摘要:

间歇性禁食(intermittent fasting,IF)是一种周期性能量限制的饮食方式,在改善肥胖个体葡 萄糖稳态、减脂减重、减轻慢性低度炎症等方面,已显示出与长期禁食和热量限制等效的代谢与免疫益 处。因其具有更好的耐受性和依从性,有望成为肥胖患者更易接受的饮食干预方式。研究表明,IF 介导 的代谢益处很大程度上归因于胰岛β细胞功能的调控和白色脂肪组织(white adipose atissue,WAT)的褐 变。重要的是,自噬途径和肠道菌群均在其中发挥着重要作用。且代谢的改善及免疫相关基因表达的调 控对于 IF 介导的免疫效益也至关重要。本文结合上述机制从糖代谢、脂代谢、免疫三方面来综述间歇性 禁食在肥胖防治中的作用,旨在为肥胖及相关代谢性疾病的生活干预和临床转化提供新思路。

Abstract:

Intermittent fasting(IF)as a periodic energy restriction,has been shown to provide metabolic and immune benefits equivalent to prolonged fasting or calorie restriction in enhancing glucose homeostasis,decreasing fat and weight,and cutting down chronic low⁃grade inflammation in obese individuals. It is expected to be a more acceptable dietary intervention method for obese patients by virtue of its better tolerance and compliance. Studies have indicated that the IF⁃mediated metabolic benefits are attributable in large part to the regulation of pancreatic β⁃cell function and the browning of white adipose tissue(WAT). What counts is that both autophagy pathway and gut microbiota play a significant role. Furthermore,the amelioration of metabolism and the regulation of immune⁃ related gene expression are also crucial to IF⁃mediated immune benefits. By combining the aforesaid mechanisms this paper reviews the role of IF in the prevention and treatment of obesity from the three aspects,namely glucose metabolism,lipid metabolism and immunity,in the hope of providing novel insights into lifestyle intervention and clinical application on obesity and related metabolic diseases.
