cesses signals by engaging multiple cerebral regions,involving multiple cerebral networks and two neural circuits. The activated area and intensity vary with different accommodative loads.

Value of neutrophil/lymphocyte ratio and its variation value in predicting risk of acute liver function deteri⁃ oration after interventional treatment of hepatocellular carcinoma

Objective To investigate the predictive value of neutrophil/lymphocyte ratio and its dynamic change on the risk of acute liver function deterioration (ALFD)after transcatheter arterial chemoembolization (TACE)in patients with hepatocellular carcinoma(HCC). Methods Clinical data of 131 patients who met the inclusion criteria in our hospital in 2021 were retrospectively analyzed. The ROC curve was drawn to determine the best cut⁃off point of NLR and △NLR. Univariate and multivariate logistic regression analysis were used to analyze the clinical factors that may affect the risk of ALFD in patients. Results The optimal cut⁃off point of NLR and △NLR for predicting the risk of postoperative ALFD were 3.81 and 4.45,respectively. Univariate analysis showed that BMI(P = 0.022),portal vein tumor thrombus(P = 0.004),number of interventions(P = 0.020),tumor size (P = 0.002),activated partial thromboplastin time(P = 0.036),total bilirubin(P = 0.031),alkaline phosphatase (P = 0.010),AST/ALT(P = 0.029)),NLR(P = 0.042)and ΔNLR(P = 0.039)may be associated with the risk of ALFD. Multivariate analysis showed that portal vein tumor thrombus(OR = 3.221),activated partial prothrombin time(OR = 1.145),NLR(≥ 3.81)(OR = 1.044),and △NLR(≥ 4.45)(OR = 3.159)were independent risk factors for ALFD(all P < 0.05). Patients were divided into four groups according to the cut⁃off value. The risk of ALFD in group C(OR = 3.231,95%CI:1.292 ~ 8.076,P = 0.012)and D(OR = 5.400,95%CI:1.610 ~ 18.113 P = 0.006)was higher than that in group A. Conclusion The combination of NLR ≥ 3.81 and △NLR ≥ 4.45 is an independent predictor of ALFD risk.

Objective To investigate the predictive value of SUMO specific protease 1(SENP⁃1)/hypoxia inducible factor⁃1(HIF⁃1α)pathway on the outcome of cardiovascular events(CVE)in patients with obstructive sleep apnea⁃hypopnea syndrome(OSAHS). Methods Eighty male patients diagnosed as OSAHS in our hospital from January 2018 to March 2021 were selected and divided into mild group(43 patients),moderate group(22 patients)and severe group(15 patients). At the same time,43 normal healthy men who had physical examination in the physical examination center of our hospital were selected as the normal group. In addition,80 OSAHS patients were divided into CVE group(28 cases)and non CVE group(52 cases)according to whether cardiovascular events (CVE)occurred during the 1⁃year follow⁃up period. Clinical baseline data and routine biochemical indexes of patients were collected;serum SENP⁃1,HIF⁃1α and VEGF of SENP⁃1/HIF⁃1α signaling pathway were detected by ELISA assay;multivariate logistic regression was used to analyze the risk factors of CVE in OSAHS patients ROC analysis of serum SENP⁃1,HIF⁃1α and VEGF level to predict on the occurrence of CVE in OSAHS patients. Results Compared with those in the mild group,the body mass index,alanine aminotransferase,glutamic oxalo⁃acetic transaminase,triglyceride,serum creatinine,AHI and Ts90% value in the moderate group and severe group were significantly increased(P < 0.05),and LSpO2 value was significantly decreased(P < 0.05),especially significantly in the severe group(P < 0.05). Compared with that in the mild group,serum SENP⁃1,HIF⁃1α and VEGF level in the moderate group and severe group were significantly higher(P < 0.05),especially higher in the severe group(P < 0.05);serum SENP ⁃1,HIF ⁃1α and VEGF level in CVE group was significantly higher than that in non CVE group(P < 0.05). Multivariate logistic regression analysis showed that serum SENP⁃1,HIF⁃1α and VEGF level were independent risk factors for CVE in OSAHS patients(OR = 1.133,1.090,and 1.066 respectively all P < 0.05). ROC curve analysis showed that serum SENP⁃1,HIF⁃1α and VEGF level had predictive value for the occurrence of CVE in OSAHS patients,and the area under ROC curve was 0.890,0.837,and 0.802 respectively (P < 0.05). Conclusion Serum SENP⁃1,HIF⁃1α and VEGF level are independent risk factors for the occurrence of CVE in OSAHS patients,which can predict the occurrence of CVE in OSAHS patients.

Objective To evaluate the short⁃ and medium⁃term efficacy of drug⁃coated balloon(DCB)in the treatment of arteriovenous fistula stenosis. Methods The clinical data of 43 patients with arteriovenous fistula stenosis treated with endovascular therapy from May 2020 to December 2021 were retrospectively analyzed. Twenty⁃ two patients were treated with DCB(DCB group),and 21 patients with plain angioplasty balloon(PAB group). Target lesion primary patency(TLPP),access circuit primary patency(ACPP),late lumen loss(LLL)and other indicators were compared between the two groups at 3,6,12 months after operation. Results The operation was successful in all cases. There was no significant difference in TLPP between the two groups 3 months after opera⁃ tion. The TLPP of DCB group was significantly higher than that of PAB group 6 and 12 months after operation [(81.8% vs. 47.6%),(54.5% vs. 23.8%),both P < 0.05)];there was no significant difference in ACPP between the two groups 3,6,12 months after operation;LLL 6 and 12 months after operation in DCB group was significant⁃ ly lower than that in PAB group(P < 0.05). Conclusion DCB has a better clinical efficacy and significant advan⁃ tage over plain angioplasty balloon in the treatment of arteriovenous fistula.

Objective To screen the prognosis ⁃ related gene SLC5A12 of esophageal squamous cell carcinoma by protein profiling and explore its effect on the malignant phenotype of ESCC cells. Methods Protein profiling was performed to screen for differentially expressed proteins in ESCC,and public databases were used to screen for SLC5A12,a gene associated with prognosis in ESCC,and to analyze the relationship between SLC5A12 expression and prognosis in ESCC patients in the database. Immunohistochemical staining of tissue microarrays made from paired tumor and normal tissues of 117 ESCC patients with perfect follow⁃up information in the biospeci⁃ men database was performed to analyze the expression of SLC5A12 in ESCC and its relationship with the clinicopa⁃ thology and prognosis of patients. Small interfering RNA knocked down the expression of SLC5A12 in ESCC cells, and the proliferation,invasion and migration ability of the cells were detected. Results A total of 235 protein expressions were upregulated and 362 protein expressions were downregulated in ESCC,in which SLC5A12 expres⁃ sion was significantly higher than that in normal tissues(log2 fold = 1.33,= 0.0004). SLC5A12 expression in ESCC patients in TCGA was significantly higher than that in normal tissues and esophageal adenocarcinoma,and N stage and SLC5A12 expression were risk factors for patient survival factors,and SLC5A12 expression was signifi⁃ cantly associated with overall survival[HR = 2.74,95%CI:1.27 ~ 5.94,= 0.012]and disease⁃specific survival [HR = 3.14,95%CI:1.24 ~ 7.95,= 0.02]in ESCC patients. Immunohistochemical staining of tissue microarraysconfirmed that SLC5A12 expression was significantly higher in ESCC than in normal tissues and significantly higher in patients with lymph node metastases than in those without metastases. SLC5A12 expression levels were signifi⁃ cantly correlated with overall survival(< 0.000 1),pathological stage(< 0.001)and lymph node metastases (< 0.001)in ESCC patients. The proliferation,invasion and migration ability of ESCC cells were significantly reduced after knockdown of SLC5A12 expression. Conclusions SLC5A12 expression in ESCC was significantly higher than that in normal tissues and significantly correlated with the prognosis,pathological stage and lymph node metastasis of patients,and SLC5A12 could promote the malignant phenotype progression of ESCC cells.

Objective To investigate the relationship between internal carotid artery wall shear stress (WSS)and relapse in patients with acute ischemic cerebrovascular disease complicated by internal carotid athero⁃ sclerotic plaque. Methods Ninety ⁃ eight patients with atherosclerotic plaques in internal carotid artery who had been hospitalized in the Second Affiliated Hospital of Guangzhou Medical University from January 2018 to December 2020 were analyzed. The wall shear stress was calculated and the relapse was recorded during a follow ⁃up of two years. Results Left WSS(WSS⁃L),right WSS(WSS⁃R)and mean WSS(WSS⁃M)in the relapse group were higher than those in the control group(10.16 ± 5.02 vs. 7.82 ± 2.50,10.22 ± 2.09 vs. 7.82 ± 2.82,and 10.19 ± 3.16 vs. 7.82 ± 2.24)dyne/cm2 ,and the WBC count in the relapse group was higher than that in the control group (8.67 ± 1.67 vs. 6.35 ± 1.93)× 109 /L,the difference was statistically significant(P = 0.039,0.015,0.008,and 0.001). Relapse was positively correlated with WSS⁃R(r = 0.372,P = 0.007),WSS⁃M(r = 0.305,P = 0.029), and WBC count(r = 0.466,P = 0.001). The predictive value of WSS ⁃M for relapse was analyzed by the ROC curve analysis. As WSS ⁃M was 7.95,the maximum sensitivity and specificity were 80% and 56%. Conclusions Internal carotid artery WSS is related to the relapse in the patients with acute ischemic cerebrovascular disease com⁃ plicated by atherosclerotic plaques.

Objective To compare the clinical efficacy and safety differences between posaconazole and voriconazole for the prevention of fungal infections after allogeneic hematopoietic stem cell transplantation(allo ⁃ HSCT). Methods Retrospective analysis of 104 patients(54 in the posaconazole group and 50 in the voricon⁃ azole group)who underwent allo ⁃ HSCT and applied posaconazole or voriconazole for fungal prophylaxis at our hospital from June 2019 to June 2021,comparing the incidence of IFI,the incidence of adverse drug reactions and other related indicators within 180 days after transplantation between the two groups. Results The incidence of IFI in the posaconazole and voriconazole groups was 7.4%(4 cases)and 20.0%(10 cases),respectively,with no statistical difference(P = 0.085);in patients at high risk of IFI,the incidence of IFI in the two groups was 10.0% (3 cases)and 34.9%(9 cases),with a statistical difference(P = 0.047);the incidence of adverse drug reactions in the two groups was 7.4%(4 cases)and 18.0%(9 cases),with no statistical difference(P = 0.091). Conclusion Compared with voriconazole,posaconazole can further reduce the incidence of IFD in high⁃risk patients after trans⁃ plantation without increasing the incidence of drug⁃related adverse reactions.

Objective To investigate the efficacy and dose selection of glucocorticoid in severe drug⁃ induced liver injury. Methods Data of 232 patients with severe drug ⁃induced liver injury who admitted to The First Affiliated Hospital of Zhengzhou University hospital from September 2018 to August 2021 were collected. According to the use of glucocorticoid and their dose during the course of disease,they were divided into hormone group(further divided into < 0.5 mg/(kg·d)group and ≥ 0.5 mg/(kg·d)group)and control group. The efficacy of ALT,AST,DBIL,IBIL,TBIL and complication were analyzed,between hormone group and control group,different dose of hormone group. Results (1)The mean hospital stay between the hormone group and the control group was statistically different(P < 0.05),the number of days of hospitalization in the hormone group was 18.00(13.00 24.00)days,and that in the control group was 21.00(15.00,29.00)days. There was statistical difference between two group in the time required for the reduction of AST,DBIL,IBIL and TBIL by 30% and 50% compared with the baseline value(P < 0.05). There was no statistical difference between two group in the time required for the reduction of ALT by 30% and 50% compared with the baseline value(P > 0.05).(2)The days of ALT,AST DBIL,IBIL and TBIL decrased by 30%,50% compared with the baseline value in the different doses of hormone group was no statistically difference(P > 0.05).(3)There were statistically difference in infection⁃related adverse reactions between the hormone group and the control group,and between different doses of hormone group(P < 0.05). Conclusion Glucocorticoid combined with conventional liver ⁃ protecting drugs can improve the levels of AST DBIL,IBIL and TBIL in the patients with severe drug⁃induced liver injury. There was no statistical difference in the improved of ALT,AST and TBIL with different doses of glucocorticoid,but when the dose of hormone group was ≥ 0.5 mg/(kg·d),the incidence of infection was higher.

Objective To verify the significance of transabdominal ultrasound measurement of transverse rectal diameter(therectumdiameterRD)in the diagnosis of cystorectal dysfunction(BBD)in children,and to explore the value of clinical interpretation combined with ultrasound imaging in the selection of diagnosis and treat⁃ ment. Methods A total of 167 children with suspected BBD in the urology clinic of our hospital from July 2020 to April 2022 were selected as the observation group,all of them had a history of abnormal urination and defecation and the abnormalities of body anatomy and nervous system were excluded. 46 parents with obvious symptoms were selected to fill in the BBD questionnaire. 172 healthy children in the same period were selected as the control group,all of them had no abnormal urination and defecation and abnormal body structure. The RD of the posterior bladder was measured through the abdominal wall in two groups of children by ultrasound. Results The average RD of the observation group was(34.14 ±7.11)mm(95%CI:33.05 ~ 35.22),and that of the control group was (23.74 ±8.84)mm(95%CI:22.41 ~ 25.07). The average level of RD in the observation group was significantly higher than that in the control group(P < 0.05). In the observation group,there was no significant difference in average RD between children with questionnaire and children without questionnaire. In the choice of treatment there was no significant difference in the choice of final treatment between the diagnostic method of clinical interpre⁃ tation + BBD questionnaire + ultrasonic RD measurement and the diagnostic method of clinical interpretation + ultrasonic RD measurement only. Conclusion Transabdominal ultrasound RD measurement provides a more intuitive and accurate physical parameter for the diagnosis of BBD in children ,and has significant clinical application value. Clinical interpretation combined with ultrasonic RD measurement is a more accurate and simplified method for BBD diagnosis.

Objective To investigate the current status and influential factors of basic psychological needs satisfaction of medical postgraduates ,and improve the mental health of medical postgraduates from the perspective of positive psychology. Methods 399 medical postgraduates from a university in Guangzhou were selected by convenience sampling. A general information questionnaire,the Basic Needs Satisfaction(BNSG ⁃ S)in General Scale and Pittsburgh Sleep Quality Index were used. Results The total score of BNSG⁃S was 98.27 ± 16.01,which is in the upper middle level. The multivariate analysis of the above variables showed that the older the postgradu⁃ ates were,the higher the sleep quality level was,and the higher the work⁃life balance of postgraduates was,the higher the psychological needs were met(< 0.05). Conclusion The satisfaction level of basic psychological needs of medical postgraduates in Guangzhou was in the upper middle level,and age,sleep quality and work⁃life balance had significant impact on the satisfaction level of basic psychological needs. The student management department of the university should focus on the above factors,formulate corresponding measures early,and im⁃ prove the satisfaction of the basic psychological needs of medical postgraduates to improve their mental health.

Objective To objectively evaluate the effect of anti vascular endothelial growth factor (VEGF)therapy combined withdifferentiation and treatment from dampness on the morphology of macular edema in retinal vein occlusion(RVO)in the real world. Methods The real world study included the patients with macular edema due to retinal vein occlusion of dampness syndrome who were treated in hospital from July 2020 to December 2021. They were divided into the observation group(differentiation and treatment from dampness combined with anti VEGF treatment)and the control group(anti VEGF treatment). After 3 months of treatment,the retinal thick⁃ ness(CMT),intraretinal fluid(IRF)height and subretinal fluid(SRF)height of macular fovea were observed before and after treatment. Results CMT,IRF and SRF in the observation group were lower than those before treatment,while CMT,IRF and SRF in the control group were lower than those before treatment. After 3 months of follow⁃up,CMT and IRF in the observation group were lower than those in the control group,with a statistical difference(P < 0.05). Conclusion The combination of anti VEGF therapy and differentiation and treatment from dampness can effectively improve the morphology of retinal vein macular edema of dampness syndrome,especially reduce the retinal fluid more significantly.

Objective Observe the clinical curative effects for the treatment of degenerative meniscus injury by self⁃made decoction of activating blood and dieresis combining with knee arthroscopy. Methods From February 2018 to February 2020,60 patients with degenerative meniscus injuries who will undergo knee arthroscopy at Danyang Hospital of Traditional Chinese Medicine will be divided into two groups:control and treatment,with 30 patients in each. After the knee art hroscopy,the control group received the treatment of anticoagulation and fluid replacement;while based on the treatment of anticoagulation and fluid replacement,the treatment group also orally took medicine of activating blood and dieresis prepared the hospital for 4 weeks continuously,the follow⁃up survey took 6 months. Postoperative curative effects,anticoagulation index,swelling value,changes in Lyscholm knee score scale,and VAS score are compared for 2 weeks before and after the operation,as well as 6 months after the operation. Results The total effective rate of the treatment group is higher thanthat of the control group(P < 0.05); two groups of anticoagulation index one week after the operation(PT,APTT)are postponed than the pre⁃operation ones,and duration of the treatment group is longer than the control group(P < 0.05);two groups of swelling value were reduced continuously on the 3rd and 7th day after the operation than the first day after the operation(P < 0.05). On the 3rd and 7th day after the operation,the value of treatment group is lower than those of the control group(P < 0.05);two groups of Lyscholm knee score scale and VAS score(2 weeks and 4 weeks after the opera⁃ tion,6 months after the operation)are obviously better than the value before the treatment,the differences have statistical significance,and treatment group(2 weeks and 4 weeks after the operation)is better than the control group.(P < 0.05). Conclusion The treatment of degenerative meniscus injury by means of knee arthroscopy is effective,the application of activating blood and dieresis can effectively reduce the early swelling postoperative of knee arthroscopy,improve coagulation function and promote recovery of knee joint function.

Kidney stone is one of the most common diseases in urology and it is reported with high incidence and recurrence rates,which results in heavy healthy and economic burden to individuals and societies. Mechanism underlying the stone formation has not been fully understood,so the current prevention from kidney stone disease is not satisfactory. microRNA is a kind of endogenous non coding single stranded RNA with a length of approximately 22 nucleotides. It can regulate the post transcriptional translation of various genes via promoting the degradation or inhibiting the translation of target mRNAs. MicroRNAs have been reported to participate in the pathogenesis of many diseases. In recent years,more and more studies report that microRNAs play an important role in the kidney stone formation,which may provide some new biomarkers and therapeutic targets for nephrolithiasis. This study concluded the role of microRNAs in kidney stone disease by reviewing the relevant literature on the disease,which aimed to improve the cognition about diagnostic and therapeutic value of kidney stone related⁃microRNAs.

Atrial fibrillation is a common arrhythmia in clinic,and its main hazard is stroke. At present,long⁃term oral anticoagulants are needed to prevent stroke in patients with medium and high risk of nonvalvular atri⁃al fibrillation. But relevant research data show that the medication therapy is ineffective or subject to contraindica⁃tions in nearly half of patients with atrial fibrillation. In recent years,the development of left atrial appendage clo⁃sure has effectively solved such problems.
