Analysis of the application value of Zero-PVA versus CP in anterior cervical discectomy and fusion for single-level cervical spondylosis based on imaging indicators

摘要:

目的 基于影像学指标分析可变角度零切迹椎间融合器(Zero-PVA)、钛板椎间融合器(CP)两种融合器在单节段颈椎病患者颈椎前路椎间盘切除融合术(ACDF)中的应用价值。 方法 回顾性选取2021年1月至2025年9月于贵州省人民医院行ACDF治疗的204例单节段颈椎病患者的临床资料,根据术中融合器类型差异,将使用Zero-PVA患者纳入Zero-PVA组(n = 101),使用CP患者纳入CP组(n = 103)。比较两组围术期指标、手术前后影像学指标、Bridwell融合情况、日本骨科协会(JOA)评分、颈椎 功能障碍指数(NDI),并统计术后3个月内并发症发生情况。 结果 Zero-PVA组术中出血量、住院时间较CP组少(P < 0.05)。术后1周及术后3个月两组颈椎前凸角、融合节段高度、融合节段Cobb角及C2-7Cobb角较术前增加(P < 0.05),且术后1周及术后3个月Zero-PVA组融合节段Cobb角大于CP组(P < 0.05)。术后3个月两组Bridwell融合分级比较,差异有统计学意义(P < 0.05)。术后3个月两组JOA评分高于术前,NDI低于术前(P < 0.05),但两组间手术前后JOA评分、NDI比较,差异无统计学意义(P > 0.05)。Zero-PVA组术后并发症发生率低于CP组(P < 0.05)。 结论 在单节段颈椎病ACDF中,Zero-PVA与CP均能有效改善患者颈椎功能,但Zero-PVA在减少术中创伤、改善融合节段Cobb角、促进骨融合及降低术后并发症发生风险方面效果更佳。

Abstract:

Objective To analyze the application value of variable-angle zero-profile interbody fusion cage (Zero-PVA) and titanium plate fusion cage (CP) in anterior cervical discectomy and fusion (ACDF) for single-level cervical spondylosis based on imaging indicators. Methods Clinical data of 204 patients with single-level cervical spondylosis who underwent ACDF in our hospital from January 2021 to September 2025 were retrospectively collected. Based on the type of cage utilized during the surgery, the patients were divided into the Zero-PVA group (n = 101) and the CP group (n = 103). Perioperative indicators, imaging indicators, Bridwell grades of interbody fusion, Japanese Orthopaedic Association (JOA) scores, Neck Disability Index (NDI), and complications within 3 months after the operation were compared between the two groups. Results The Zero-PVA group exhibited less intraoperative blood loss and a shorter hospital stay compared to the CP group (P < 0.05). At 1 week and 3 months post-operation, the cervical lordosis angle, the height of the fusion segment, the Cobb angle of the fusion segment, and the Cobb angle of C2-7 in both groups increased when compared to the preoperative values (P < 0.05). At both time points, the Cobb angle of the fusion segment in the Zero-PVA group was larger than that in the CP group (P < 0.05). At 3 months post-operation, there was a statistically significant difference in the Bridwell grades of interbody fusion between the two groups (P < 0.05). At 3 months post-operation, the JOA scores increased and the NDI decreased in both groups compared to the preoperative values (P < 0.05), but no significant differences were found in the JOA scores or NDI between the two groups either before or after the surgery (P > 0.05). The postoperative complication rate in the Zero-PVA group was lower than that in the CP group (P < 0.05). Conclusions In ACDF for single-level cervical spondylosis, both Zero-PVA and CP can effectively enhance the cervical function of patients. Nevertheless, Zero-PVA is more effective in minimizing intraoperative trauma, improving the Cobb angle of the fusion segment, facilitating bone fusion, and reducing the risk of postoperative complications.
