COVID⁃19 is currently a public health problem forall mankind. The spread of the epidemic has severely affected the global economy,health,population,social development and people′s living habits. Although most patients are mostly non ⁃ severe with a good prognosis,a few patients are severeand progress rapidly. There⁃ fore,it is important to explore the progress factors of COVID⁃19 for prediction of out come and adjustment of the treatment strategies. Here we summarizes the factors affecting the prognosis of COVID⁃19.

Efficacy and safety of Chinese herbal decoction combined with western medicine in treatment of COVID⁃ 19:A meta⁃analysis

Objective To assess the treatment efficacy and safety of Chinese herbal decoction combined with western medicine on COVID⁃19 patients. Methods We searched related studies in PubMed,Cochran library Web of science,MEDLINE,Embase,CNKI,VIP,CBM,Wanfang Data,etc. and manual retrieval of relevant reference was conducted as a supplement. All eligible studies published from December 2019 to September 2020 were included and data were screened and extracted independently by two researchers. Review Manager 5.4 soft⁃ ware was used for meta ⁃analysis. Results Meta ⁃analysis showed that compared with the control group(western medicine group),experimental group(Chinese herbal decoction combined with western medicine)witnessed short⁃ ened hospital stay,and alleviation of cough and fatigue. There was no significant difference in terms of recovery rate,clinical effective rate,adverse drug events,death rate,CT improvement rate and alleviation of fever and polypnea between the two groups. Conclusion Chinese herbal decoction combined with western medicine has no significant advantages in terms of recovery rate ,clinical effective rate ,adverse drug events ,death rate,CT improvement rate,and alleviation of fever and polypnea,but has certain advantages in shortening hospital stay and relieving cough and fatigue. However,due to small sample size and uneven quality of the literature,random clinical trials with high⁃quality large samples are still needed for further verification.

Osteoporosis is a systemic metabolic disease. Bisphosphonates are the first ⁃line drugs in the treatment of osteoporosis,which have been proved to have significant effects on improving bone density and reduc⁃ ing the risk of fracture. This article systematically reviewed the mechanism of action,clinical efficacy,adverse reactions and drug holiday of several bisphosphonates,which might provide useful information for future research.

Objective To investigate the role of basic fibroblast growth factor in mouse skin injury in⁃ duced by clobetasol. Methods Skin damage was induced by continuously topical administration with clobetasol followed by tape stripping in C57BL/6J mice′s dorsal skin. bFGF containing gel was applied at 30 min after clobeta⁃ sol treatment. Dorsal skin image and trans ⁃epidermal water loss(TEWL)were recorded,H&E staining was con⁃ ducted in skin tissue with further histological analysis. Cell viability and tight junction protein expressions in human keratinocyte cell line,HaCaT were also determined via MTT assay and western blotting respectively. Results Compared with negative control group,skin injury was confirmed in clobetasol treated mice by rash,edema and increased TEWL,accompanied by shedding and death of epidermic cells and infiltration around the dermal layer in H&E stained skin tissue slides. However,bFGF containing gel treatment significantly attenuated skin damage and TEWL. We also noticed reduced death of epidermic cells,infiltration around the dermal layer and hemorrhage. Moreover,clobetasol stimulation mediated inhibition of cell proliferation and downregulation of tight junction protein,Claudin 1 and Claudin 2 in HaCaT in vitro. Conclusion bFGF containing gel could effectively alleviate mouse skin injury induced by successive application of clobetasol combined with tape stripping,thus provides prospective therapeutic approach for clinic corticosteroid addiction associated side effects.

Objective To investigate the effect and mechanism of activated platelet⁃induced angiogenesis on EMT of IMECs. Methods The effect of activated platelets on the angiogenesis of IMECs was investigated by comparing the changes in cell migration capacity and lumen formation of IMECs with and without PI3K/mTOR inhibitor BEZ235.To observe the effects on the protein expression of phosphorylation⁃related proteins and HIF⁃1α in the PI3K/AKT/mTOR signaling pathway,and the effects on the expression changes of fibrosis⁃related factors E⁃cadherin,α ⁃SMA,and CollagenI and gene expression levels. Results The IMECs′s proportion obtained by culture and identified as endothelial cells by morphological features of inverted microscope was over 95% . After adding ADP to resting platelets ,the concentration of p ⁃selectin antibody detected by FCM was significantly increased(P < 0.01),indicating that the number of activated platelets was significantly increased. Compared with the addition of PI3K/mTOR inhibitor BEZ235,activated platelet stimulation enhanced the cell migration ability and lumen formation ability of IMECs,and inhibited PI3K,AKT,mTOR phosphorylated protein and HIF⁃1α protein,the protein production and gene expression of fibrosis ⁃ related factors α⁃SMA and CollagenI were signifi⁃ cantly suppressed,while E⁃cadherin protein production and gene expression were up⁃regulated. Conclusion The mechanism of activating platelet⁃induced EMTs in IMECs cells may be achieved by promoting angiogenesis through the PI3K/AKT signaling pathway.

Objective To investigate whether ferroptosis is present in atherosclerosis(AS)induced by high⁃fat diet and in the process of foam cell formation. Methods ApoE-/- mice fed with high⁃fat diet were used to establish an AS model;RAW264.7 macrophages pretreated with oxidized low density lipoprotein(ox ⁃LDL)were used to establish a foam cell model. Serum levels of triglyceride(TG),total cholesterol(TC),low⁃density lipoprotein cholesterol(LDL⁃C)and high density lipoprotein cholesterol(HDL⁃C)were detected by an automatic biochemistry analyzer. H&E and Oil Red O staining were used to observe plaque formation and lipid accumulation in mice aortic roots. mRNA and protein expressions of p53,GPX4 and SLC7A11 in aortic tissues and foam cells were detected by qRT ⁃ PCR and Western blot. Intracellular levels of malondialdehyde(MDA),glutathione(GSH)and Fe2 + were measured by MTS colorimetry;and level of reactive oxygen species(ROS)was detected by Fluorescent probe DCFH⁃DA. Results As compared with the control group,mRNA and protein expression of ferroptosis⁃related P53 was markedly increased,In RAW264.7 macrophage⁃derived foam cells induced by ox⁃LDL,mRNA and protein expressions of p53 increased significantly,whereas mRNA and protein expressions of GPX4 and SLC7A11 decreased significantly. Production of lipid peroxide ROS and MDA in cells increased,Fe2 + level increased,and GSH content significantly decreased. Conclusions Ferroptosis is involved in development of atherosclerosis induced by high⁃fat diet in ApoE-/- mice and lipid peroxidation and ferroptosis in RAW264.7 macrophage ⁃ derived foam cells induced by ox ⁃ LDL are in⁃ creased,which may be related with the formation of atherosclerosis.

Objective To investigate the effect of Rho kinase inhibitor fasudil combined with puerarin on the TGF ⁃β1/Smads pathway and inflammatory factors in hypoxia ⁃induced pulmonary hypertension(HPH). Methods Sixty SD rats were divided into Sham group,model group,Puerarin group,Fasudil group and Fasudil and Puerarin combined treatment group. HPH rat model was constructed by hypoxia treatment in an atmospheric hypoxia chamber. Manometric catheters were used to measure the hemodynamic indicators RVSP,mPAP,PASP and PAWP,and the right ventricular hypertrophy index(RVHI)was calculated. HE staining was used to observe the injury of pulmonary artery vascular smooth muscle layer. Western blotting was used to detect the expression of related factors in the TGF⁃β 1/Smads pathway. qRT ⁃PCR was used to detect the expression of inflammation ⁃ related factors. The expression of Gal⁃3 protein was detected by immunohistochemistry. Results Pulmonary artery wall thickness and inflammatory infiltration of the combined treatment group declined;RVSP,mPAP,PASP,PAWP and RVHI decreased(P < 0.05); TGF⁃β1/Smads pathway related factors,inflammation⁃related factors and Gal⁃3 protein expression decreased(P 0.05)when compared with those of the model group,Puerarin group and Fasudil group. Conclusion Rho kinase inhibitor fasudil combined with puerarin can improve the damage of hypoxic pulmonary hypertension to pulmonary artery,artery,and provide a theoretical basis for clinical treatment of hypoxic pulmonary hypertension.

Objective To investigate the effects of propofol on the proliferation,migration and invasion of ovarian cancer SKOV3 cells. Methods SKOV3 cells were divided into blank control group,solvent control group,25 μmol/L propofol group,50 μmol/L propofol group and 100 μmol/L propofol group. Immunofluorescence assay and Western blot were used to detect the effect of propofol on the expression of yes associated protein(Yap in SKOV3 cells. Whether Yap over⁃expression interfered with the inhibitory effect of propofol on proliferation migration and invasion of SKOV3 cells was observed. Results Propofol inhibited the proliferation,migration and invasion of ovarian cancer SKOV3 cells;Propofol down regulated the expression of Yap protein in the nucleus of SKOV3 cells and Yap overexpression interfered with the inhibitory effect of propofol on the proliferation,migration and invasion of SKOV3 cells. Conclusion propofol may inhibit the proliferation,migration and invasion of SKOV3 cells by down regulating the expression of Yap protein.

Objective To explore the role of miR⁃488⁃3p in invasion and migration of placental tropho⁃ blast cells in preeclampsia. Methods Expression of miR⁃488⁃3p in placenta was detected by qRT⁃PCR,and its correlation with the clinical characteristics of patients with preeclampsia was analyzed. Human chorionic trophoblast cells were cultured in vitro and transfected with miR⁃488⁃3p mimic and inhibitor;Transwell was applied to observe the effect of miR ⁃488⁃3p on trophoblast cell invasion and migration. Bioinformatics software predicted the down⁃ stream targets of miR⁃488⁃3p and verified its expression in placenta;qRT⁃PCR was then used to detect the effect of miR⁃488⁃3p on MMP2 expression in trophoblast cells. Results As compared with that in normal pregnant women placental expression of miR⁃488⁃3p in patients with preeclampsia was significantly increased,and positively corre⁃ lated with systolic blood pressure,diastolic pressure;while expression of MMP2 was decreased. Overexpression of miR⁃488⁃3p inhibited trophoblast invasion and migration,MMP2 expression decreased;while inhibition of miR⁃488⁃ 3p expression developed a contrary result. Conclusions miR⁃488⁃3p increases in the placenta of patients with pre⁃ eclampsia,which may be involved in the pathogenesis of preeclampsia by silencing MMP2 to inhibit invasion and migration of trophoblast cells.

Comparison of clinical efficacy between injured vertebra mono⁃segmental fixation and across injured verte⁃ bra short segmental fixation in the treatment of thoracolumbar fracture

Objective To compare the clinical effect of injured vertebra mono ⁃ segmental fixation and across injured vertebra shortsegmental fixation in the treatment of thoracolumbar fractures. Methods From March 2015 to March 2018,90 patients with thoracolumbar fractures in our hospital were selected and randomly divided into observation group(n = 45)and control group(n = 45). Patients in the observation group were treated with injured vertebra mono⁃segmental fixation,while patients in the control group were treated with across injured vertebra short segmental fixation. All of patients were followed up for 24 months. The anterior height ratio of injured vertebral body sagittal Cobb angle,loss of correction,operation time,blood loss,incision length,quality of life and postoperative complications were compared between the two groups. Results There were no significant differences in operation time and quality of life between two groups(P > 0.05). There were significant differences in postoperative complica⁃ tions,blood loss,incision length and quality of life at the last follow⁃up between the two groups(P < 0.05). The quality of life,anterior vertebral height and Cobb angles were significantly restored in both of two groups,but the last follow ⁃ up,the anterior height and sagittal Cobb angle of the fractured vertebra body in the mono ⁃ segment fixation group were better than those in the across injured vertebra short segment fixation group,and the difference was statis⁃ tically significant(P < 0.05). Conclusions The short ⁃term and medium ⁃term clinical effect of injured vertebra mono⁃segment fixation in the treatment of thoracolumbar fractures is better than that ofacross injured vertebra short segmental fixation,causes less trauma and complications.

Objective To evaluate the analgesic effect of quadratus lumborum block(QLB)and transver⁃ sus abdominis plane block(TAP)on patient undergoing cesarean section. Methods We searched literature from Pubmed,Cochrane,Embase,Web of Science,CNKI,WanFang and Weipu Databases for randomized controlled trials(RCTs)on different effects of QLB and TAP in cesarean section. Primary outcome was the Visual Analogue Score(VAS)at different time points and secondary outcome was the total consumption of sufentanil. Meta⁃analysis was performed with Review Manager 5.3. Results Seven RCTs were included. Compared with TAP group,QLB group had a decreased VAS score 4 h(MD = -0.32,95%CI:-0.56 ~ -0.07,P = 0.01),8 h(MD = -0.6,95%CI:-0.86 ~ -0.34,P < 0.000 01),12 h(MD = -0.81,95%CI:-1.34 ~ -0.27,P = 0.003),24 h(MD = -0.41,95%CI:-0.77 ~ 0.06,P = 0.02)at movement after surgery and a lower VAS score 12 h(MD = -0.3,95%CI:-0.55 ~ -0.06,P = 0.02)at rest after surgery. QLB group also experienced a less consumption of sufentanil. Conclusion QLB has a better analgesic effect than TAP after cesarean section at movement within 24 hours,and there is less consump⁃ tion of sufentanil.

Objective To explore the clinical efficacy and safety of early onset of highly active anti⁃AIDS retroviral therapy(HAART)in asymptomatic postpartum women with HIV infection. Methods A total of 104 asymptomatic HIV postpartum women with CD4 cells ≥ 400 /μL in Guangxi were selected. They received HIV prevention of mother⁃to⁃child transmission(PMTCT)during pregnancy and continued to receive HAART after delivery(52 patients in group A)or discontinued postpartum HAART treatment(52 patients in group B). Regular follow ⁃ups were done postpartum 0 ~ 96 weeks to examine immune function,liver/kidney function and viral load and to analyze the effect of HAART on viral clearance,immune reconstitution,liver and kidney function in post⁃ partum women. Results CD4 count and CD4/CD8 between the two groups from 24 to 96 weeks were significantly different(P < 0.01). CD4 and CD4/CD8 significantly increased in group A(P < 0.05). After 72 weeks,CD4/CD8 ratio of group A was reversed(mean CD4/CD8)≥ 1.0). In Group B,CD4 and CD4/CD8 decreased significantly at 24 weeks postpartum(P < 0.01),and gradually decreased with time. Group A had a higher rate of viral suppression (RNA < 20 copy/mL)with prolonged treat time(P < 0.01). There was no significant difference(P > 0.05)in renal function(TBIL and CREA)between the two groups;but there was statistically significant difference(P < 0.05 in liver function(ALT and AST)from 48 to72 weeks that the levels of ALT and AST in group A were lower than those in group B and the AE of ALT and AST in group A was significantly lower than that in group B. Conclusion Early HAART is beneficial for virus removal and immune system reconstruction,promoting normal proportion of immune cell subpopulations. It is safe for liver and kidney in HIV asymptomatic women.

Proliferation,migration and invasion of glioma pancreatic carcinoma regulated by LncRNA PCAT6 via targeting miR ⁃ 185 ⁃ 5p

Objective To investigate the expression of long noncoding RNA(lncRNA)prostate cancer associated transcript 6(PCAT6)in pancreatic carcinoma tissues and cells and its molecular mechanism of regulating proliferation ,migration and invasion in pancreatic carcinoma cells. Methods The PCAT6 and miR ⁃185⁃5p expression in pancreatic carcinoma tissues,paired adjacent normal tissues and pancreatic carcinoma cells was detected by qRT⁃PCR. CCK8,Transwell assay was used to detect the ability proliferation,migration and invasion of pancreatic carcinoma cells. Bioinformatics prediction and dual luciferase reporter gene assay were used to demon⁃ strate the targeting relationship between PCAT6 and miR⁃185⁃5p. Results PCAT6 was up⁃regulated in pancreatic carcinoma compared with adjacent normal tissue and over⁃expression of PCAT6 was correlated with advanced TNM stage(P < 0.05)and lymph node invasion(P < 0.05). The exogenous silencing of PCAT6 could inhibit the prolif⁃ eration,migration and invasion of pancreatic carcinoma cells. The activity of luciferase significantly reduced after co⁃transfection of PCAT6⁃WT and miR⁃185⁃5p mimics(P < 0.05),but there was no obviously change of luciferase activity when PCAT6⁃WUT and miR⁃185⁃5p mimics co⁃transfected. Conclusion LncRNA PCAT6 regulates prolif⁃eration,migration and invasion of pancreatic carcinoma cells by targeting miR⁃185⁃5p.

 

Application of enhanced recovery after surgery in benign anorectal diseases

Objective To explore the safety,feasibility and economic efficacy of enhanced recovery after surgery(ERAS)strategy in benign anorectal diseases. Method This is a retrospective study. Patients with benign anaorectal diseases were enrolled and were divided into ERAS group and conventional group according to outpatient service(general outpatient service or ERAS outpatient service). The ERAS group adopts an optimized strategy while the conventional group adopts a traditional strategy. Clinical indicators including postoperative complications pain scores,length of hospital stay and expenses were observed in the two groups. Results 162 cases were included 82 in the ERAS group and 80 in the conventional group respectively. There were no statistically significant differ⁃ ences in age,nutrition risk score and anesthesia risk score between the two groups .VAS scores of the two groups showed a downward trend,VAS score of patients in the ERAS group was lower than that in the conventional group. Length of stay in the ERAS group was 3.0 days,shorter than that of conventional group,8.0 days. Median Hospital costs is 10 522.1 yuan in the ERAS group,lower than 13 905. 8 yuan in conventional group. It showed no signifi⁃ cant difference between two groups in complications. Conclusion This study has proved that ERAS strategy for benign anorectal diseases has obvious value and significance in controlling postoperative pain,shortening hospital stay and reducing hospital expenses,and no additional complications . It is worth popularizing and applying.

Objective To investigate the pathological features of pregnancy associated breast cancer (PABC)and its influence on clinical prognosis. Methods We retrospectively analyzed the data of 54 PABC patients,and age ⁃matched with 108 controls from January 2009 to December 2017. The clinical and pathological characteristics including the rates of(overall survival,OS)and(disease⁃free survival,DFS)of the patients in the two groups were compared and the factors affecting the prognosis of breast cancer were analyzed. Results PABC tumors were more advanced in tumor size,axillary lymph node involvement,carcinoma in a vessel,the negative rates of progesterone receptor(PR)and estrogen receptor(ER)(P < 0.05). The OS and DFS of PABC group were lower than those in the control group(P < 0.05). Multivariate analysis revealed that pregnancy status advanced tumor size,nodal involvement,and carcinoma in a vessel were related with poor DFS and OS(P < 0.05). Conclusion Pregnancy ⁃ associated status is a strong prognostic factor of OS and DFS in BC. Further studies should focus on biologic specificities and pathological features of PABC,providing evidence for the treatment and prognosis of PABC.

Objective To investigate the efficacy of drug⁃coated balloon(DCB)versus drug⁃eluting stent (DES)for the treatment of in⁃stent restenosis(ISR)with heterogeneous neointimal pattern. Methods In⁃stent restenosis patients with heterogeneous neointimal pattern who had been confirmed by coronary angiography and optical coherence tomography from January 2017 to December 2018 were retrospectively studied. The patients were assigned to DCB group(n = 6)or DES group(n = 12)according to the therapeutic strategies. The clinical data were collected. Major adverse cardiovascular events(MACEs)including cardiac death,nonfatal myocardial infrac⁃ tion,target vessel revascularization,and stent thrombosis were recorded during the period of follow ⁃ up. Results The clinical baseline data had no statistical difference between DCB group and DES group(P > 0.05). The target vessels showed by coronary angiography and the diameter of the preoperative reference vessels,length of lesions and degree of stenosis did not differ statistically the two groups(P > 0.05). The percentage of residual stenosis was postoperatively higher(P < 0.05)in DCB group than in DES group;there were no statistical difference in the areas of in ⁃stent,vascular lumen and neointima,neointimal load,lipid plaque,and thrombus ratio between the two groups(P > 0.05);the area of vascular lumen was larger in DES group than in DCB group(P < 0.05). The follow⁃period last 14(13,16.5)months in DCB group and 20(16,24)months in DCB group. No MACEs occurred in both groups during the follow⁃up period. Conclusions Both DCB and DES are safe and effective in treatment of IRS with heterogeneous neointimal pattern.

Objective To preliminary screen comorbidities related to short⁃term prognosis of patients with AECOPD and respiratory failure in four comorbidity scoring systems. Methods A retrospective observational study of the clinical data of 210 AECOPD patients with respiratory failure in our hospital was conducted. The comor⁃ bidities included in the Charlson comorbidity index,Elixhauser comorbidity index,COTE index and COMCOLD index were recorded. According to the survival status of patients during the hospitalization,they were divided into survival group and death group. Logistic regression was used to screen out comorbidities related to the prognosis of patients. Results Univariate analysis showed that age,myocardial infarction,congestive heart failure,dementia rheumatic disease,diabetes mellitus with end⁃organ damage,moderate/severe kidney disease,pulmonary fibrosis deficiency anemia and atrial fibrillation/flutter were statistically significant(P < 0.05). Multivariate logistic regres⁃ sion analysis showed that congestive heart failure(OR = 3.821,95%CI:1.518 ~ 9.619)and dementia(OR = 4.569 95%CI:1.237 ~ 16.877),diabetes mellitus with end⁃organ damage(OR = 6.842,95%CI:1.588 ~ 29.484),and moderate/severe kidney disease(OR = 3.274,95% CI:1.141 ~ 9.392)were independent risk factors related to prognosis. Conclusion Among the comorbidities associated with short⁃term prognosis in patients with respiratory failure due to AECOPD,congestive heart failure,dementia,diabetes mellitus with end⁃organ damage and moder-ate/severe kidney disease may be independent risk factors.

Objective To investigate the effect of percutaneous endoscopic lumbar discectomy(PELD on the functional recovery and trauma of patients with large lumbar disc herniation(LLDH). Methods A total of 92 LLDH patients in our hospital from June 2017 to July 2019 were selected for a prospective study. They were divided into observation group(= 46)and control group(= 46)according to a random number table. The control group underwent posterior lumbar interbody fusion(PLIF),and the observation group was treated with PELD technology. The operation conditions,complications,and serum trauma index[C⁃reactive protein(CRP),inter⁃ leukin⁃6(IL⁃6),β⁃endorphin(β⁃EP),prostaglandin E2(PGE2)]levels before and 1 d and 3 d after the opera⁃ tion,spinal cord function indexes(the incubation period of the tibial nerve and the common peroneal nerve,nerve conduction velocity)before operation,1 month and 3 months after operation were compared between the two groups. The postoperative follow⁃up was 1 year,and the curative effects and the Oswestry dysfunction index(ODI and visual analog pain score(VAS)before operation,6 months and 12 months after operation of the two groups were calculated. Results The radiation exposure of the observation group was higher than that of the control group but the operation time and hospital stay were shorter than those of the control group,and the intraoperative blood loss was lower than that of the control group(< 0.05). There was no significant difference in the incidence of complications between the two groups(> 0.05);serum CRP,IL⁃6,β⁃EP,and PGE2 levels in the observation group were lower than those in the control group at 1 and 3 days after surgery(< 0.05). The latency of the tibial nerve and common peroneal nerve in the observation group was lower than that of the control group at 1 and 3 months after operation,and the nerve conduction velocity was higher than that of the control group(P<0.05)The ODI and VAS scores of the observation group at 6 months and 1 year after operation were lower than those of the control group(< 0.05);there was no significant difference in the excellent and good rate of operation 1 year after operation in the two groups(> 0.05). Conclusion PELD technology can achieve surgical results equivalent to PLIF in the treatment of LLDH patients,and has the advantages of less trauma,short operation time,and quick postoperative recovery. Moreover,it has the advantages of less trauma,short operation time,and quicker recovery after surgery. It can reduce traumatic stress,and more effectively improve the symptoms of low back pain,spinal cord function,and lumbar function,but the radiation exposure is increased.

Objective To compare the analgesic effects between continuous serratus plane block and parathoracic block after thoracoscopic surgery in elderly patients. Methods Elderly patients undergoing thoraco⁃ scopic surgery in our hospital were selected and randomly divided into group S and group T. The patients in group S received continuous serratus plane block under ultrasound guidance before anesthesia induction,while group T continuous thoracic paraspinal block. Time of block operation and success rate of one puncture were recorded. VAS scores of at rest and cough pain 2,6,12,24 and 48 h after the operation were recorded. Pressing times of analgesia pump,the number of cases of sufentanil remedial analgesia and the score of satisfaction were recorded. PSQI scores 24 h before,24 h and 48 h after operation,the incidence of adverse reactions within 48 hours after the operation were recorded. Results Block operation time was significantly reduced(P<0.05);but success rate of one puncture and satisfaction rate was significantly increased(P<0.05)and the incidence of hypotension was signif⁃ icantly reduced(P<0.05)in group S when compared with those in group T. There was no significant difference in terms of VAS score of at rest and cough pain,times of pressing analgesia pump and the number of sufentanil reme⁃ dial analgesia cases between the two groups(P>0.05). Conclusion Both of the two methods can reduce the pain after thoracoscopic surgery in the elderly,and the analgesic effect of the two methods is similar. In contrast,contin⁃ uous serratus plane block has shorter operation time,higher success rate of puncture,lower incidence of hypoten⁃ sion during analgesia,improved sleep quality and higher satisfaction.

Objective To explore the diagnostic value of granzyme B stimulated by ESAT⁃6/CFP⁃10 in pediatric patients with tuberculosis. Methods A total of 86 children(26 children with ATB,20 with LTBI,20 Non⁃TB,20 healthy)from Wuhan Jinyintan Hospital and Wuhan Children′s Hospital from January to August 2019 were selected. ELISA was used to detect the level of GrB in peripheral blood after ESAT⁃6/CFP⁃10 stimulation and flow cytometry to detect the expression of GrB and IFN⁃ γ in CD4+ /CD8+ T cells. Results After ESAT⁃6/CFP⁃10 stimulation,the level of MTB⁃specific GrB of tuberculosis patients was significantly increased when compared with the control group(P < 0.001),especially,the level of MTB⁃specific GrB of LTBI patients was notably higher that of ATB patients(P < 0.001). GrB can be secreted jointly by CD4+ T and CD8+ T cells. Conclusion MTB⁃specific GrB may have great value in the differential diagnosis of active tuberculosis in pediatric patients.

Objective To investigate the correlation among serum Sphingosine ⁃1⁃phosphat(S1P),iron metabolism levels and the risk of peritoneal dialysis(PD)associated peritonitis. Methods Retrospective analysis of 160 patients with long ⁃term maintenance PD in our hospital from August 2016 to August 2019 was conducted. According to the occurrence of peritonitis,34 patients with PD ⁃ associated peritonitis were chosen as peritonitis group and 126 patients with PD⁃associated non⁃peritonitis as non⁃peritonitis group. Blood biochemical index,S1P level and iron metabolism index were compared between the two groups,and the incidence of PD associated perito⁃ nitis among different iron metabolites were evaluated. The risk factors of PD⁃associated peritonitis and the diagnos⁃ tic value of serum CRP,S1P and SF levels for PD⁃associated peritonitis were analyzed. Results The serum levels of S1P and total iron Binding force(TIBC)were significantly decreased,and the serum levels of c⁃reactive protein (CRP),serum ferritin(SF)and transferrin saturation(TSAT)were significantly increased in peritonitis group when compared with those in non⁃peritonitis group(All P < 0.05). Compared with that in iron deficient group and iron normal group,the incidence of PD⁃related peritonitis was significantly increased in ferritin group. Multivariate logistic analysis showed that CRP,S1P and SF were independent risk factors for PD associated peritonitis(P < 0.05). ROC curve showed that serum CRP,S1P and SF levels were of diagnostic value for PD ⁃ associated peritonitis (All P < 0.05). Conclusion Serum CRP,S1P and SF can be used as specific bio⁃markers for patients with PD⁃ associated peritonitis.

Objective To observe the effect of Guanxinshutong(GXST)Capsule on myocardial microcir⁃ culation and prognosis in patients with Acute ST⁃segment Elevation Myocardial Infarction(STEMI)after Emergency percutaneous coronary intervention(PCI). Methods A total of 210 patients with STEMI who underwent emergency PCI were randomly divided into two groups. One group was treated with conventional therapy(control group,n = 112)while the other with conventional therapy plus GXST Capsule(GXST group,n = 98). Cardiac ultrasound and myocardial contrast echocardiography were performed on admission and after 1 ⁃ year follow ⁃ up respectively. LVEDV,LVEF and contrast score index(CSI)were recorded and compared. they were followed up to determine whether MACE occurred 1 year later. Results CSI on admission had no difference between the two groups but was significantly lower in GXST group after 1⁃y follow⁃up(P = 0.039). MACE in GXST group(14.3%)was significantly lower than that in the control group(25.9%)(P = 0.041). Multivariate logistic regression analysis demonstrated that the CSI after 1 ⁃ y follow up was an independent predictor for MACE(HR = 4.21,P = 0.040). Conclusion GXST Capsule can improve myocardial microcirculation function in patients with STEMI after emergency PCI and it can also inhibit myocardial remodeling and reduce the occurrence of MACE. CSI level after 1⁃y follow⁃up is an inde⁃pendent risk factor for MACE.

Objective To observe the clinical effects of cardiopulmonary rehabilitationin ICU⁃AW patients under multidisciplinary assistance mode. Methods A total of 100 ICU ⁃AW patients admitted in the ICU of our hospital from June 2016 to June 2018 were selected as the control group,and cardiopulmonary rehabilitation treat⁃ ment intervention was followed. A total of 100 ICU patients hospitalized in ICU ⁃AW from July 2018 to July 2020 were selected as the observation group,and the MDT model was adopted to develop and implement cardiopulmonary rehabilitation treatment intervention. Mrc⁃score scores,length of mechanical ventilation,length of ICU stay,patient satisfaction,and occurrence of ventilator ⁃associated pneumonia(VAP)were observed on day 1,4,and 7 after mechanical ventilation in the 2 groups. Results On the first day of mechanical ventilation,there was no statistical difference in mrc⁃score score between the two groups(P > 0.05),but the MRC score of the 4th and 7th day of mechanical ventilation was higher than that of the control group(P < 0.05). The length of stay in ICU,length of stay in hospital duration of mechanical ventilation,and satisfaction scores of the observation group were all better than the control group,with statistically significant differences(P < 0.05). Conclusion The cardiopulmonary rehabilitation therapy under MDT mode can effectively restore the body activity ability of patients,shorten the stay time and overall treat⁃ ment time of patients in ICU,and improved patients′ satisfaction,is worth to be promoted and applicated.

Objective To explore the effect of Bio Master virtual scenario interactive rehabilitation com⁃ bined with combined breathing training on lung function recovery in elderly COPD patients. Methods A total of 124 elderly patients with COPD treated in our hospital from May 2019 to May 2020 were selected and divided into control group and observation group according to random number table method,with 62 cases in each group. The control group received conventional nursing intervention for elderly COPD patients,and the observation group Bio Master virtual scenario interactive rehabilitation with combined breathing training. The changes of FEV1,PEF and FEV1/FVC were detected by pulmonary function tester. SAS and SDS scores were used to evaluate the anxiety and depression of patients. The 6⁃minute walking distance of patients was recorded;the severity of dyspnea was evaluated by CAT,and the cognitive ability of patients by MoCA. Results After intervention,FEV1,PEF and FEV1/FVC were higher,but SAS and SDS scores lower in the observation group than those in the control group(P < 0.05). The 6⁃minute walking distance was longer;MoCA score was higher but CAT score lower in the observation group when compared with those in the control group(P < 0.05). Families were more satisfied in the observation group (P < 0.05). Conclusion Bio Master virtual scenario interactive rehabilitation combined with breathing training is effective for improving pulmonary function,reducing anxiety and depression and improving self⁃efficacy,cognitive ability and exercise endurance for elderly COPD patients.

Under normoxic conditions,enhanced glycolysis is one of the important characteristics of many tumors. This process is called aerobic glycolysis or Warburg effect,which is a necessary condition for virus replica⁃ tion and survival. Therefore,viral infection is very likely to enhance glycolysis Solution process. Tumor viruses induce aerobic glycolysis and lactic acid production during latent infection,suggesting that virus⁃induced glycolysis may play a key role in tumor ⁃ related progression. Viruses and virus ⁃encoded related proteins affect the glycolysis process by regulating glucose absorption and lactic acid metabolism,increasing glycolytic enzyme activity and regulating glucose metabolism signals. This article outlines the correlation between viral infections and glycolysis and specifically explains the changes in glycolysis caused by viral infections,with a view to finding new targets for antiviral therapy from glycolysis pathways.

Congenital heart disease(CHD),the most common congenital malformation,results mainly from mutual reactions of hereditary and environmental factors. Exposure to heavy metals as an environmental factor has been proved to induce DNA methylation,which is one of the epigenetic mechanisms regulating gene expres⁃ sion. The alterations in epigenetic level may be critical for the development of CHD. By literature review,the corre⁃ lations of exposure to heavy metals such as lead(Pb),cadmium(Cd),arsenic(As),nickel(Ni),mercury (Hg)with DNA methylation as well as CHD are comprehensively investigated,for further researches.
