Post⁃covid⁃19 syndrome(PCS)is a condition in which some patients suffer from persistent respiratory,motor,psychosomatic and other dysfunctional sequelae that cannot be explained by other diagnoses. Traditional forms of rehabilitation are poorly adhered to and costly. Therefore,how to reduce the burden of disease and increase the cure rate of PCS has become a key challenge to be addressed. In recent years,digital therapies with their excellent safety,compliance and affordability,have opened new opportunities for the treatment of PCS patients and are potential candidates for improving the outcome of PCS treatment. Currently,early clinical studies have reported the unique benefits of digital therapies in the treatment of PCS in both single and combination thera⁃ pies. Taken together,the aim of this article is to review the recent clinical trials and applications of digital thera⁃ pies in PCS and to discuss their safety and efficacy in order to provide some reference for clinical application.

This paper intends to discuss the effect ,mechanism and existing problems of air pollution on ART pregnancy based on relevant studies at home and abroad,so as to provide assisted pregnancy guidance for couples of childbearing age improve the probability of conception,and reduce the incidence of adverse effects caused by air pollution.

Chimeric antigen receptor(CAR)T cell therapy is a new immunotherapy in the field of tumors. Due to the immunosuppressive microenvironment and antigen expression of solid tumors,CAR⁃T treatment in solid tumors faces many challenges. Increasing evidences indicated that treatment with CAR ⁃T cells could significantly improve the prognosis and thus promising. However,the real characteristics and mechanism of CAR ⁃T cell com⁃ bined immunomodulators are not clear. This paper reviews the current situation and research progress of CAR ⁃ T cell combined immune modulators in solid tumors. We discuss the key principles of the combination strategy,sum⁃ marize the existing preclinical and clinical studies,and discuss the immune checkpoint inhibitors,interferon gene stimulating agonists,lenalidomide combined with CAR⁃T cells,and future prospects and challenges,aiming at exploring the benefit of more cancer patients.

Objective To investigate the effect of miR⁃143⁃3p on lipopolysaccharide(LPS)⁃induced alveolar epithelial cell injury by regulating the Chemokine(C⁃x3⁃C unit)ligand 1/Chemokine(C⁃x3⁃C unit)receptor 1(CX3CL1/CX3CR1)signaling pathway. Methods Peripheral blood serum were collected from patients with acute lung injury/acute respiratory distress syndrome (ALI/ARDS)and healthy individuals. The samples were classified into ALI/ARDS group and normal group. Dual luciferase experiment was performed to confirm the targeting relationship between miR⁃143⁃3p and CX3CL1. Human alveolar epithelial cells A549 were cultured in vitro,and the cells were separated into normal control group,LPS group,LPS+NC mimic group(transfected with NC mimic), LPS+miR⁃143⁃3p mimic group(transfected with miR⁃143⁃3p mimic),LPS+miR⁃143⁃3p mimic+pcDNA group (co⁃transfected with miR⁃143⁃3p mimic and pcDNA),and LPS+miR⁃143⁃3p mimic+pc⁃CX3CL1 group(co⁃trans⁃ fected with miR⁃143⁃3p mimic and pc⁃CX3CL1). QRT⁃PCR was performed to measure the expression levels of miR⁃143⁃3p,CX3CL1 and CX3CR1 mRNAs. Western blot was performed to measure CX3CL1 and CX3CR1 proteinexpression. The viability and apoptosis of alveolar epithelial cells were detected by MTT and flow cytometry. ELISA was performed to analyze the levels of tumor necrosis factor⁃α(TNF⁃α),interleukin⁃1β(IL⁃1β),interleukin⁃6 (IL⁃6),vascular endothelial growth factor(VEGF)and procalcitonin(PCT). Results Compared with the Normal group,the expression of miR ⁃143⁃3p(0.52 ± 0.05)in the peripheral blood of the ALI/ARDS group decreased and the mRNA(1.79 ± 0.08),(1.65 ± 0.06)and protein(1.03 ± 0.15),(0.98 ± 0.12)expression of CX3CL1 and CX3CR1 increased(P < 0.05). Compared with the Control group,the expression of miR⁃143⁃3p in the A549 cells of the LPS group decreased,the cell viability decreased,and the apoptosis rate[(23.56 ± 0.85)%]and the contents of TNF⁃α,IL⁃1β,IL⁃6,VEGF,and PCT increased(P < 0.05). After miR⁃143⁃3p was overexpressed cell viability was obviously increased,and the apoptosis rate[(13.74 ± 0.69)%]and the contents of TNF⁃α,IL⁃ 1β,IL⁃6,VEGF,and PCT were obviously reduced(P < 0.05). The results of dual luciferase assay showed that miR⁃143⁃3p directly negatively regulated the expression of CX3CL1. CX3CL1 overexpression significantly reversed the effect of miR⁃143⁃3p overexpression on LPS⁃induced A549 cell damage and the inhibitory effect on CX3CR1 mRNA and protein expression(P < 0.05). Conclusion MiR⁃143⁃3p can reduce the injury of alveolar epithelial cells induced by LPS by inhibiting the CX3CL1/CX3CR1 signaling pathway.

Objective To study the effect of salidroside(Sal)on the inflammatory response and oxidative stress of coronary arteries in Kawasaki disease and its mechanism. Methods The cells were divided into normal control group,TNF⁃ α group,50 μmol/L and 100 μmol/L Sal group. The effect of Sal and TNF⁃ α at different concentrations on HCAEC survival was detected by CCK⁃8. The relative protein expression levels of phosphorylated phosphatidylinositol 3 kinase(p⁃PI3K),phosphorylated protein kinase B(p⁃Akt),phosphorylated NF⁃ κBp65 (p⁃P65),interleukin 6(IL⁃6),IL⁃1β,tight junction protein(ZO⁃1)and vascular endothelial calherin(VE⁃cad⁃ herin)were detected by Western blot. The expression levels of inflammatory factors and adhesion molecule mRNAs were detected by RT⁃qPCR. Intracellular reactive oxygen species(ROS)production was observed by DCFH⁃DA fluorescent probes. Intracellular superoxide dismutase(SOD)and glutathione peroxidase(GSH⁃Px)were analyzed with spectrophotometers. Results Compared with the control group ,the protein expression levels of p⁃PI3K p⁃Akt,ZO⁃1 and VE⁃cadherin were significantly reduced(P < 0.05),the protein expression levels of p⁃P65,IL⁃6 and IL⁃1β were significantly increased(P < 0.05),and the mRNA expression levels of IL⁃6,IL⁃8,IL⁃1β,ICAM⁃ 1,and VCAM ⁃1 were significantly increased(P < 0.05). The generation of ROS was significantly increased and the viability of SOD and GSH⁃Px were significantly decreased in the TNF⁃α group(P < 0.05),while Sal could significantly reverse these effects. Conclusion Salidroside can attenuate TNF⁃α⁃induced inflammatory response and oxidative stress levels in HCAEC cells probably by way of activating PI3K/Akt and inhibiting NF⁃ κB signal⁃ ing pathway.

Objective Explore the pathological mechanism of stroke hyperalgesia by using a rat model of thalamic BDNF signaling. Methods Wistar male rats(8 weeks old)were used in the study. The model of focal cerebral ischemia and reperfusion in rats were established by middle cerebral artery occlusion (MCAO). The change of thermal pain threshold were assessed by hot plate tests. The rats were divided into two groups randomly SHAM and central post ⁃stroke pain(CPSP). Rats with hyperalgesia 3 days after surgery were screened for the post⁃stroke central pain group. TTC staining,HE staining,Nissl staining and Neun immunohistochemical staining were used to assess thalamus damage. Immunofluorescence was used to detect the number of activated microglia and BDNF. The expression of BDNF,TrkB and GABAAR proteins was determined by Western blot. Results When compared to the SHAM group,the CPSP rats had a significantly higher neurological deficit score,a lower heat pain threshold,thalamic VPL pathology injury,a lower number of nissl body and Neun positive expression higher microglia and BDNF expression,higher thalamic BDNF and Trkb protein expression,and lower GABAAR protein expression. Conclusions Ischemia/reperfusion cerebral injury can result in hyperalgesia,with a 30% dis⁃ ease incidence. The concomitant BDNF ⁃Trkb of thalamic microglia activation plays a major role in mediating the pathological progression of hyperalgesia after stroke.

Objective To investigate the relationship between KRAS and BRAF gene mutations and prog⁃ nosis in colorectal cancer patients. Methods From January 2017 to January 2020,tissue samples from 60 patients with colorectal cancer submitted at the pathology department of our hospital were examined for KRAS and BRAF gene mutations using next⁃generation sequencing(NGS)technology. Analysis was done on the relationship between patient clinic pathological features and KRAS and BRAF gene mutations,as well as how these factors affected prog⁃ nosis. The COX regression model was employed to examine the variables affecting OS in CRC patients. Results The KRAS gene mutation rate of 60 colorectal cancer patients was 41.7%(25/60),t with the second exon mutation being the most common,and the BRAF gene mutation rate was 8.3%(5/60),all of which were exon 15 mutations with no co ⁃ mutation found. KRAS gene mutation is significantly related to pathological type and N stage,while BRAF gene mutation is mainly related to pathological type and distant metastasis. The disease control rate of KRAS mutation patients was significantly lower than that of wild ⁃type patients. Pathological type,T stage,N stage,and BRAF mutation are all independent factors that influence OS. BRAF mutation patients had significantly lower OS and PFS than wild⁃type patients,while KRAS mutation had no effect on OS and PFS. Conclusion KRAS mu⁃ tation rates were high in colorectal cancer,and they were closely related to patients′ degree of differentiation and lymph node metastasis. BARF mutation rates was low,which was related to the degree of differentiation,distant me⁃ tastasis,and poor prognosis.

Objective To observe the clinical efficacy of iRoot SP single ⁃tip filling for diabetic patients complicated with chronic apical inflammation. Methods Eighty diabetic patients complicated with chronic apical inflammation were selected and divided into two groups according to root canal filling procedure. The observation group(n = 40)was treated with iRoot SP single⁃cone obturation,and the control group(n = 40)was treated with warm gutta ⁃percha vertical condensation with AH Plus sealer. The two groups were followed up and compared for emergency reactions within 1 week after root canal filling and the clinical efficacy at 3 months and 1 year after obtu⁃ ration. Results Significant differences in the degree of emergency reactions within 1 week after obturation was noted(Z = 2.806,P = 0.037),which were lower in the observation group than in the control group. No statistically significant difference in the efficacy rate between the two groups at 3 months and 1 year after operation was found (χ2 = 0.000,P = 1.000);There was a significant difference in clinical efficacy between the two groups at 3 months after obturation(Z = 2.269,P = 0.023),and the cure rate of the observation group was higher than that of the control group. However,no significant difference in clinical efficacy between the two groups at 1 year after operation was found(Z = 0.612,P = 0.54). Conclusion iRoot SP single⁃cone obturation and warm gutta⁃percha vertical condensationhad have similar therapeutic effect for diabetes complicated with periapical inflammation,but iRoot SP single⁃cone obturation was beneficial to reduce the occurrence of emergency reactions after root filling,and the periapical tissue healing after treatment may be faster than warm gutta⁃percha vertical condensation

Comparison of the efficacy of peripheral nerve pulsed radiofrequency and short⁃term spinal cord stimula⁃ tion in the treatment of postherpetic neuralgia

Objective To compare the efficacy and safety of peripheral nerve pulsed radiofrequency and short ⁃term spinal cord stimulation in the treatment of postherpetic neuralgia. Methods The research subjects were 109 patients with postherpetic neuralgia who were treated in the Department of Pain at Southwest Medical University′s Affiliated Hospital from January 2020 to January 2022. Based on propensity score matching,peripheral nerve pulse radiofrequency(26 cases in the radiofrequency group)was compared with that in the radiofrequency group. Short⁃term spinal cord stimulation(26 cases in the electrical stimulation group). The electrical stimulation group received treatment with implanted electrodes under the supervision of DSA,while the radiofrequency group received treatment with peripheral nerve pulsed radiofrequency under the supervision of ultrasonography. The Numerical Rating Scale(NRS)and Pittsburgh Sleep Quality Index(PSQI)scores were compared at 1 week after surgery and at 1,3,and 6 months after surgery,and complications were observed. Results The NRS and PSQI scores of the two groups decreased after operation compared to preoperative(all P < 0.05). The NRS score of the electrical stimulation group was lower at 1 month after surgery compared to the radiofrequency group(P < 0.05); There was no significant difference in the NRS score between the two groups at 1 week,3 and 6 months after surgery. The NRS score of the electrical stimulation group was lower at 1 month after surgery compared to the radiofrequency group(P < 0.05). Conclusion Both peripheral nerve pulsed radiofrequency and spinal cord electrical stimulation can effectively relieve pain in postherpetic neuralgia patients,there was no difference in mid⁃ and long⁃term effica⁃ cy,and the incidence of peripheral nerve pulsed radiofrequency under ultrasound guidance is low.

Influencing factors of clopidogrel high on⁃treatment platelet reactivity in non⁃diabetics

Objective Patients with clopidogrel high on⁃treatment platelet reactivity(HTPR)have a higher risk of recurrent vascular events. We attempted to explore risk factors associated with Clopidogrel HTPR in non⁃ diabetics. Methods A total of 1 822 non⁃diabetics with vascular events received clopidogrel were included. Maxi⁃ mum fibrin clot strength induced by ADP(MAADP)was identified by Thrombelastography,and CYP2C19 genotype was determined by DNA microarray. Results Clopidogrel HTPR was found in 13.2% of the patients(n = 241). Multivariate analysis of logistic regression showed that gender,age,platelet distribution width(PDW),CYP2C19 were associated with Clopidogrel HTPR. The Clopidogrel HTPR predictive model had a good calibration by the Hosmer⁃Lemeshow goodness⁃of⁃fit test(P = 0.252),and the area under cure of ROC was 0.704(95%CI:0.670 ~ 0.738). Conclusions Clopidogrel HTPR in non⁃diabetics was significantly influenced by gender,age,PDW and CYP2C19 polymorphisms. The predictive model of HTPR has a possibly helpful discrimination and good calibration.

目的 探讨中低位局部进展期直肠癌行新辅助放化疗后临床完全缓解直肠癌患者采取等 待观察(wait & watch,W&W)策略的预后及影响因素分析。方法 回顾性纳入2015年1月至2019年12月在 河北北方学院附属第一医院普通外科和河北医科大学第四医院外二科行新辅助放化疗后临床完全缓解 中低位局部进展期直肠癌患者,比较采取 W&W 策略(23 例)与全直肠系膜切除术(TME)(30 例)患者的 预后情况,采用单因素和多因素Cox 比例风险回归模型分析W&W 患者无病生存期和总体生存的影响因素。 结果 中位随访时间为 36 个月,W&W 患者与 TME 患者的中位无病生存期、中位总生存期、3 年无病生存 率、3 年总生存率及复发率分别为 32.6 个月、35.7 个月、80.43%、89.13%,19.57% 35.1 个月、38.4 个月、 83.33%、93.33%、16.67%,差异无统计学意义(P > 0.05);单因素分析显示,肿瘤下缘距肛缘距离≤ 4 cm、淋 巴转移阳性W&W患者的无病生存期明显缩短,差异有统计学意义,P < 0.05;淋巴转移阳性W&W患者的总生 存期明显缩短,差异有统计学意义(P < 0.05);多因素分析显示,肿瘤下缘距肛缘距离≤ 4 cm(HR = 16.371 95%CI:3.2369 ~ 134.542,(P = 0.003)、淋巴转移阳性(HR = 5.863,95%CI:1.127 ~ 12.334,P = 0.006)是 W&W 患者无病生存期缩短的独立危险因素,淋巴转移阳性(HR = 4.602,95%CI:1.053 ~ 15.772,P = 0.038 W&W患者总生存期缩短的独立危险因素。结论 新辅助放化疗后临床完全缓解直肠癌患者采取W&W 策略与TME的预后无明显差异,肿瘤下缘距肛缘距离及淋巴转移状态是W&W患者预后的影响因素。

Predictive value of D⁃dimer in prevention of PTE after VTE with parenteral anticoagulants

Objective To observe the predictive value of D⁃dimer in the prevention of pulmonary thrombo⁃ embolism(PTE)after venous thromboembolism(VTE)by parenteral anticoagulants. Methods Ninety⁃four patients with high risk of VTE or other conditions with anticoagulant indicators and low risk of bleeding were given parenteral prophylactic anticoagulation for 7 ~ 12 days from May 2020 to June 2021. According to whether with PTE within 6 months after anticoagulation,the patients were divided into PTE group and non⁃PTE group. Age,sex,Padua score changes of platelet,liver function,renal function and the predictive value of D ⁃ dimer were analyzed. Results Greater age in PTE group(79.09 ± 6.78)was found when compared with that in non⁃PTE group(74.49 ± 10.45 P < 0.05). The level of D⁃dimer in PTE group[2 159.5(1 252.25,4 497.75)(μg/L)]was significantly higher than that of non ⁃PTE group[683.5(516,1 172.75)(μg/L)](P < 0.001). There were no significant changes in Padua score before anticoagulation,and D⁃dimer,platelet,liver function and renal function before and after anti⁃ coagulation. There was no statistical difference in the above indexes before and after anticoagulation in the non⁃PTE group,but there were changes in platelet,D⁃dimer,aspartate transaminase and creatinine in the non⁃PTE group (P < 0.01 or P < 0.05). Age was positively correlated with D⁃dimer after anticoagulation(rs = 0.266,P = 0.010); Padua score was positively correlated with D⁃dimer before and after anticoagulation(rs = 0.520,P < 0.001;rs = 0.236 P = 0.022),and D ⁃ dimer level after anticoagulation was positively correlated with whether with PTE or not(rs = 0.512,P < 0.001). The area under ROC curve was 0.849(95%CI:0.760 ~ 0.938,P < 0.001);best cut off value1 026.5 μg/L,with sensitivity and specificity of 90.9%,and 72.2% respectively. Conclusion The level of D⁃dimer after the prevention of VTE by parenteral anticoagulantsin the PTE group does not decrease significantly but is significant higher than that in the non⁃PTE group.High D⁃dimer level has high predictive value for the occurrence of PTEwithin half a year.

Objective The aim of this study was to compare the clinical effects of oblique lumbar inter⁃ body fusion(OLIF)combined with lateral plate fixation(OLIF ⁃ LP)or posterior bilateral pedicle screw fixation (OLIF⁃BPS)in the treatment of single⁃segment lumbar degenerative diseases(LDD). Methods A total of 64 patients with single ⁃ segment LDD who received lumbar decompression and fusion surgery in our hospital from May 2020 to March 2021 were selected. The patients were divided into OLIF⁃LP group(28 cases)and OLIF⁃BPS (36 cases)according to the surgical methods. The operation time,intraoperative bleeding volume,creatine kinase (CK),average hospitalization days and complications of patients in both groups were recorded. VAS for lumbar and leg pain and ODI scores were used to compare the clinical efficacy of the two groups. Disk height(DH),Cross⁃ sectional area(CSA)and lumbar lordosis(LL)were measured at different time points by imaging,and the inter⁃ body fusion was evaluated. Results The operation time(79.61 ± 14.11)min,the intraoperative bleeding volume (64.86 ± 15.01)mL in OLIF⁃LP group were less than those in OLIF⁃BPS group(108.61 ± 20.69)min,(99.58 ± 22.28)mL. The CK at 1 and 7 days postoperation were significantly lower in OLIF⁃LP group than those in OLIF⁃ BPS group(P < 0.05). The VAS score and ODI of low back pain in both groups were significantly lower than thosepreoperation. The VAS score of low back pain in OLIF ⁃ LP group was significantly lower than that in OLIF ⁃BPS group at 7 days postoperation(P = 0.04),but there was no significant difference between the two groups at the last follow⁃up(P = 0.43). DH,LL and CSA at each time point postoperation were significantly improved as compared with those preoperation(P < 0.001),there was no significant difference between the two groups(P > 0.05). At half a year postoperation,the fusion rate was comparable(89.29% vs. 91.67%,P = 0.75). Complications occurred in 5 cases(17.86%)and 7 cases(19.44%)in OLIF⁃LP group and OLIF⁃BPS group respectively,and the incidence was not statistically significant(P = 0.87). According to the subsidence criteria,the subsidence rate of OLIF⁃LP group was 21.43%(6/28),and that of OLIF⁃BPS group was 19.44%(7/36). Without symptom in all subsidence cases. Conclusions Under the premise of strictly grasping the surgical indications,OLIF ⁃LP technique could obtain comparable effective outcomes as OLIF⁃BPS surgery for the treatment of LDD,and patients with OLIF⁃LP have less trauma,less pain after surgery.

Objective To investigate the effect of brain⁃computer interface(BCI)training on the improve⁃ ment of lower limb motor function in patients with spinal cord injury. Methods Sixty patients with spinal cord injury who were treated in the Department of Rehabilitation Medicine of our hospital were selected and divided into control group and study group,with 30 cases in each group. The modified Barthel index(MBI)was used to evaluate the activities of daily living(ADL),and functional ambulation category scale(FAC)to evaluate walking function of the two groups. The resting⁃state functional magnetic resonance imaging(rs⁃fMRI)was performed,and Pearson test was used for correlation analysis. Results The BBS score,ASIA motor and sensory score and MBI score of the two groups after 2 and 4 weeks of intervention were higher than those before,and the changes were more obvious after 4 weeks of intervention(P < 0.05);after 2 and 4 weeks of intervention,the numbers of patients with quadriceps muscle and hamstring muscle MMT and FAC of grade 3 or above ,and the number of patients with FAC of grade 3 or above increased in both groups,especially after 4 weeks of intervention(P < 0.05);after 2 and 4 weeks of intervention,the above indexes of the study group changed significantly(P < 0.05). After 4 weeks of intervention,the FC values between BA4L/BA41R and BA5L/BA48R in the study group were significantly higher than those before(P < 0.05),and were positively correlated with BBS score,ASIA motor and sensory score and MBI score(P < 0.05). Conclusion BCI training on the basis of routine rehabilitation is helpful to improve motor function,sensory function and the quality of life of patients with spinal cord injury,and has a certain role in pro⁃ moting the improvement of balance and walking function.

Objective The study tried to investigate the predictive value of serum omentin⁃1(Omentin⁃1), transforming growth factor ⁃ β1(TGF ⁃ β1)and ferritin(SF)levels of nonunion after thoracolumbar osteoporotic fracture internal fixation. Methods A total of 358 patients with thoracolumbar osteoporotic fractures were treated with vertebroplasty and internal fixation. Serum levels of Omentin⁃1,TGF⁃β1 and SF were measured before and 4 weeks after operation,the influencing factors of nonunion after internal fixation and predictive values of single and combined detection of the above indexes for postoperative bone nonunion were analyzed. Results The levels of serum Omentin⁃1 and TGF⁃β1 in all patients at 4 week after operation were higher than those before operation,and the serum SF level was lower than that before operation(P < 0.05). After 6 months of follow⁃up,the incidence rate of bone nonunion was 5.59%. 4 weeks after operation,the serum Omentin ⁃1 and TGF ⁃β1 level in bone healing group were higher than those in bone nonunion group but the serum SF levels were lower than those in bone non⁃ union group(P < 0.05). Blood supply damage,4 weeks after operation,serum Omentin⁃1,TGF⁃β1 and SF levels were the influencing factors of bone nonunion after internal fixation of thoracolumbar osteoporotic fractures(P < 0.05). The receiver operating characteristic(ROC)curve results showed that the sensitivity and area under curve (AUC)of omentin⁃1,TGF⁃β1 and SF levels 4 weeks after surgery were higher than those of omentin⁃1,TGF⁃β1 and SF levels alone in predicting postoperative nonunion(P < 0.05). Conclusion Serum levels of Omentin ⁃1 TGF⁃β1 and SF at 4 weeks after operation have good predictive value for bone nonunion after thoracolumbar osteo⁃ porotic fracture internal fixation but the combined predictive value is higher.

Objective To investigate the effects of IABP in combination with rhBNP in patients undergo⁃ ing emergency percutaneous coronary intervention(PCI)for acute anterior myocardial infarction. Methods The study included 91 patients who had an acute anterior myocardial infarction and needed emergency PCI. They were divided into three groups based on the application of IABP and rhBNP:group A received IABP combined with rhBNP,group B received IABP,and group C received rhBNP. Cardiac function index,average length of stay adverse events during hospitalization and post⁃discharge follow⁃up were all examined. Results (1)the peak NT⁃proBNP concentration in group A was lower than that in group B and C(P < 0.05).(2)There were no significant differences in LVDD or LVEF among the three groups at admission(P > 0.05). During the follow ⁃up,LVDD in group A was smaller than that in group B and group C(P < 0.05). In addition,compared with baseline,the LVDD decreased and LVEF increased in group A at 6 months after discharge(P < 0.05). However,there were no statistical differences in LVDD and LVEF between groups B and C when compared to baseline(P > 0.05).(3)During hospital⁃ ization,the average length of stay and adverse events did not differ statistically(P > 0.05).(4)During the follow⁃up period,the incidence of heart failure requiring hospitalization was 2.17% in group A,17.86% in group B,and5.88% in group C,and the difference was statistically significant(P < 0.05). However,there was no significant difference in revascularization among the three groups(P > 0.05). Conclusion The combination of IABP and rhBNP can improve cardiac function and reduce the incidence of heart failure requiring hospitalization after dis⁃ charge in patients with acute anterior STEMI undergoing emergency PCI.

Objective To compare the efficacy of acute traumatic cervical spinal cord injury patients with AIS grade C and D under different surgical timing,and to analyze the related factors affecting the surgical timing. Methods Retrospective analysis was performed on the patients with acute traumatic cervical spinal cord injury (TCSCI)in our hospital from March 2013 to April 2021,who rated as American Spinal Injury Association Injury Scale(AIS)grade C and D. The patients who had been followed up for more than 6 months in the last outpatient clinic were divided into three groups based on operation timing:group A less than 48 hours,group B more than 48 hours and less than 1 week,and group C more than 1 week. Collect the case data in the three groups after admission,including:gender,age,injury cause,injury plane,AIS grade,AMS score(ASIA Motor Score)at admission,AMS at 6 months after surgery,and improvemental score of AMS after surgery. Compare and contrast the three groups′ AMS improvement scores following surgery. Regression analysis was performed on related factors affecting the surgical timing. Results A total of 79 patients were enrolled,including 28 patients with AIS grade C and 51 patients with AIS grade D at admission as well as 60 males and 19 females with an average age of(49.87 ± 11.18)years old. There were 11 high fall injuries,33 motor vehicle collisions,8 heavy bruises,and 27 cases of falling down. The damage plane with C1⁃4 and C5⁃8 was 60 and 19 cases respectively. Group A had 21 cases Group B had 31 cases,and Group C had 27 cases. There were no significant differences in gender,age,the causes of trauma,AIS grade,preoperative and postoperative AMS,and injury plane among the three groups. At the lastpostoperative follow⁃up,there were significant differences in AMS scores between groups A,B,and C(P = 0.04). There was no statistically significant difference between groups B and C. Ordered multiple Logistic analysis showed that the factors that prolonged the timing of surgery included:early respiratory complications(OR = 2.89),associ⁃ ated injury(OR = 4.39),basic diseases of internal medicine(OR = 6.25),the number of white blood cells in labo⁃ ratory was more than 10 × 109 /L(OR = 5.50). Conclusions Acute traumatic cervical spinal cord injury patients with AIS grades C and D fare much better within 48 hours of surgery. The most significant factor contributing to the operation time delay was basic diseases of internal medicine.

Objective To assess the changes in ultrasonic indexes in Hashimoto′s thyroiditis patients of different ages,and to determine the ultrasonic diagnostic indices and threshold value by using conventional ultra⁃ sound and shear wave elastography(SWE). Methods A total of 147 patients with clinically diagnosed HT in our hospital were selected as a HT group,while 124 healthy people were selected as a control group. Both groups were divided into subgroups of ages 21 ~ 35(47,40 cases),36 ~ 50(53,43 cases)and 51 ~ 65(47,41 cases). All the patients underwent ultrasonography. Thyroid volume(TV),isthmus thickness,peak systolic velocity(PSV of superior thyroid artery,echo grade,blood flow grade,SWEmean and SWVmean were recorded. The differences of ultrasonic parameters in different age groups of HT patients and controls were compared,and the correlation between ultrasound parameters and age were analyzed. Ultrasonic parameters between HT and controls (overall and in different age groups)were compared. The ROC curves were used to determine the ultrasonic diagnostic indexes and cutoff values of HT in different age groups. Results In different age groups of HT patients,there were statistically significant differences in thyroid echo grade,SWEmean and SWVmean(P < 0.05);while in the control group,there were significant differences in SWEmean and SWVmean(P < 0.05). The age of HT patients and controls was positively correlated with SWEmean and SWVmean(r = 0.231,0.236;r = 0.476,0.408;P < 0.05). The ROC showed:in all the subgroups,SWEmean had the highest AUC value for diagnosing HT,and as the cutoff values were 13.53,13.78,15.96 kpa,both sensitivity and specificity were higher,which were 95.7% and 80.5%,96.2% and 89.4%,89.4% and 62.5%,respectively. Conclusions Conventional ultrasound and SWE can be used to objectively assess the changes in thyroid gland in HT patients of different ages. Thyroid echo decreases whereas elasticity increases with age. SWEmean can be used as the best ultrasound diagnostic index for HT patients aged 21 ~ 35,36 ~ 50 and 51 ~ 65,with its diagnostic cutoff values of 13.53,13.78,and 15.96 kpa,respectively.

Objective To explore the diagnostic value of China Ultrasound Thyroid Imaging Reporting and Data System,CEUS and BRAFV600E gene in Thyroid Cytopathology Reporting System classⅢthyroid nodules. Methods A total of 120 cases of thyroid nodules with preoperative cytopathological diagnosis of TBSRTC classⅢ and postoperative pathology were selected. To compare the diagnostic efficacy of the three methods alone or in com⁃ bination,CEUS,BRAFV600E gene,and ultrasound C⁃TIRADS grading were performed. Results Single diagnostic method:C⁃TIRADS has higher sensitivity and accuracy than CEUS(P < 0.05)and BRAFV600EP > 0.05);BRAFV600E has higher specificity than C⁃TIRADS and CEUS(P < 0.05);Combined diagnosis:The sensitivity,specificity and accuracy of C⁃TIRADS+BRAFV600E were higher than those of C⁃TIRADS+CEUS and CEUS+BRAFV600EP < 0.05), and the difference was statistically significant. Conclusion Through the use of C ⁃ TIRADS and the BRAFV600E gene,the diagnostic coincidence rate of benign and malignant TBSRTC class Ⅲ nodules is increased.

Objective To investigate the molecular network characteristics of three major subtypes among HIV positive men who have sex with men(MSM)in Shenzhen in order to provide clues for controlling the transmis⁃ sion of HIV. Methods Data and blood samples were collected from newly reported MSM who infected with HIV in Shenzhen from 2017 to 2019. Subtype analysis was performed by online tools and Mega software. Pairwise genetic distances were calculated by HyPhy. Molecular network visualization was constructed by Cytoscape,and statistical data analysis was used by SPSS software. Result A total of 2 848 genetic sequences of pol region of HIV were obtained from newly reported HIV positive MSM and successfully classified into 11 subtypes,of which three major subtypes including CRF07_BC,CRF01_AE,CRF55_01B accounted for 91.22%. The overall access rate of the three main subtypes was 32.32%,forming 194 clusters and 1291 edges. Entering the molecular network,MSM infected with HIV were mainly under 35 years old,Han nationality,unmarried,from other provinces or foreign countries commercial service,no sexual history,201 ~ 500 CD4+ T cells/μL,MSM infected with CRF01_AE mainly concen⁃ trated in high school or technical secondary school,MSM infected with CRF07_BC mainly concentrated in junior col⁃ lege or above,and MSM infected with CRF55_01B mainly concentrated in junior high school or below. MSM infected with CRF01_AE,the access rate of the group of CD4 counts with 201⁃500 cells/μL was the highest. MSM infected with CRF07_BC were prone to forming large clusters. MSM infected by CRF55_01B had the highest access rate in junior high school and primary school. Conclusion MSM infected with CRF01_AE,the transmission risk of CD4+ T cells was highest in the range of 201 ~ 500 /μL,and the transmission risk was higher in CRF07_BC clusters. MSM infected with CRF55_01B,the transmission risk was highest in junior high schools and primary schools.

Endometriosis is a complicated and chronic inflammatory disease. Despite the fact that several therapeutic strategies for the treatment of endometriosis have been established,there is a high recurrence rate after drug and surgical treatments.Macrophages are the primary immune cells in the human body,and recent research has found that macrophage recruitment in endometriotic lesions,as well as their polarized phenotype,contribute to the development and maintenance of endometriosis. The role of different polarized phenotypes of macrophages in the etiology and pathophysiology of endometriosis remains unknown. Here,we review the mechanism of macrophage M1/M2 phenotypic transition in heterotopia,explore the role of macrophages with different phenotypes in the endo⁃ metriosis⁃related microenvironment,and shed new scientific light on EMs';therapeutic approaches.

Glioma is the most common primary tumor of the central nervous system. Due to the lack of targeted drugs,tumor chemotherapy is ineffective. Doxorubicin is one of the widely used chemotherapeutic drugs in clinical practice. According to different surface modifications of doxorubicin liposomes,the application of doxorubi⁃ cin liposomes in the targeted therapy of brain glioma is introduced. Most of the doxorubicin liposomes are used in basic research,and some in clinical trials. Therefore,this paper mainly describes the surface modification of doxo⁃ rubicin liposome and its application in the treatment of glioma.
