The present review briefly discusses the Coronavirus Disease 2019(COVID ⁃19),lung ultra⁃ sound,and the application of lung ultrasound in COVID⁃19,aiming to evaluate the application value of lung ultra⁃ sound in patients with COVID⁃19 and provide the valuable information to screen,diagnose and evaluate the condi⁃ tion of COVID⁃19 for the clinicians.

Objective To elucidate the role of mutual regulation of IRE1a and miR ⁃346 in maintaining ERS strength,and it′s role in cisplatin resistance of cervical cancer cells. Methods We screened cisplatin⁃resistant cervical cancer cells via gradually increasing the concentration of cisplatin,and detected the expression of IRE1a and miR⁃346 via gene array assay,quality PCR assay,flow cytometry assay and dual⁃luciferase reporter assay. We further validated the mutual regulation between IRE1a and miR⁃346,and its role in cispaltin resistance of cervical cancer cells. Results In contrast with the parental cervical cancer cells,there was a basic ERS in cisplatin resistant cells,and ERS enhanced the expression of miR⁃346 in IRE1a dependent manner which took part in the cisplatin resistant of cervical cancer cells. MiR⁃346 down⁃regulated the expression of IRE1a,which inhibited the degrada⁃ tion of pri⁃miR⁃125b,maintained the ERS strength in cisplatin⁃resistant cell and promoted cellular survival under cispaltin treatment. Conclusion Mutual regulation of IRE1a and miR⁃346 promotes cispaltin resistance of cervical cancer cells through maintaining ERS strength of cisplatin⁃resistant cells.

Mechanism of TRPV1 attenuating blood ⁃brain barrier damage and anxiety ⁃like behaviors after cerebral ischemia in mice by inhibiting p ⁃JNK and p ⁃p38 MAPK signaling pathways

Objective To explore the effect of TRPV1 on blood⁃brain barrier injury and anxiety⁃like behaviors after cerebral ischemia⁃reperfusion in mice and its and mechanism. Methods A model of middle cerebral artery embolism(MCAO)was constructed by use of thread embolization. Capsazepine(CPZ)of 7.5 μg/3 uL was injected into the lateral ventricle. HE staining was used to detect pathological changes in amygdala;Immunofluores⁃ cence staining was applied to detect vascular damage in amygdala;Western blot was used to detect levels of p⁃JNK and p ⁃ p38 MAPK;High plus maze was used to detect anxiety ⁃like behaviors. Results As compared with I/R group,the moving distance and length of stay in the open arms of high plus maze in I/R+CPZ group increased significantly(P < 0.001). HE staining showed that nucleus pyknosis and edema,as compared with I/R group were reduced in I/R + CPZ group. As compared with I/R group,Claudin5 expression was increased in I/R + CPZ group,whereas levels of p⁃JNK and p⁃p38 MAPK were significantly reduced(P < 0.05). Conclusions Inhibition of TRPV1 can alleviate damage in blood⁃brain barrier and anxiety⁃like behaviors after cerebral ischemia in mice by reducing levels of p⁃JNK and p⁃p38 MAPK.

Objective To investigate the expression of miR⁃128⁃3p in colorectal cancer(CRC)and its effect on the invasion and migration of CRC cells and the underlying mechanism. Methods The canceroustissues and paired para⁃cancerous tissues of CRC patients(n = 56)who underwent surgical treatment in our hospital from⁃ June 2015 to May 2017 were collected. CRC cell lines and human normal colorectal mucosal cells were obtained. The expression of miR⁃128⁃3p in CRC tissues and cells were detected by qPCR and its relationship with the clinico⁃ pathologic feature of CRC patient was further analyzed. miR⁃128⁃3p mimic,mimic NC,miR⁃128⁃3p inhibitor and inhibitor NC were transfected into SW620 cells,respectively. Transwell assay was used to detected the invasion and migration of SW620 cells. Luciferase reporter assay was used to validate the targeted relationship between miR⁃128⁃3p and ZEB1. After transfection with ZEB1plasmid alone or in combination with miR⁃128⁃3p mimics,the invasion and migration ability of SW620 cells was detected by transwell assay,the expression of ZEB1 protein or mRNA in SW620 cells was detected by WB and qPCR,respectively. Results miR⁃128⁃3p was down⁃regulated in CRC cells and tissues(P < 0.05),and its expression level was related to the degree of tumor differentiation lymph node metastasis and distant metastasis(P < 0.05). Up⁃regulated expression of miR⁃128⁃3pcould significantly inhibit the invasion and migration of SW620 cells and down⁃regulated expression of miR⁃128⁃3p reversed the inhibi⁃ tory effect of miR⁃128⁃3p mimics on SW620 cells. Theluciferase reporter assay confirmed that miR⁃128⁃3p directly targeted to regulated ZEB1. Over ⁃ expression of ZEB1 could significantly promote the invasion and migration of SW620 cells,whereas the collaborative transfection of miR⁃128⁃3p mimics could down⁃regulate the expression of ZEB1 protein and mRNA,and reduce the invasion and migration ability of SW620 cells. Conclusions The low expression of miR⁃128⁃3pwas correlated with tumor differentiation,lymph node metastasis and distant metastasis of CRC patients. miR⁃128⁃3p could inhibit the invasion and migration of CRC cells by targeting ZEB1

Objective To explore the role of rehmannia glutinosa polysaccharide in proliferation and apoptosis of prostate cancer PC ⁃ 3 cells by studying vascular endothelial growth factor(VEGF)/protein kinase B (Akt)signal pathway. Methods PC⁃3 cells in logarithmic growth phase were divided into blank group and low⁃ medium⁃ and high⁃dose groups,rehmannia glutinosa polysaccharides of 0,20,40,or 60 μmol/L were added to culture. MTT assay was used to detect the inhibition rate of PC ⁃ 3 cell proliferation. The invasion ability of PC ⁃ 3 cells was detected by Transwell invasion assay. The apoptosis rate of PC ⁃ 3 cells was detected by flow cytometry. The protein expression of VEGF/Akt signaling pathway was detected by Western blot. Results With an increase in rehmannia glutinosa polysaccharide concentration,the inhibition rate of PC⁃3 cell proliferation increased gradually in a time⁃dependent way(P < 0.05);the number of penetrating cells in PC⁃3 cells decreased gradually and the apoptosis rate increased gradually,vascular endothelial growth factor receptor 2(VEGFR2),phosphorylated phosphatidylinositol 3⁃kinase(p ⁃PI3K)and phosphorylated p ⁃Akt protein were gradually down ⁃ regulated in a concentration⁃dependent manner(P < 0.05). Conclusions Rehmannia glutinosa polysaccharide can inhibit prolif⁃ eration and invasion of prostate cancer PC ⁃3 cells and promote PC ⁃3 apoptosis,which may result from inhibiting VEGF/Akt signaling pathway.

Objective To observe the Hyperoside(Hyp)therapeutic effect of collagen ⁃induced arthritis (CIA)in mice. Methods The DBA/1 mice were randomly divided into normal control group(= 10 mice per group)and model group(= 35 mice per group). To induce the CIA model,the model group were given bovine type Ⅱ collagen and complete Freund adjuvant emulsifier on day 0 and day 21,the normal control group was injected with saline. To examine the therapeutic effect of hyperoside,on day 31 arthritic mice were randomly divided into three groups(= 8 mice per group)including the CIA group and hyperoside⁃treated group(25 mg/kg and 50 mg/kg). The measurement of paw thickness and the changes of clinical arthritis index in mice were dynamically observed. On the day 52,the mice were killed,the pathological changes of ankle joint were observed by HE staining. The serum levels of tumor necrosis factor⁃α(TNF⁃α)and interleukin⁃6(IL⁃6)were measured by ELISA. Results CIA model was successfully established,then to measure the paw thickness and clinical arthritis index scores on the day 28. The paw thickness and clinical arthritis index scores were significantly decreased in hypero⁃ side⁃treated group(< 0.01). Cytokines were measured by ELISA,the contents of serum TNF⁃α and IL⁃6 in the hyperoside ⁃treated group were significantly lower than those in the CIA group,with significant statistical signifi⁃ cance. Histopathological observation results showed that compared with the CIA group,the synovial hyperplasia and inflammatory cell infiltration of the ankle joint in the hyperoside ⁃treated group were significantly reduced. Conclusion Hyperoside may have a therapeutic effect on rheumatoid arthritis.

Objective To investigate the relationship between pelvic floor dysfunction(PFD)and female sexual dysfunction(FSD)and the influence factors of FSD. Methods Participants included 841 multiparous women whose baseline data were collected. PFD and FSD screening,analysis of surface electromyography(EMG),pelvic organ prolapse quantification(POP ⁃Q),and evaluation of pelvic floor muscle(PFM)strength and self ⁃ rating depression scale(SDS)were implemented in all participants. Univariate analysis and multivariate logistic regres⁃ sion were used to identify the relationship between PFD and FSD as well as influence factors of FSD. Results Among all the participants,603 had PFD(71.4%);581 encountered with FSD(61.6%),including 285 cases of desire disorder(33.9%),292 of dyspareunia(34.7%),137 of arousal disorder(16.3%),and 190 of orgasm disorder(22.6%). There were 397(65.8%)FSD patients among those with PFD. After univariate analysis,multi⁃ variate logistic regression showed CPP was independently associated with increasing risk of arousal disorder,dyspa⁃ reunia and orgasm disorder. Age and SDS were risk factors of desire disorder and arousal disorder;age was a pro⁃ tective factor but SDS a risk factor of dyspareunia;menopause,POP⁃Q stage of posterior vaginal wall(Stage I), and SDS were risk factors for orgasm disorder(P < 0.05). Conclusion There is high prevalence of FSD among PFD patients. CPP has strong association with FSD. The critical influencing factors of FSD are age,menopause and depression.

Objective To investigate the correlation between CYP2D6,and SULT1A1 gene polymor⁃ phisms and treatment effect of tamoxifen(TAM)in patients with breast cancer. Methods From January 2018 to January 2019,203 patients treated with TAM were included in the study. The genotype of CYP2D6(100C > t and SULT1A1 638(G > A)was detected,and the correlation between genotypes,clinicopathological parameters and the prognosis was analyzed. Kaplan Meier method was used for survival analysis,and log rank test was used to test whether there was survival difference among patients. Results Among 203 cases,44 cases(21.67%),117 cases (57.64%)and 42 cases(20.69%)were CYP2D6(100C > t)CC,CT and TT respectively. Sult1A1 638(G > A was all GG type,and no GA or AA type was found. The 2⁃year OS rate of patients with CC and CT was higher than that of patients with TT(P = 0.001). The OS of CC and CT patients was significantly longer than that of TT patients (P = 0.001). Conclusion The OS of TT patients after TAM treatment is significantly lower than that of CC and CT patients. No allele mutation is found in SULT1A1 638(G > A).

Methods A total of 184 patients with cervical cancer in the Department of Gynecology and Obstetrics of our hospital from January 2016 to December 2017 were enrolled. The IL⁃33 expression in cervical cancer and adjacent tissues were detected by immunohistochemistry. The relationship between IL⁃33 expression and clinic⁃pathological characteristics in patients with cervical cancer was analyzed. The factors influencing the prognosis was analyzed by Cox proportional hazards analysis. Results The IL⁃33 expression was negative or weak in cervical adjacent tissues. There were 126 and 58 cases with high and low expression of IL⁃33 in cervical cancer tissues,respectively. IL⁃33 is mainly expressed in the cytoplasm of cells,showing brownish yellow and tan granules. The IL⁃33 expression level in cervical cancer tissues were significantly associated with the cell differentiation degree,depth of muscular invasion,FIGO stage and lymph node metastasis(P < 0.05). By the end of follow⁃up,141 cases of cervical cancer patients had recurrence or death,while 43 cases had no recurrence or death. Among them,103 cases had higher IL⁃33 expression and 38 cases had lower IL⁃33 expression in patients with recurrence or death. Another 23 cases had higher IL⁃33 expression and 20 cases had lower IL⁃33 expression in patients without recurrence or death. The expression level of IL⁃33 in cervical cancer tissues between patients with poor and good prognosis was significantly different(P = 0.016). The muscular invasion depth,FIGO stage,lymph node metastasis and higher IL ⁃ 33 expression were risk factors for poor prognosis in patients with cervical cancer (P < 0.000). Conclusions The expression level of IL⁃33 in cervical cancer tissues was higher,which is closely related to poor prognosis.

Objective To investigate the value of placental blood perfusion monitoring in drug treatment of pregnant women with gestational hypertension. Methods 228 patients with gestational hypertension in our hospital from 2016 to 2018 were selected,including 93 patients with mild preeclampsia and 145 patients with severe preeclampsia. All patients received conventional drug treatment,while normal pregnant women were selected as the control group. Three dimensional power Doppler ultrasound was used to evaluate the efficacy of placental blood perfusion monitoring. Results The hemorheological indexes of VI,fi,VFI,s/d of umbilical artery were significantly different in the three groups before treatment,and there was no significant difference in hemorheological indexes of the three groups. Before treatment ,the ultrasound parameters of the women in Group A and Group B were significantly lower than those of the control group,and the Pi of the left and right uterine arteries in Group B was significantly lower than that of Group A,while the scores of PI and Apgar in the middle cerebral artery of Group B were significantly higher than those in Group A(P < 0.05). There was no significant correlation between F1,2D color Doppler index and Apgar score,but there was significant correlation between placental V1,placenta VFI 2D color Doppler index and Apgar score. The total effective rate of Group A and Group B was 96.77% and 95.56% respectively. After drug treatment,placenta VI,FI and VFI showed significant differences under different therapeutic grades. Conclusion The monitoring of placental blood flow by three⁃dimensional power Doppler ultrasound can effectively evaluate the drug treatment effect of pregnant women with gestational hypertension,and has strong clini⁃ cal application value.

Objective To observe the effect of different ventilation modes on the diameter of optic nerve sheath in elderly patients undergoing laparoscopic colorectal cancer surgery. Methods Sixty patients who underwent laparoscopic resection of rectal and sigmoid tumors were selected,including 31 males and 29 females whowere from 60 to 80 years old with ASA Ⅱ grade and BMI 18.5 ~ 28 kg/m2 . They were randomly divided into Group V or Grand P by random number table method. Group V used VCV ventilation mode,and Group P used PCV ventilation mode. Recorded the cross⁃sectional and sagittal average ONSD values of left and right eyes before induction of anesthesia(T0),10 minutes after induction of anesthesia in the supine position(T1)10 minutes(T2)and 60 minutes (T3)after the CO2 pneumoperitoneum Trendelenburg position established,and 10 minutes after the pneumoperito⁃ neum disappeared and supine position resumed(T4);recorded the peak airway pressure(Ppeak),mean airway pressure(Pmean),PaCO2,PETCO2 at T1⁃T4 ,and HR,MAP,CVP,SpO2 at T0⁃T4;recorded the incidence of nausea and vomiting,dizziness,headache and pulmonary complications during the 3 days after surgery. Results Compared with T1,the ONSD of the two groups increased significantly at T2⁃T4(P < 0.05),compared with T3,the ONSD of the two groups was significantly reduced at T4(P < 0.05),and the ONSD of the P group was more signifi⁃ cant than that of the V group at T2⁃T3 Decrease(P < 0.05). At T2⁃T3,Ppeak,PaCO2 and CVP in group P were sig⁃ nificantly lower than those in group V(P < 0.05). There were no significant differences in postoperative complica⁃ tions such as nausea,vomiting,dizziness,and headache between the two groups. Conclusion Compared with volume controlled ventilation,pressure controlled ventilation can effectively alleviate the increase in intracranial pressure during pneumoperitoneum and Trendelenburg position during laparoscopic colorectal cancer surgery.

Objective To investigate the predictive effect of lipoprotein ⁃ associated phospholipase A2 (Lp ⁃ PLA2)on patients with obstructive sleep apnea syndrome(OSAHS)with ischemic cerebrovascular disease (ICVD). Methods The clinical data of 93 patients with OSAHS were retrospectively analyzed. According to whether they had ICVD or not,they were divided into OSAHS with ICVD group(38 cases)and OSAHS without ICVD group(55 cases). The differences in demographic data,vascular risk factors and Lp⁃PLA2 levels were ana⁃ lyzed,and the predictive effect of Lp⁃PLA2 on ICVD in OSAHS patients was analyzed by binary Logistic regression analysis and receiver operating characteristic curve(ROC). Results (1)Compared with the OSAHS without ICVD group,the OSAHS with ICVD group was older,with higher levels of Apnea⁃hypopnea index(AHI),oxygen desat⁃ uration index(ODI),and Lp⁃PLA2,and a higher prevalence of hypertension. The difference was statistically sig⁃ nificant(P < 0.05).(2)Logistic regression analysis showed that the younger the age,the lower the AHI were pro⁃ tective factor for patients with OSAHS complicated with ICVD. The elevated Lp⁃PLA2 was a risk factor for patients with OSAHS complicated with ICVD.(3)The area under the ROC curve of Lp⁃PLA2 for predicting ICVD in patients with OSAHS was 0.805(95%CI:0.710 ~ 0.880,P < 0.001),and the best predictive value of Lp⁃PLA2 was 257.39 ng/mL(sensitivity 86.8%,specificity 67.3%). Conclusion Lp⁃PLA2 is related to the occurrence of ICVD in OSAHS patients,and elevated Lp⁃PLA2 levels may have certain predictive value for OSAHS patients with ICVD.

Methods A prospective study on 81 patients with cTBI admitted to our three intensive care units from December 2016 to October 2020,and be randomly divided into head and neck cooling group(HNC group 41)and normal ice cap cooling group(HC group 40). At admission,24 and 72 hours after admission,venous blood was taken to detect the concentration of NSE and Glu. Glasgow Coma Scale(GCS)Score,cochlear temperature,coagu⁃ lation function,blood pressure,heart rate,shivering,and 28⁃day case fatality were monitored. At 6 months after injury,the Glasgow outcome scale(GOS)was used to assess the clinical outcome. Patients were divided into death and survival subgroups according to the 28⁃day prognosis. Results HNC for 72 hours,the concentration of NSE (56.8 ± 34.0)ng/mL was significantly lower(t = 4.390,P < 0.01)than the 24 hour′s of itself,be decreased by (12.45 ± 18.16)ng/mL,which was significantly higher(t = 3.315,P < 0.01)than that of ice cap group(-2.18 ± 21.45)ng/mL;In HNC group,24 h concentration of Glu[(185.8 ± 42.7)μmol/L]was significantly higher(t = 2.916 P < 0.01)than that of ice cap group(158.0 ± 43.1)μmol/L. 72 h Glu(160.6 ± 21.3)μmol/L showed a significant decrease(t = 4.951,P < 0.01)to the 24 hour′ s of itself,which also significantly lower than that of the ice cap group[(177.3 ± 43.2)μmol/L,t = 2.190,P < 0.05]. There was no significant difference in 28⁃ day mortality (31.7% vs. 32.5%,χ2 = 0.006,P > 0.05)between the two groups. Compare to the ice cap group,there wasn′t a difference be found in GCS scores[(7.82 ± 1.86)vs.(8.00 ± 2.60),t = 0.293,P > 0.05]at discharge. But at 6 months after injury,the GOS of the HNC group be significantly improved(41.5% vs. 20.5%,χ2 = 4.083,P < 0.05 to the HC group,and there was no significant difference in adverse reactions between the two groups. In the survival subgroup,NSE and Glu showed a more remarkable performance. In the death subgroup,HNC can′t make NSE a significant change,Glu can be significantly reduced at 72 h. Conclusion HNC for 72 hours can significantly reduce NSE of patients with cTBI,markedly decrease the concentration of Glu after a transitorily increase,and significantly improve the long⁃term prognosis of patients.

Objective There is no research on the relationship between neutrophil to lymphocyte ratio (NLR)and red blood cell distribution width(RDW)and brain damage caused by febrile convulsions in children. Therefore,this study was aimed to explore the expression of NLR and RDW in febrile convulsions in children,and to analyze its relationship with brain damage. Methods A total of 212 children with febrile seizurestreated in our hospital from April 2017 to August 2019 wereselectedand divide into simple group(n = 126,simple febrile seizures and complex group(n = 86,complex febrile seizures)according to their clinical manifestations,another 86 children with fever but no convulsions received in the outpatient department of our hospital were selected as the fever but with no convulsions group. The blood routine test index levels of the 3 groups were compared ,the differential diagnosis value of NLR,RDW for simple and complex febrile seizures were analyzed using ROC curve;a total of 212 cases of febrile seizures were divided into brain injury group(n = 30)and no brain injury group(n = 182 according to whether the children complicatedbrain injury,the relationship between NLR,RDW and brain injury caused by febrile convulsion in children were analyzed using ROC curve. Results The levels of WBC,RBC NLR and RDW in the simple group and the complex group were higher than those in the fever butwith no convul⁃ sions group,and the levels of NLR and RDW in the complex group were higher than those in the simple group(all P < 0.05). ROC curve analysis results showed that the diagnostic sensitivity and specificity of NLR for simple and complex febrile convulsions were 70.05% and 65.55%,area under the curve(AUC)was 0.766;RDW was 68.77% 60.04% and 0.748,respectively. Logistic regression analysis showed peripheral blood levels of NLR and RDW were positively correlated with brain injury in children with febrile seizures (P < 0.05);ROC curve analysis showed that the sensitivity and specificity of NLR for the diagnosis of brain injury in children with febrile seizures were 75.40% and 70.06%,AUC was 0.848;RDW was 77.70%,68.88% and 0.870,respectively. Conclusions The levels of NLR and RDW in the peripheral blood of children with febrile seizures are abnormally increased,and the levels of NLR and RDW in the peripheral blood of children with complex febrile seizures are significantly higher than those of simple febrile seizures,and thelevels of NLR and RDW increase when accompanied with brain injury more obviously,the level of peripheral blood NLR and RDW could be used to differentially diagnose the types of febrile convulsions in children and predict whether it is accompanied by brain injury.

Objective To analyze the predictive value of tumor infiltrating lymphocytes(TILs)in patients with different subtypes of breast cancer treated with neoadjuvant chemotherapy. Methods The clinicopathological data of 56 female patients who underwent neoadjuvant chemotherapy in our hospital from July 2019 to July 2020 were analyzed. The proportion of TILs in the punctured tissue before chemotherapy was calculated and divided into low,medium,and high groups. Postoperative pathological assessment of the chemotherapy response of gross tumor tissues and statistical analysis were performed. Results The total effective rate of neoadjuvant chemotherapy was 76.8%(43/56),the effective rate of neoadjuvant chemotherapy in high TILs proportion group was higher than that in the low TILs proportion group(P < 0.05),and the TILs rate of triple negative breast cancer patients was signifi⁃ cantly higher than that of Luminal type and HER⁃2 positive breast cancer(P < 0.05). Multivariate analysis showed that the efficacy of neoadjuvant chemotherapy was only related to TILs ratio,but not to age,HR state,HER ⁃2 state and Ki ⁃67 index. Conclusions TILs proportion is an independent predictor for the efficacy of neoadjuvant chemotherapy in breast cancer. A high proportion of TILs indicates a better therapeutic effect,which is more obvi⁃ ous in patients with triple⁃negative breast cancer.

Objective To observe and investigate the effects of different laser cutting optical regions on high order aberrations in the personalized surgery of corneal topography guided Smart pulse Technology ⁃ Transepithelial photorefractive keratectomy. Method A total of 127 cases(203 eyes),treated by Corneal Topography Guided Smart pulse Technology⁃Transepithelial photorefractive keratectomy,According to the myopia degree,the low nearsighted⁃ ness was regarded as group A,and the effective cutting optical zone of 6.3 mm was set as group A6.3,6.7 mm as group A6.7,and 7.0 mm as group A7.0. Group B of moderate myopia was divided into group B6.3,group B6.7 and group B7.0 and high myopia C group was optically divided into C6.3 group,C6.5 group and C6.8 group. The naked visual acuity and high order aberrations of the anterior surface of the cornea were compared before and after 6 months operation. Results (1)The naked eye visual acuity of all patients six months after surgery reached or exceeded the preoperative best corrected visual acuity,and the high order aberrations were significantly higher than those before surgery(P < 0.05).(2)The comparison of the increased value of total high ⁃order aberration in the moderate ⁃low myopia group showed that the difference between 6.3 group and 6.7 group as well as 7.0 group was statistically significant(P < 0.05). There was no significant difference between 6.7 group and 7.0 group(P > 0.05).(3)In the high myopia group,the increased value of the higher order aberration was compared with that of the 6.5 group and the 6.8 group,and the difference was statistically significant(P < 0.05). There was no significant difference between 6.5 group and 6.8 group (P > 0.05). Conclusion Corneal Topography Guided Smart pulse Technology ⁃ Transepithelial photorefractive keratectomy can improve postoperative high order aberrations by selecting as large an effective cutting area as possible.

Objective To investigate the prevention efficiency of Pegylated recombinant human granulo⁃ cyte stimulating factor(PEG⁃rhG⁃CSF)on chemotherapy⁃naïve osteosarcoma patients treated by pirarubicin and cisplatin. Methods 77 chemotherapy ⁃naïve osteosarcoma patients were enrolled for the study. The patients were non ⁃ randomly divided into a control group(rh ⁃CSF group)with 25 patients and an experiment group(rhG ⁃CSF combined with PEG⁃rhG⁃CSF group)with 52 patients,according to whether they received PEG⁃rhG⁃CSF after AP chemotherapy. The prevention effect of PEG⁃rhG⁃CSF on reduction of neutrophils after chemotherapy were evaluat⁃ ed. Results Prophylaxis of PEG⁃rhG⁃CSF significantly reduced the incidence of Ⅲ⁃Ⅳ degree of ANC reduction shorten the duration of ANC reduction,decreased the dosage and days of supplemental usage of rhG⁃CSF,reduced the incidence of FN and shorten the days of antibiotic usage(all P < 0.05). Conclusion Prophylaxis of PEG⁃rhG⁃ CSF can effectively prevent the occurrence and duration of neutropenia with good safety. It is beneficial to ensure the following high⁃dose and high⁃intensity chemotherapy for osteosarcoma patients.

Objective To explore the safety and effectiveness of Cook cervical ripening balloon(CCRB and Simpubexant(dinoprostone)in inducing labor in pregnant women with hypertension in the third trimester. Methods A retrospective analysis was conducted to research into the data of full⁃term pregnant women with hypertension during pregnancy who were admitted to Anhui Maternity and Child Health Hospital from September 2018 to September 2020 with indications of labor induction and with a cervical Bishop score of < 6 points 104 cases were enrolled in the CCRB group and another 64 cases in the Simpubesheng group. The effects of promoted cervical ripening,the effect of labor induction,the fluctuation of blood pressure during labor induction,and the adverse maternal and infant complications of the two methods were compared. Results Simpubesheng was more effective in promoting cervical ripening than CCRB. The Bishop score of the cervix before the induction of labor in the Simpbe⁃ son group was significantly lower than that of the CCRB group(P < 0.05),but there was no significant difference between the two groups in the Bishop score of the cervix and the total effective rate of cervical ripening after induction of labor(P > 0.05). T The rate of vaginal delivery within 12 hours of intervention was significantly higher than that in the CCRB group(P < 0.05). The maximum blood pressure fluctuation in the Simpubeson group was significantly higher than that in the CCRB group(P < 0.05),but the two groups had adverse delivery complications and neonatal asphyxia. There was no statistical difference in incidence(P > 0.05). Compared with CCRB,Simpubexen was more likely to cause side effects of excessive or frequent uterine contractions(P < 0.05). Conclusion For pregnant women with hypertension during full⁃term pregnancy,Simpubexen and CCRB both is safe and effective in labor induction. When the cervical Bishop score is low,Simpubexen is more effective than CCRB in promoting cervical ripening,but it is easier. It can cause excessive uterine stimulation and large blood pressure fluctuations,so it must be used under close supervision.

Objective To explore the clinical value of betahistine combined with vestibular training for residual symptoms of benign paroxysmal positional vertigo (BPPV)following manipulative reduction. Methods 160 BPPV patients with residual symptoms in our hospital from January 2018 to December 2019 were randomly divided into a control group(79 patients)and a study group(81 patients). The control group received routine vestibular training alone,whereas the study group received betahistine in addition to vestibular training for 2 months. Visual analogue scale(VAS),frequency of residual symptoms,scores on dizziness disorder scale(DHI),and adverse reactions were compared between the two groups before and after treatment. Results Before treatment there were no significant differences between the two groups in the total scores on DHI physical disorder(DHIP), DHI emotional disorder(DHIE),DHI dysfunction(DHIF)and DHI(t = 0.228,0.054,0.193 and 0.267;P > 0.05). After treatment,the total scores on DHIP,DHIE,DHIF and DHI were lower in the study group than in the control group(t = 17.051,8.267,8.644 and 19.449;P < 0.05). Before treatment,there was no significant difference in the VAS score of vertigo between the two groups(t = 0.234,P > 0.05). After treatment,the VAS score of vertigo in the study group was lower than that in the control group(t = 13.583,P < 0.05). The frequency of walking instability floating feeling,drowsiness,and dizziness was lower in the study group than in the control group(t = 15.915 14.748,11.004 and 8.350;P < 0.05);There was no significant difference in the rate of adverse reactions between the two groups(χ2 = 0.231,P > 0.05). Conclusions Vestibular training combined with betahistine can effectively reduce the degree of vertigo disorder and lower the frequency of residual symptoms in BPPV patients after manual reduction.

Objective Using the enhanced CT radiomics in differentiation of renal angiomyolipoma without visible fat from homogeneous clear cell renal cell carcinoma. Methods Retrospective analysis was performed on 28 cases of homogeneous clear cell renal cell carcinoma and 22 cases of renal angiomyolipoma without visible fat which were confirmed by postoperative pathology. The arterial and venous phase images manual sketch tumor area were concerned,About 1 706 radiomics features,data after normalization and by space dimension reduction,were extracted from the image. Features were screened to establish LR,SVM,RF model respectively,five ⁃fold cross validation and selecting cross validation were used to set the highest AUC model as the best model. The clinical features were analyzed to determine the predictors and the clinical model. The comprehensive model was established by multi⁃factor binary Logistic regression based on the predicted values of the selected optimal radiomics model and clinical predictors. The nomogram based on combined with clinical factor and radiomics model. The test of Hosmer⁃ Lemeshow was used to evaluate the fitness of the line chart. Decision curve analysis was applied for clinical use. Results The best radiomics model was LR model,and the AUC of the model verified by Bootstrap method was 0.836(95%CI:0.701~0.927). The AUC value of the comprehensive model was 0.869(95%CI:0.701~0.927),and the calibration curve of the nomogram showed good consistency. Decision curve analysis verified the clinical usefulness of the predictive nomogram. Conclusion The combination of clinical factors and radiomics features has a strong ability to distinguish renal angiomyolipoma without visible fat from homogeneous clear cell renal cell carcinoma

Objective To analyze the volumetric measurement of post⁃stroke hemiplegic edematous hands using 3⁃D scanning and water displacement. Methods Hand volumes of 100 post⁃stroke hemiplegic patients were measured using a non⁃invasive 3⁃D scanner. We defined the hand mark as the line between the ulnar styloid and the radius styloid,then 3⁃D forms of the hands were captured by the 3⁃D scanning system. Traditional water displacement was used for volumetric measurement as well. Statistical analysis was applied to compare the hand volumetric results between the two measurement methods. Results There was no significant difference between two methods of hand volume measurement(asymptomatic hand:t = 1.378,P = 0.171;edematous hand:t = 1.973 P = 0.051). A significantly strong positive correlation was observed between the asymptomatic hand(r = 0.992 P < 0.001)and edematous hand volume(r = 0.997,P < 0.001)calculated via 3⁃D scanning and water displacement. The Bland⁃Altman analysis showed that the two measuring volume methods were within the 95% consistency limits of agreement(LOA). Conclusion The contactless three⁃dimension scanning method has the same effect in mea⁃ suring post⁃stroke edematous hand volumes when compared with the traditional method of water displacement. It can be used toconduct an accurate volumetric analysis of an edematous hand and provide reference for future research on the recovery of hand edema.

Objective Analyze the staffing,bed allocation,use of Traditional Chinese Medicine(TCM and TCM diagnosis and treatment equipment in community health care service centers and township healthcare centers in Guangdong Province,and understand the current situations of TCM resources in primary medical and health institutions in Guangdong Province through the comparison between regions and cities in the province,in order to provide policy suggestions for the further planning and development of TCM resources at grass⁃root level. Methods Using the national key specialties of TCM to build a monitoring indicator system,a total of 1,164 com⁃ munity health care service centers and 1,153 township health care centers in 21 cities and provinces in Guangdong Province were selected as the survey subjects,and descriptive statistics on the resources of TCM in primary medical and health institutions in Guangdong Province in 2019 analysis. Results The province′s TCM resources at grass⁃root level are relatively sufficient,and the regional development of grass⁃root medical and health institutions is different. The development of TCM resources at grass⁃root level in various prefecture⁃level cities is uneven ,where TCM resources usually account for a relatively low proportion. There are many types of TCM methods and a large number of equipment,but simple therapies are the main ones. Conclusions Continue to increase financial support to primary medical institutions,especially thosein the eastern,western and northern regions of Guangdong,andimprove the allocation and use of resources. Balance the speed ofdevelopment of prefecture⁃level cities,and improve develop⁃ ment efficiency,while strengthening development momentum. Strengthen technical investment,and improve soft⁃ ware and hardware,while improving the professional level of grass⁃root level TCM personnel.

A prospective cohort study on the sleep duration and risk of progression of proteinuria among patients with early diabetic kidney disease

Objective To investigate the relationship between sleep duration and the risk of progression of proteinuria in patients with early diabetic kidney disease(DKD). Methods 857 patients with early DKD admit⁃ ted to our hospital were studied. Cox proportional hazards regression models were used to evaluate the associations between different levels of the sleep duration and the risk of progression of proteinuria in patients with early DKD. Results A total of 765 patients with early DKD were enrolled in our study with an age of(59.1 ± 6.2)years and the median follow⁃up time was 2.8 years. A total of 123 cases with progression of proteinuria were identified in the cohort study. The total incidence of progression of proteinuria was 16.1%,and those in sleep duration < 6,6 8,> 8 h/d were 29.4%(45/153),12.5%(70/560)and 15.4%(8/52),respectively,with significant difference (P < 0.05). Compared with sleep duration 6~8 h,the multivariate adjusted hazard ratio(95% CI)of progression of proteinuria were 1.82(1.14 2.91)for < 6 h and 1.04(0.75 1.44)for > 8 h. According to gender,age BMI,physical activity,smoking and drinking habits,date of the stratified analysis showed that this was consistent with the whole adjusted population. Shorter sleep duration was associated with the risk of progression of proteinuria but no significant association was found between longer sleep duration and the risk. Conclusion Shorter sleep duration seemed to have increased the risk of proteinuria in patients with early DKD.

Objective To explore the therapeutic effect of Erxiao decoction(Xiaoqinglong decoction and Xiaochaihu decoction)on allergic rhinitis(AR)in patients with lung qi deficiency cold patten and the role of regulatory cells(Tregs)related factors. Methods A total of 64 patients were randomly assigned into observation group(n = 32)and control group(n = 32). The observation group was treated with Erxiao decoction,and the control group was treated with Loratadine tablets. After 28 days of treatment,the symptom score and serum trans⁃ forming growth factor including transforming growth factor⁃β,TGF⁃β,Interleukin⁃10(IL⁃10),interleukin⁃17(IL⁃ 17)content and quality of life were observed. Results The scores of sneezing,nasal itching and nasal congestion in the observation group were significantly lower than those in the control group(P < 0.05). In the observation group,the levels of TGF⁃β in serum were significantly higher than those in the control group,IL⁃10 was signifi⁃ cantly higher than that of the control group(P < 0.05),and IL⁃17 was significantly lower than that of the control group(P < 0.05). The rhinoconjunctivitis quality of life scale(RQLQ score)of the observation group was signifi⁃ cantly better than that of the control group(P < 0.05). Conclusion Erxiao decoction could relieve the symptoms of AR and increase the serum TGF⁃β and IL⁃10. It is worthy of further study on its mechanism and long⁃term efficacy.

Objective To study the clinical effect of modified Qibaisan combined with Q ⁃switched laser on chloasma of qi stagnation and blood stasis type via investigating its effect on malondialdehyde(MDA),lipid peroxide(LPO)and superoxide dismutase(SOD)in serum. Methods One hundred and twenty⁃six patients were randomly divided into treatment groups,control group 1 and 2 using the random digital table,treated with modified Qibaisan combined with Q⁃switched laser treatment,Q⁃switched laser alone and modified Qibaisan alone all for three treatment courses,one month as one course,respectively. At the end of the courses,the three groups were compared in terms of the curative effect. Results The skin lesions in treatment group were significantly improved,with the effective rate of 92.8%(39/42),while the effective rates of control groups 1 and 2 was 64.2%(27/42)and 71.4% (30/42),respectively. The differences in the effective rate between the three groups were statistically significant (P < 0.01). The serum MDA/LPO of treatment group was significantly lower than that of control group 1 or 2,and the SOD was significantly increased(P < 0.01). Conclusion Modified Qibaisan combined with Q ⁃ switched laser is more effective in treatment of patients with chloasma,and it is worthy of clinical application

Diabetic macroangiopathy is a common complication of diabetes characterized by persistent arteriosclerosis in the macrovascular bed. Several cohort studies have confirmed that intestinal flora and their metab⁃ olites can influence the occurrence and development of diabetes and arteriosclerosis through a variety of metabolic pathways. This paper focuses on the interaction between intestinal flora and diabetic macroangiopathy,analyzes the relationship between intestinal flora,diabetes and vascular lesions,and explores the potential of intestinal flora in the prevention and treatment of diabetic macrovascular complications ,in order to provide new strategies and research targets for the prevention and treatment of complications of type 2 diabetes.

With high morbidity and high mortality,bloodstream infection is a serious disease caused by pathogenic microorganisms invading the blood. Rapid and definite diagnosis of bloodstream infection,especially identification of pathogenic microorganisms and drug susceptibility analysis,are the premise for timely implementa⁃ tion of targeted antibiotic treatment ,which is particularly important for improving the prognosis of patients. Although blood culture technology is developing continuously,there is always huge challenge in the rapid diagnosis of bloodstream infection. At the moment,it is impossible to provide a report within 3 hours. Therefore,new rapid diagnostic techniques are urgently needed for clinical application. This article reviews the current research progress in laboratory diagnostic technology of bloodstream infection.
