腰椎融合术中应用聚醚醚酮棒与钛棒疗效对比的Meta分析

摘要:

目的 对腰椎融合术中应用聚醚醚酮棒(polyetheretherketone,PEEK)与钛棒的临床疗效进行 系统的评价与对比。方法 检索中国生物医学文献数据库、中国知网、维普数据库、万方数据库、Pubmed Embase Cochrane Library 中有关使用 PEEK 棒系统行腰椎融合术治疗腰椎退行性病变的临床试验,日期为各数据库建库至 2020 11 月。对纳入文献进行资料提取和质量评价,采用 NOS 量表对 10 项研究进行方法学质量评价。采用 RevMan 5.3 软件对其 JOA 评分改善率、VAS 评分、融合率、椎间隙高度进行 Meta 析。结果 纳入的 10 项研究中 8 项为高质量,2 项为中等质量。Meta 分析显示,术后中期的 JOA 改善率两组差异无统计学意义(P>0.05)。术后1月钛棒组腰痛VAS评分显著更低(P<0.05),而两组术后6、12个月 的腰痛VAS评分差异无统计学意义(P>0.05)。两组术后 1、6、12 个月的下肢痛 VAS 评分差异无统计学 意义(P>0.05)。PEEK 棒组在术后 3 月、6 月的融合率均显著高于钛棒组(P<0.05)。而两组术后 6、12 月的椎间隙高度差异无统计学意义(P>0.05)。结论 在腰椎融合术中,使用PEEK棒系统可促进椎体融 合,获得与钛棒系统相似的治疗效果。

Abstract:

Objective This article systematically evaluates and compares the clinical effects of Polyether⁃ ether ketone(PEEK)rods and titanium rods in lumbar fusion surgery. Methods A computer search of the Chinese Biomedical Literature Database,CNKI,VIP Database,Wanfang Database,PubMed,Embase and Cochrane Library for clinical trials on the use of PEEK rod in lumbar spine fusion for lumbar degenerative lesions,dated all databases will be built until November 2020. Data extraction and quality evaluation of all the included literatures were carried out,and the NOS scale was used to evaluate the methodological quality of 10 studies. Using RevMan5.3 software to conduct meta⁃analysis on JOA score,ODI score,VAS score,fusion rate,intervertebral space height and operation time. Results A total of 10 studies were included,of which 8 were high quality and 2 were medium quality. Meta⁃analysis showed that there was no significant difference between the two groups in the JOA improve⁃ ment rate in the middle postoperative period(P>0.05). In the VAS score of low back pain,the VAS score of low back pain in the titanium rod group was significantly lower at 1 month after surgery(P<0.05),and there was no significant difference at 6 months(P>0.05)and 12 months after surgery(P>0.05). In the lower limb pain VAS score,there was no significant difference in 1 month(P>0.05),6 months(P>0.05)and 12 months(P>0.05). The fusion rate in the PEEK rod group was significantly higher than that of the titanium rod group(P<0.05). There was no significant difference in the height of the posterior intervertebral space between the two groups at 6 months after surgery(P>0.05). Conclusion In lumbar fusion surgery,the use of PEEK rod system could promote verte⁃ bral body fusion,obtain similar therapeutic effects as titanium rod system,and has better lumbar function perfor⁃ mance in the middle postoperative period.
