Reasonable application of medications for primary/secondary prevention and treatment is ofgreat significance in the treatment of coronary heart disease. Recent studies have shown that nicorandil has compara⁃ble efficacy to classic anti⁃angina drugs in the treatment of coronary heart disease. Nicorandil is recommended as animportant therapeutic drug for relieving angina pectoris or myocardial ischemia in Chinese Coronary Heart DiseaseMedication Guidelines published in June 2018 by Committee of Experts of Rational Use of Medicine of NationalHealth and Family Planning Commission of P.R.China. Based on the guidelines,this article reviews the currentprogress of nicorandil in the treatment of coronary heart disease and its application prospect,and provides refer⁃ence for the rational use of medicine in the treatment of coronary heart disease.

Coronavirus disease 2019(COVID⁃19)caused a global pandemic and seriously endangeredhuman health. With the deepening of research,the abnormal coagulation function and venous thromboembolism(VTE)complications in COVID⁃19 patients have attracted the attention of many researchers. We review,in thisarticle,the clinical value of elevated D⁃dimer in COVID⁃19 patients and related research on VTE and summarizethe possible mechanism of thrombosis induced by SARS⁃CoV⁃2,hoping to provide reference for the prevention ofthrombosis in COVID⁃19 patients.

Objective To investigate the predictive value of neutrophil to lymphocyte ratio on the prognosisof COVID⁃2019. Methods Twenty patients with SARS⁃coV⁃2 infection treated in Xiangyang NO.1 People′s Hospitalaffiliated to Hubei University of Medical from February 2020 to March 2020 were retrospectively analyzed. Theywere divided into moderate group(including normal and light 10 cases)and severe group(including severe andcritical 10 cases),according to clinical characteristics and conditions. Laboratory tests and flow cytometry resultswere collected on the day after admission,1 week after admission and leaving hospital,and calculated the neutro⁃phn to lymphocyte ratio(NLR),ΔNLR1(NLR of 1 week after admission⁃NLR of admission),ΔNLR2(NLR ofleaving hospital ⁃ NLR of 1 week after admission),compared the differences between the two groups. Flow cytome⁃try was used to detect the scattered distribution of lymphocytes,neutrophils and monocytes in peripheral blood ofthe two groups of patients,the positive rate of HLA⁃DR on T cells and CD15 on the surface of neutrophils as amarker of activation. Results There was a significant change in the NLR of the two groups during treatment. TheNLR of the severe group increased more significantly at 1 week of treatment. When the patients were cured anddischarged,the NLR of both groups decreased significantly. The frequency of CD15+ neutrophils in the moderategroup was relatively lower than that in the severe group,and there was no significant fluctuation during treatment.The frequency of CD15+ neutrophils in the severe group increased significantly during 1 week of treatment,anddecreased significantly when cured and discharged;The frequency of HLA⁃DR+ T cells in both groups had a dynamicand significant change during the treatment period,which increased significantly at 1 week of treatment,anddecreased significantly when cured and discharged,and there were differences between the groups at each timepoint of detection. Conclusion Dynamic monitoring of NLR,CD15+ neutrophil frequency,and HLA⁃DR+ T cellfrequency can reflect the condition and prognosis of COVID⁃2019.

Objective To analyze ultrasonic monitoring data of severe COVID⁃19 patients in ICU,and toexplore the application value and characteristics of ultrasound. Methods The ultrasonic data of chest,heartand blood vesselsof severe and critical COVID⁃19 patients in the First Affiliated Hospital of Guangzhou MedicalUniversity were recorded;the ultrasound demand and positive rate of patients during the treatment were statisticallyanalyzed. The monitoring value of ultrasound was analyzed. Results Sixteenpatients with 318 times of ultrasoundwere included in this studyand the clinical needs of ultrasound examination in an ascending order were heart,vein,pleural effusion and digestive system. The positive rate of ultrasound was 74% in heart,60% in lower limbvein,59% in internal jugular vein,77% in digestive tract and 66% in hydrothorax.The positive rate of cardiovascu⁃lar and abdominal ultrasound examination in patients with ECMO was significantly higher than that in those without(P < 0.05). Conclusion The demand and positive rate of ultrasound in the treatment of severe and critical COV⁃ID⁃19 patients in ICU are relatively high. Moreover,ultrasound can reflect the severity of the disease.

Objective To explore the effect of intrathecal injection of adeno⁃associated virus(AAV)⁃mediated brain⁃derived neurotrophin factor(BDNF)in spinal nerve ligation(SNL)rats on neuropathic pain andits possible mechanism. Methods Fifty⁃six SD rats were randomly divided into normal,SNL,normal saline(NS),BDNF⁃free AAV(AAV0),AAV⁃BDNF(BDNF),AAV⁃BDNF +potassium chloride cotransporter 2(KCC2)antagonist(BDNF + Furos)and AAV⁃BDNF + NS(BDNF+NS)groups. Mechanical withdrawal thresholds(MWTs)and thermal withdrawal latencys(TWLs)of the rats in each group were measured before modeling and 0,3,7,14,and 21 days after intrathecal injection. The expression of BDNF and KCC2 in the spinal dorsal horn was detectedby Western Blot after test. Results Compared with the normal group,the MWTs and TWLs of other groupsdecreased significantly after modeling(P < 0.001). As compared with the SNL,NS and AAV0 groups,the MWTand TWL of the BDNF group increased at 14 and 21 days after injection(P < 0.001). After intrathecal injection,the MWT and TWL of the BDNF+Furos group was significantly lower than those of the BDNF and BDNF+NSgroups(P < 0.001). At 21 days after injection,the expression of BDNF and KCC2 in the BDNF group was signifi⁃cantly higher than those in SNL,NS and AAV0 groups(P < 0.001). The expression of KCC2 in the BDNF+Furosgroup decreased compared to those in the BDNF and BDNF+NS groups(P < 0.001). Conclusion The intrathecalexpression of BDNF mediated by AAV can play an analgesic role in SNL rats,which is related to the upregulationof KCC2.

Objective To investigate the protective effect of mTOR inhibitor Rapamycin(RAPA)on post⁃epileptic brain injury and the regulation mechanism of PI3K/AKt signaling pathway. Methods 40 male Sprague⁃Dawley(SD)rats were randomly divided into 4 groups:control group,KA group,KA+RAPA 5 mg/kg group andKA+RAPA 10 mg/kg group. A model of epilepsy in rats was made by intraperitoneal injection of kanic acid(KA).The control group was injected with normal saline only. RAPA was administered intraperitoneally 2 h beforeKA⁃induced epilepsy. Rats were sacrificed 24 h after epilepsy and the cerebral cortex was removed. Western blotand qRT⁃ PCR was used to detect PI3K,AKt and mTOR protein and mRNA expression,Immunohistochemicalstaining was used to observe microglia activation;Western blot was used to detect the protein expression of NF⁃kBand IL⁃6 in the inflammation pathway. Results The protein and mRNA levels of PI3K、AKt and mTOR,theprotein levels of NF⁃kB and IL⁃6,microglia activation significantly increased after KA⁃induced epilepsy,but theseeffects were inhibited by RAPA treatment. Further analysis found the effect of RAPA dose 10 mg/kg is more obvious.Conclusions The mTOR inhibitor RAPA can decrease the PI3K/AKt signaling pathway,microglia activation andreduce inflammatory response after epilepsy,thus protecting brain damage after epilepsy.

Objective To investigate the effects of bone sca⁃1 marrow mesenchymal stem cells on acutelung injury after lung transplantation. Methods 24 adult Sprague⁃Dawley rats,weighing 250 ~ 300 g,wererandomly allocated to 3 groups(n = 8)using a random number table. Rats in group C without any treatment wereblank control group(n = 8). Rats in group M underwent autologous left lung orthotopic transplantation were modelgroup(n = 8). Rats in group B were injected with bone marrow sca⁃1 mesenchymal stem cells via the caudal veinafter autologous left lung orthotopic transplantation. After 48 h,rat arterial blood was analyzed and lung tissue sam⁃ples were taken for moisture content and pathological examination. ELISA was used to detect H2O2,MDA,IL⁃6,andTNF⁃α levels in lung tissues. Western blot was used to detect the expression levels of type Ⅱ alveolar epithelial cellmarker SPB and airway mucosal epithelial cell marker CC10 in lung tissues. Results Compared with group C,PaO2in the arterial blood was decreased,the bleeding and congestion of lung tissue were serious,the infiltration of inflam⁃matory cells was increased,the pulmonary edema was serious,the concentrations of H2O2,MDA,IL⁃6,and TNF⁃αwere significantly increased in lung tissues(P < 0.05). Expression levels of type II alveolar epithelial cell markerSPB and airway mucosal epithelial cell marker CC10 were down⁃regulated in group M. Compared with group M,PaO2in the arterial blood was increased,the bleeding and congestion of lung tissue were mild,the infiltration of inflamma⁃tory cells was decreased,the pulmonary edema was mild,the concentrations of H2O2,MDA,IL⁃6,and TNF⁃α weresignificantly decreased in lung tissues(P < 0.05),the expression levels of type Ⅱ alveolar epithelial cell markerSPB and airway mucosal epithelial cell marker CC10 were up⁃egulated in group B. Conclusion Bone marrow sca⁃1mesenchymal stem cells can alleviate acute lung injury after lung transplantation by inhibiting inflammatoryresponse and oxidative damage,repairing damaged epithelial cells and improving lung function.

Objective To study the inhibitional effect of the icariin Ⅱ on epithelial⁃esenchymal transition(EMT)in tongue squamous cell carcinoma(TSCC)and explore its regulation through Notch1/PTEN/FAK signalingpathway. Methods Tca⁃8113 cells in logarithmic phase were randomly divided into blank group and ICSⅡlow,middle,high⁃dose groups. The comparisons were made among the groups in terms of cell proliferation inhibitionrate,cell migration,intervention ability,relative expression of Notch1,PTEN,FAK,p⁃FAK protein of eachgroup. Results The proliferation inhibition rate of ICSⅡ low,medium,high⁃dose groups showed an upward trendat 24,48 and 72 hours(< 0.05). The mobility and the number of transmembrane cells per field,Notch1 proteinrelative expression and p⁃FAK/FAK of the blank group,ICSⅡ low,medium,high⁃dose groups showed a down⁃ward trend(< 0.05),of which the PTEN protein relative expression showed an increasing trend(< 0.05).Conclusion ICSⅡ can inhibit TSCC EMT in a dose⁃ependent manner,which is speculated to be related to thedown⁃regulation of Notch1 expression,up⁃regulation of PTEN expression,and inhibition of FAK phosphorylation.

Objective To construct Acinetobacter baumannii SmpA/Omp22/NlpA(1 ⁃ 128 trivalent fusionprotein and investigate its immunogenicity. Methods The smpA/omp22/nlpA(1⁃384 fusion gene was obtained byartificial synthesis and PCR amplification,the fusion gene fragment was cloned into plasmid pColdI and trans⁃formed into E.coli BL21 for prokaryotic expression. BALB/c mice were inoculated subcutaneously with the purifiedrecombinant fusion protein formulated with Freund′s adjuvant three times. At the same time,the adjuvant controlgroup and the PBS control group were set up. Blood was collected from the mice orbital canthus on day 7 and 21after the last immunization,and the serum antibody titers were detected by ELISA. Spleen lymphocytes were isolatedand stimulated by the recombinant fusion protein in vitro,the proliferation of lymphocytes and the release level ofIFN⁃γ and IL⁃ were detected. Results A 5.9 kb recombinant plasmid was constructed and identified. The recom⁃binant plasmid can express a recombinant fusion protein with molecular weight about 56 kDa. Antigen specific anti⁃body IgG was detected in the serum of the recombinant fusion protein immunized mice. The proliferative activity ofspleen lymphocyte from the recombinant fusion protein immunized mice was significantly higher than that from theadjuvant and PBS control group(P < 0.001). The IL⁃4 release level of the spleen lymphocytes from the recombinantfusion protein immunized mice was significantly higher than that from the control group(P < 0.01),while the IFN⁃γrelease level showed no significantly difference. Conclusion The SmpA/Omp22/NlpA(1⁃128 fusion protein of Aci⁃netobacter baumannii was successfully prepared and showed strong immunogenicity after mice immunization.

Objective To analyze gonadotropin level,lipid metabolism,vitamin D and insulin resistanceindex of girls with idiopathic central precocious puberty,and analyze the correlation of these indexes with bodymass. Methods A total of 111 cases of girls admitted to our hospital with precocious puberty were selected anddivided into control group consisting of 51 patients with simple early breast development and observation group of60 cases with idiopathic central precocious puberty. Height,weight,secondary sex characteristic,the determinationof fasting plasma glucose(FPG),fasting insulin(FINS),triglyceride(TG),cholesterol(CHO),insulin⁃likegrowth factor 1(IGF⁃1),25⁃(OH)D3,luteinizing hormone(LH),follicle stimulating hormone(FSH),FSH peaklevel,LH peak level,and calculate the body mass index(BMI),insulin resistance index(HOME⁃IR),LH peak/FSH peak were tested and analyzed. The differences of gonadotropin level,lipid metabolism,vitamin D,HOME⁃IRbetween two groups were compared,and the correlation analysis of the above indexes was also conducted. Results In the observation group,BMI was positively correlated with LH ,LH peak level /FSH peak level(r = 0.476 and0.470 respectively,P < 0.05),and negatively with vitamin D(r = -0.290,P < 0.05). IGF⁃1 was positively correlatedwith LH,TG,LH peak value,LH/FSH,IHOME⁃IR and BMI(r = 0.553,0.355,0.351,0.459,0.531 and 0.330respectively,P<0.05). LH was positively correlated with HOME⁃IR,BMI,IGF⁃1(r = 0.320,0.476 and 0.553respectively,P<0.05),and negatively with vitamin D(r = -0.266,P<0.05). HOME⁃IR was positively correlatedwith LH,TG,LH peak level /FSH peak level and IGF⁃1(r = 0.320,0.298,0.430 and 0.531 respectively,P<0.05). Vitamin D deficiency was found in 39 of 51 girls in the control group and in 46 of 60 girls in the observation group. Conclusion Glucolipid metabolism in girls with idiopathic central precocious puberty may be affected bysex hormone levels. Vitamin D deficiency and increased BMI are important factors for girls in their early sexualdevelopment.

Objective To compare the therapeutic effects of conventional dose of sacubitril⁃valsartan(ARNI)and target dose of ACEI in patients with heart failure with reduced ejection fraction(HFrEF). Methods Totally 223 patients with heart failure diagnosed as dilated cardiomyopathy were continuously included in our hospital,of which 111 patients were treated with ARNI (experimental group and 112 with ACEI standard treatment(control group),in addition to the treatments followed the guidelines. Follow⁃up treatment for 18 months wasconducted. Results The average dose of ARNI was 138.96 mg,and 1.8% of the patients achieved target dose.The target dose reached in 47.3% of the patients in the control group. The clinical efficiency,6⁃minute walking testand SBP decrease in the experimental group were all higher than those in the control group,and BNP and MLHFQscores,readmitted time and times were all lower than those in the control group(P < 0.05). Conclusions In thereal world treatment of HFrEF patients in our hospital,a very small number of patients achieve the target dose ofARNI. However,the effect of frequently⁃used dose of ARNI is not inferior to that of target dose of ACEI in terms ofinhibiting ventricular remodeling,while the improvement of quality of life is even obvious.

Objective The treatment of degenerative lumbar spinal stenosis by minimally invasive posteriorspinal decompression and fusion under electron microscope was compared with that of traditional posterior spinaldecompression and fusion. Methods A total of 110 patients with degenerative lumbar spinal stenosis who underwentfusion surgery in our hospital from March 2017 to March 2019 were selected. There were 58 patients who underwentminimally invasive posterior spinal decompression and fusion guided by electron microscope and selected as theminimally invasive group,Other 52 patients who underwent traditional posterior spinal decompression and fusionwere selected as the traditional group. The surgical indicators,postoperative hospital stay,postoperative complica⁃tions,visual analog score(VAS)and Oswestry dysfunction index(ODI)scores of the two groups were comparedand analyzed. The MacNab standard was used at the last postoperative period of follow⁃up to evaluate the clinicaleffects. Results Compared with traditional operation group,the operation time,intraoperative blood loss,surgi⁃cal incision length,postoperative hospital stay,and postoperative complications of the minimally invasive groupwere significantly shorter or less(P < 0.05). The VAS score and ODI score of the two groups were all significantlyimproved after treatment(P < 0.05)but there was no significant difference in VAS score and ODI score betweentwo groups(P > 0.05). Conclusion Compared with traditional decompression and fusion,electron microscope⁃guided minimally invasive posterior approach decompression and fusion not only guarantees the curative effect butalso has many advantages for the treatment of degenerative lumbar spinal stenosis. The advantages includes lesstrauma,less bleeding,shorter operative time and hospital stay,and lower postoperative complications. It is worthyof clinical preference.

Objective To compare the effect of three different surgical approaches for intramedullarynailing for tibia fractures. Methods A retrospective analysis was conducted of the 99 patients with tibial fracturewho had been treated from January 2018 to 2019 and completed follow⁃ups,there were 69 males and 30 females,with an average age of 47 years(range from 18 to 65 years),33 were treated through suprapatellar approach,30 weretreated through infrapatellar approach and 36 were treated through parapatellar approach,The operative time,bloodloss,times of X⁃ray exposure,length of hospital stay,fracture healing time,lysholm scoring,anterior knee pain,removal time of internal fixation,complication rate and postoperative excellence rate were recorded and comparedamong the three groups. The data were analyzed by SPSS22.0. Results In the parapatellar approach group,theoperative time,blood loss,times of X⁃ray exposure,length of hospital stay and anterior knee pain were shorter,thelysholm scoring was highest. The difference was statistically significant(P<0.05). There were no significantlydifference in the postoperative excellence rate,complication rate,blood loss and removal time of internal fixation(P>0.05). Conclusion Different surgical approach are factors related to treatment of tibial fracture. The parapatellarapproach for tibial fractures doesn′t injury of ligamentumpatellae and patellofemoral,it also has the advantages ofshorter operative time,less times of X⁃ray exposure,less pain and fewer complications,may be choosed firstly

Objective To explore the effect of pulse irrigation combined with tranexamic acid on periopera⁃tive blood loss,coagulation function and hip joint functionin elderly patients with intertrochanteric fracture undergo⁃ing artificial femoral head replacement. Methods A retrospective study was performed in 67 patients with intertro⁃chanteric fracture undergoing artificial femoral head replacement. The patients were randomly divided into threegroups before surgery. Tranexamic acid was diluted by 20 mg/kg to 250 mL is osmotic saline one hour before surgery.In group A,patients received 200ml intravenous drip,intraoperative routine irrigation,and 50 mL tranexamic acidsolution was injected into hip cavity after skin suture. In group B,pulse irrigation was changed intraoperatively. Ingroup C,routine intraoperative irrigation was performed without tranexamic acid. Blood loss,postoperative coagula⁃tion function and hip function of the three groups were compared. Results There was no difference in the dominantblood loss between group A and C(P > 0.05),and the dominant blood loss in group B was less than that in group C(P < 0.05). The total and hidden blood loss of group A and B were less than that of group C(P < 0.05),and that ofgroup B was less than that of group A(P < 0.05). Blood coagulation was increased in group A when compared withthat in group C(P < 0.05),while it was not increased in group B(P > 0.05). Harris score of hip joint in group A andGroup B was higher than that in group C one month after surgery(P < 0.05),and the score in group B was higherthan that in group A(P < 0.05). Conclusion In elderly patients with intertrochanteric fractures,pulse irrigationcombined with tranexamic acid during artificial femoral head replacement can significantly reduce perioperativeblood loss without increasing blood clotting,which is conducive to the early and rapid recovery of the patients.

Objective To investigate the effect of patient compliance on the efficacy of neoadjuvant therapyfor colorectal cancer based on the data from Database from Colorectal Cancer(DACCA). Method The version ofDACCA was updated on Nov.14th,2019. We extracted the structured data of 328 patients with colorectal cancerwho received neoadjuvant therapy,and patients were divided into complete compliance group(n = 154)and partialcompliance group(n = 174). A 1∶1 propensity score matching(PSM)was conducted according to basic data of thepatients,neoadjuvant therapy,patient compliance and other baseline data,and 88 cases were successfullymatched in each group. The efficacy of neoadjuvant therapy treatment was compared and analyzed between the twogroups. Results Before PSM,The compliance of the two groups of colorectal cancer patients was not associatedwith BMI,constitution,tumor location and clinical stage,(P > 0.05),but with age,gender and neoadjuvant therapytreatment cycle(P < 0.05). Additionally,the degree of compliance positively correlated with neoadjuvant therapytreatment cycle(rs = 0.366,P < 0.001),but negatively with age(rs = -0.151,P = 0.006). After PSM,there wasno statistically significant difference in clinical response degree between the two groups(P = 0.532),and in tumorregression grade(TRG)between the two groups(P = 0.612). Conclusions For patients with colorectal cancerreceiving neoadjuvant therapy,patient compliance cannot yet be used as a powerful indicator for predicting or eval⁃uating the efficacy of neoadjuvant therapy.

Objective To explore the clinical application value of simple puncture endoscopic monitoringtube placement method in PELD. Methods A statistical analysis was performed on 200 patients with singlesegment lumbar disc herniation treated by PELD. The patients were divided into two groups(routine puncturegroup and simple puncture endoscopic monitoring tube placement group). The indexes of puncture time,frequencyof fluoroscopy,total length of surgical duration and length of endoscopic duration were compared between the twogroups. Visual analog score(VAS)and Oswestry disability index(ODI)were used to evaluate the clinical effectafter operation. Results No surgical complications occurred in both groups. The puncture time and length of surgi⁃cal duration were shorter,and frequency of fluoroscopy was less in the simple group than in the routine group(P <0.05). The VAS score and ODI in both groups were significantly lower than those before operation(P < 0.05). Therewere no significant differences in VAS score and ODI between the two groups. Conclusions Simple punctureendoscopic monitoring tube placement can shorten the puncture time and length of surgical duration,reduce fre⁃quency of fluoroscopy. It is safe and effective,which is worth popularizing and applying in PELD.

Objective The research tried to investigate the reliability of platelet⁃rich plasma compositesβ⁃alcium phosphate for the treatment of early Osteonecrosis of the Femoral Head. Methods 108 patients of earlyOsteonecrosis of the Femoral Head(unilateral lesion)were randomly divided into compound group(52 cases)andcontrol group(56 cases). The control group was treated with osteonecrosis bone scraping,biomaterials β⁃alciumphosphate particles filling and pressing as well as β⁃alcium phosphate columnar ceramic rod implantation. Thecomposite group mixed biomaterials β ⁃alcium phosphate particles to match platelet⁃ich plasma filling andcompression and columnar calcium phosphate ceramic rod implantation. The VAS and Harris scores,imaging evalu⁃ation and quantitative indexes were compared between the two groups. Results Compared with multiple time sitesin 24 months after operation,the implant mineralization and new bone crawling growth replacement in the compos⁃ite group were significantly better than those in the control group(P < 0.05). The VAS and Harris in the compositegroup were also better than those in the control group(F = 30.12,17.88,P<0.05). Conclusion Platelet⁃richplasma composites β⁃alcium phosphate have a reliable advantage in the treatment of early Osteonecrosis of theFemoral Head,which could accelerate the growth and mineralization of new bone crawling,promote the fusion ofcalcium phosphate implants with their own host bone and facilitate functional recovery.

Objective To study the related factors of postpartum discontinuation of mother infant antiviralblocking leading to hepatitis activity among pregnant women with chronic HBV. Methods A prospective analysisof pregnant women with HBV infections in our hospital and the Affiliated Hospital of Nantong University fromJanuary 2016 to January 2020 was conducted. Totally 93 pregnant women with chronic HBV that met the criteriawere selected as the research subjects,and antiviral drugs were given during pregnancy,which should be stoppedimmediately or 1 ~ 3 months after delivery. Liver function,HBV⁃ M and HBV⁃DNA titers were followed up beforeantiviral treatment,when antiviral drug was stopped,and 1 month,3 and 6 months after antiviral treatment discon⁃tinuation,to find out the status of hepatitis activity and possible related factors after antiviral treatment discontinua⁃tion. Results Thirty⁃three patients(35.48%)had hepatitis activity within six months after antiviral treatmentdiscontinuation. According to the liver function 6 months after antiviral treatment discontinuation,the observationobjects were divided into hepatitis activity group and hepatitis⁃free activity group. Comparison of related itemsbetween the two groups found that previous antiviral treatment,combined with autoimmune diseases,second preg⁃nancy,antiviral gestational weeks,antiviral drug(telbivudine and tenofovir),discontinuation of antiviral drugs2 ~3 months postpartum,antiviral treatment time,and HBeAg titers when antiviral treatment discontinuation werestatistically different(P<0.05). Multivariate logistic regression analysis found that HBeAg titer were independentlycorrelated with hepatitis activity when antiviral drugs were discontinued(OR = 3.253,P = 0.025,95%CI:1.225 ~8.163). ROC curve analysis was used and showed that the area under the curve was 0.733;the maximum Youdenindex 0.561;the optimal critical value 258 S/CO,and the corresponding sensitivity and specificity were 72.73% and 83.33% respectively(P<0.001,95%CI:0.632 ~ 0.820). Conclusion Tenofovir can be used in pregnantwomen with chronic HBV in the second and third trimester of pregnancy for mother infant antiviral blocking.Dynamic monitoring of HBeAg titer could provide clinical reference for the discontinuation of antiviral.

Objective The research aimed to investigate the effects of goal⁃directed fluid therapy on hemo⁃dynamics and S100β protein in patients undergoing meningioma resection. Methods Forty⁃eight patients undergo⁃ing meningioma resection with supine position were randomly divided into 2 groups:routine fluid therapy group(group C)and GDFT group(group G). The strategy in group G is to maintain stroke volume variation(SVV)<13%. Fluid therapy was based on the normal artery pressure ,hemoglobin and hematocrit. The hemodynamicindexes were recorded at all time intervals. S100 β protein and rehabilitation indicators in two groups perioperative.Results Compared with group C,the urine volume,amount of colloids,MAP and CI were significantly increased⁃while the amount of crystals,the total fluid intake and output,the concentration of S100 β protein,the length ofstay in the ICU and hospital were significantly decreased in group G(P < 0.05). Conclusion During meningiomaresection,GDFT guided with SVV not only maintain the effective circulating blood volume and systemic perfusionbut also decrease the concentration S100 β protein,improve clinical outcomes.

Objective To investigate the relationship between QRS⁃T angle and coronary artery disease(CHD)and its predictive value for MACE after PCI. Methods 180 patients with CHD admitted to our hospitalwere selected as the case group,and 85 non⁃CHD were selected as the control group. All patients underwent thethree⁃dimensional electrocardiogram to measure spatial QRS⁃T angle. According to the Gensini score,the casegroup were divided into three group,mild subgroup,moderate subgroup and severe subgroup. 174 patients who un⁃derwent PCI were followed up for 6 months. The spatial QRS⁃T angle and composition ratio between groups werecompared and analyzed. Explore the relationship between spatial QRS⁃T angle and MACE in patients after PCI. Results The Spatial QRS⁃T angle of the case group was larger than the control group[(91.86 ± 45.67)° vs.(50.83 ± 40.58)°,P<0.05]. The ratio of QRS⁃T angle > 100° in the severe subgroup was higher than that in themild subgroup(76.9% vs. 39.7 %)and moderate subgroup(76.9% vs. 42.3%). Logistic regression analysis resultsshowed that,QRS⁃T angle > 100° is a risk factor for MACE in patients with CHD after PCI(OR = 3.096,95%CI:1.106 ~ 8.671,P < 0.05). The sensitivity and specificity of the QRS⁃T angle > 100° on the diagnosis of the severesubgroup with CHD were 76.9% and 59.1%,the area under ROC curve was 0.680(95%CI:0.600 ~ 0.761,P <0.05).The sensitivity and specificity of the QRS⁃T angle > 100° on prediction MACE was 70.5% and 52%,the areaunder ROC curve was 0.612(95%CI:0.517 ~ 0.708,P = 0.027). Conclusions QRS⁃T angle > 100° has certainclinical value on preliminary assessment of the degree of coronary stenosis ,and on prediction MACE after PCI.

Objective To investigate the significance of serum matrix metalloproteinase⁃9(MMP⁃9)andmatrix metalloproteinase tissue inhibitor 1(TIMP⁃1 in the development of respiratory syncytial virus(RSV)bronchiolitis to asthma. Methods The serum MMP⁃9 and TIMP⁃1 of RSV bronchiolitis children(RSV group)andhealth control were determined,and the RSV group were divided into non⁃asthmatic subgroup and asthma subgroupaccording to the follow⁃up results. ROC curve and logistic regression were used to analyze the performance ofMMP⁃9,TIMP⁃1 and MMP9/TIMP⁃1 in the diagnosis of RSV bronchiolitis and its progression to asthma. Results Compared with those in the control group,MMP⁃9,TIMP⁃1 and MMP9/TIMP⁃1 in the RSV group were significantlyincreased(P < 0.05). The area under the curve(AUC)of MMP⁃9,TIMP⁃1 and MMP9/TIMP⁃1 for the diagnosisof RSV bronchiolitis was 0.942,0.579,and 0.827,respectively. Compared with those in non⁃asthmatic group,MMP⁃9 and MMP9/TIMP⁃1 in asthma group were significantly increased(P < 0.05). MMP⁃9 and MMP⁃9/TIMP⁃1were independent risk factors in the development of RSV bronchiolitis to asthma(P < 0.05). Conclusion SerumMMP⁃9 and TIMP⁃1 have certain significance in RSV bronchiolitis and its progression to asthma.

Objective To investigate the correlations between the breast cancer clinical prognostic stageⅠ⁃Ⅲ from the AJCC(Eighth Edition)and ultrasonographic characteristics by BI⁃RADS classification. Methods A total of 386 breast cancer masses in 343 patients classified as stages Ⅰ⁃Ⅲ according to the AJCC(Eighth Edition)were selected and divided into three groups. χ2 test was used to analyze the relationships between breast cancer ultrasonic characteristics and AJCC stages Ⅰ⁃Ⅲ. Results The ultrasonographic characteristics such as oval shapes or irregular shapes of the masses,long axes non⁃paralleled to the skin,uncircumscribed margin,indistinct margin,speculated margin,and shadowing posterior features were significantly different across the prognosis stages Ⅰ⁃Ⅲ according to the AJCC(Eighth Edition). Conclusion There are significant differences in tumor mor⁃phology orientation,marginal morphology and posterior features in patients across different stages of breast cancer prognosis.

Objective To understand the prevalence of hypertension in coal miners and analyze its influ⁃encing factors. Methods From April 2017 to June 2017,we selected 652 male coal miners from a state⁃ownedenterprise in Shanxi Province as research subjects. Hypertension was determined according to the diagnostic criteriaof the 2010 Chinese guidelines for the prevention and treatment of hypertension. The concentrations of 4 PAHsmetabolites in urine were determined by HPLC. The influencing factors of hypertension in coal miners were analyzedby chi⁃square test,Wilcoxon rank⁃sum test and multivariate logistic regression model. Results The overall prevalenceof hypertension among coal miners was 33.7%. Wilcoxon rank⁃sum test showed that there was no statisticallysignificant difference in the level of urinary PAHs metabolites between the groups of normal blood pressure andhypertension(P > 0.05). Logistic regression analysis showed that age > 40,alcohol consumption,overweight andobesity were all risk factors for hypertension in coal miners and the protective factors of hypertension in coal mineworkers were the underground first⁃line work and underground auxiliary job. Conclusion No association is foundbetween urinary PAHs metabolites and hypertension. Coal miners aged over 40 years,with BMI ≥ 24 kg/m2,andpoor living habits have a higher risk of hypertension. Health education and daily monitoring of such people shouldbe strengthened.

The NLRP3 inflammasome is an immportant component of the innate immune system thatmediates caspase⁃1 activation and the secretion of pro⁃inflammatory cytokines IL⁃1β/IL⁃8. However,the aberrantactivation of the NLRP3 inflammasome has been linked with several diseases,such as parkinson disease,athero⁃sclerosis,acute lung injury and etc. Non⁃coding RNAs,such as miRNA and lncRNA,refer to RNAs which cannot code for proteins but can regulate the expression of NLRP3 at transcription or post⁃transcription level and partic⁃ipate in the progress of diverse NLRP3⁃related diseases. In this review,the latest developments in this area will beelucidated to provide new insights for further treatment of NLRP3⁃related diseases.

Breast cancer is the most common malignant tumor in women. The status of axillary lymphnodes is of great significance to breast cancer patients. Ultrasound is one of the most important method to evaluatethe status of axillary lymph nodes. Conventional ultrasound can evaluate the status of axillary lymph nodes from theshape,internal echo and blood flow distribution,while elastic imaging and contrast⁃enhanced ultrasound can betterjudge the benign and malignant of axillary lymph nodes through the hardness and blood flow perfusion of axillarylymph nodes. Ultrasound⁃guided biopsy can provide pathological information of axillary lymph nodes and furtherimprove the accuracy of diagnosis. The development of artificial intelligence technology also provides more refer⁃ence for ultrasound to evaluate axillary lymph nodes of breast cancer patients. The advances of different ultrasoundtechniques used in axillary lymph nodes of breast cancer was reviewed in this article.
