Arthroscopic fixation with RigidFix vs. RigidLoop in anterior cruciate ligament reconstruction:A compar⁃ative study

Objective To investigate the clinical effects of knee arthroscopic reconstruction of anteriorcruciate ligament(ACL)with RigidFix and RigidLoop. Methods From June 2016,121 patients with ACL ruptureunderwent anterior cruciate ligament reconstruction with Rigidfix or Rigidloop under total arthroscopy. patientsThey volunteered to be randomly divided into two groups:Rigidfix group(n = 61)and Rigidloop group(n = 60).The ACL ruptures in the two groups were diagnosed preoperatively by MRI,without significant difference inLysholm score between the Rigidfix group and Rigidloop group[(49.5 ± 6.5)vs.(48.9 ± 6.7),P > 0.05]. Results All of the patients were followed up for(27.9 ± 3.5)months. At the end of follow⁃up,the Lysholm scores of theRigidfix and Rigidloop group were(91.7 ± 3.9)and(91.5 ± 3.8),respectively. The IKDC scores of the Rigidfixgroup were normal and close to normal in 58 cases,abnormal in 2 cases,much more abnormal in 1 case,with anexcellent rate of 95.1%,and the IKDC scores of the Rigidloop group were normal and close to normal in 56 cases,abnormal in 3 cases and much more abnormal in 1 case with an excellent rate of 93.3%. There were no significantdifferences between the two groups(P > 0.05). Conclusion Both RigidFix and RigidLoop under total arthroscopycan achieve anatomical reconstruction and stability of knee joints of patients with ACL ruptures.

Objective To observe the efficacy of double rotator interval slide technique in arthroscopicrepair of massive,posterosuperior,contracted rotator cuff tears. Methods Twenty ⁃eight patients with massive,posterosuperior,contracted rotator cuff tears were randomly assigned to two groups. The control group(n = 14)was treated with anterior rotator interval slip technique during arthroscopic repair. The observation group(n = 14)received double rotator interval slip technique both anterior and posterior. The operation durationand postoperativecomplications were observed,and the improvement degree of VAS score,constant⁃Murley shoulder score,Sugayaimaging grade of MRI and range of active motion of shoulder were compared between the two groups 12 monthsafter surgery. Results All operations were successfully completed and all patients were followed up for 12 months.There was no significant difference in the improvement degree of constant ⁃Murley shoulder score between the twogroups 12 months after surgery(P > 0.05). The results showed the observation group was better than that of thecontrol group in terms of the improvement degree of VAS score,range of active motion of shoulder and Sugayaimaging grading of MRI 12 months after surgery(P < 0.05). Conclusion The double rotator interval slip techniquecould significantly improve the VAS score,active forward flexion motion of shoulder joint and internal and externalrotation motion,and the Sugaya imaging grading of MRI after the rotator cuff tears suture,which is worthy of clini⁃cal promotion.

Objective To investigate the clinical effect of wrist arthroscopy combined with open reductionand internal fixation for elderly distal radius fractures complicated with triangular fibrocartilage complex injury.Methods A total of 68 elderly patients with distal radius fractures treated in the arthroscopy department of HenanLuoyang Orthopedic Hospital(Henan Orthopedic Hospital)from January 2018 to January 2020 were analyzedretrospectively. All patients were divided into two groups according to the operation method. The treatment group(n = 33)was given wrist arthroscopy combined with open reduction and internal fixation;The control group(n = 35)was treated with open reduction and internal fixation alone. The intraoperative indexes,wrist range of motion,gripstrength and DASH score before and 12 months after operation were compared between the two groups. Theimproved Mcbridei wrist function score and complications were followed up 12 months after operation. Results Significant differences in intraoperative indexes,wrist range of motion,grip strength and DASH score between thetwo groups before and 12 months after operation were found(P < 0.05);The improvement degree of the above wristjoint indexes in the treatment group was better than that in the control group(P < 0.05);12 months after operation,the excellent and good rate in the treatment group was 90.9%,which was higher than 70.4% in the control group(χ2 = 4.169,P < 0.05). During the follow⁃up,the incidence of arthritis and uneven articular surface in the treat⁃ment group was lower than that in the control group(χ2 = 5.184,P < 0.05). Conclusion Wrist arthroscopy couldrealize anatomical reduction and fixation of the fracture end under direct vision in elderly distal radius fractures,diagnose and repair the complicated injury of triangular fibrocartilage complex and ligament,effectively reduce theoccurrence of wrist pain and complications and the clinical effect is accurate and reliable.

Objective To investigate the effectiveness and safety of knee joint lavage combined withpulsed radiofrequency for knee osteoarthritis. Methods A total of 49 patients with knee osteoarthritis were selectedin the Affiliated Hospital of Southwest Medical University from January 2019 to January 2021. According todifferent treatments,25 cases were assigned to knee lavage group(GX group)and 24 cases to knee lavage + pulsedradio frequency group(GX+PRF group). The pain numerical rating scale(NRS),Western Ontario and McMaster University(WOMAC)osteoarthritis index were compared between the two groups before operation,1 week afteroperation,1 month after operation,3 months after operation,and 6 months after operation. The long⁃term efficacywas also evaluated. Results The NRS score and WOMAC index between the two groups were significantly decreasedafter treatment,and the decline in the GX+PRF group was more obvious. Compared with the GX group,the differ⁃ence was significant in 1 month after surgery(P < 0.05). At 6 months postoperatively,the clinical efficacy of theGX+PRF group was better than that of the GX group. Conclusion Knee lavage combined with pulsed radiofrequencywas effective and safe for knee osteoarthritis. The medium and long⁃term effect was better than knee lavage alone,and it is worthy of clinical application and promotion.

Objective To investigate the effect of two different osteotomy methods on lower limb align⁃ment and cartilage degeneration in patients with total knee arthroplasty(TKA). Methods From January 2019 toApril 2021,97 patients in our hospital who were scheduled to undergo TKA were selected and randomly dividedinto a gap group(n = 49)and a measurement group(n = 48)by random number table. Bone method. Perioperativeindicators,lower limb alignment indicators,cartilage degeneration indicators,trauma stress indicators,visualanalogue scale(VAS),New York Hospital for Special Surgery(HSS)knee joint score,and complications werecounted between the two groups. Results (1)The amount of distal femoral osteotomy and the thickness of thepolyethylene cushion in the gap group were greater than those in the measurement group(P < 0.05);(2)SerumCor and NE levels in the two groups were higher than those before surgery on the 3rd day after surgery(P < 0.05);(3)The OPG and PANKL levels in the 1 month and 3 months interval group were lower than those in the measure⁃ment group(P < 0.05);(4)MPTA and LDFA in the 1 month and 3 months interval group were lower than thosein the measurement group,and ROM was higher than that in the measurement group(P < 0.05);(5)VAS scoresin the 1 month and 3 months interval group were lower In the measurement group,the HSS score was higher thanthat in the measurement group(P < 0.05);(6)There was no statistically significant difference in the total inci⁃dence of complications between the two groups(P > 0.05). Conclusion The gap balance method is better thanthe measurement osteotomy method in terms of lower limb force line,cartilage degeneration,pain and knee functionrecovery,and it is especially suitable for patients with TKA.

Effective analgesic management is the guarantee of rapid recovery following surgery. Regionalanalgesic techniques,including epidural analgesia,intrathecal morphine and peripheral nerve blocks,play animportant role in postoperative analgesia. Epidural analgesia is considered the gold standard for postoperative painrelief following open surgeries,but has a certain rate of failure and adverse events. The superiorities of intrathecalmorphine analgesia include low dose requirement,high analgesic potency,and long duration of action. Ultrasound⁃guided peripheral nerve block has advantages of easy to conduct,less invasive,high success rate,and guaranteedeffect. Given that the advantages and risks of analgesic techniques may not be identical,appropriate analgesic strategiesare supposed to be selected according to the patient condition for better analgesic efficacy and less complications.

Objective To investigate the effect of long⁃stranded non⁃coding RNA(LNCRNA)Linc00858on the proliferation,migration and invasion of hepatoma cells by regulating targeted small molecule RNA⁃3126⁃5P(MIR⁃3126⁃5P). Methods Real⁃time fluorescent quantitative PCR(qRT⁃PCR)was used to detect the expressionsof Linc00858 in hepatocellular carcinoma(HCC),normal para ⁃ cancerous tissues,normal human hepatocytesLO2 and human hepatoma cell lines. Human hepatoma cell lines HUH ⁃ 7 and MHCC ⁃97H were transfected withLinc00858⁃overexpressed plasmid and Linc00858⁃expressing recombinant lentivirus,respectively. The transfectioneffect was determined by qRT⁃PCR,and proliferation of tumor cells was detected by CCK⁃8 and Transwell assay.Western Blot assay was used to determine the effects of link00858 on epithelial⁃mesenchymal transformation and expression of cell cycle related proteins. Bioinformatics and double luciferase assay were used to determine the association of Linc00858 with miR⁃3126⁃5p. After the level of Linc00858 expression was adjusted,the expressionof miR ⁃3126⁃5p in hepatoma cells was detected by qRT PCR. miR ⁃3126⁃5p mimics and miR ⁃3126⁃5p inhibitorwere transfected respectively;then the proliferation,migration,invasion and expression of related proteins wereanalyzed by the above methods. Results Linc00858 expression was higher in cancerous tissues than in LO2 andnormal tissues. Proliferation,migration and invasion of HUH ⁃ 7 cells were increased after overexpression ofLinc00858. The protein expressions of Vimentin,Cyclin E1 and CDK2 increased obviously,while those of E⁃cad⁃herin and P27 decreased obviously. Proliferation,migration and invasion of MCHH⁃97H cells were inhibited bysilencing Linc00858. The expression levels of Cyclin E1,CDK2 and Vimentin ⁃ related proteins decreased signifi⁃cantly,while those of E ⁃cadherin and P27 increased significantly. Linc00858 regulated miR ⁃3126⁃5p expression,miR ⁃ 3126 ⁃ 5p mimics significantly reversed proliferation,migration and invasion of HUH ⁃ 7 cells induced byLinc00858 overexpression. miR ⁃3126⁃5p inhibitor reversed the impact of Linc00858 silencing on proliferation,migration and invasion ability of MHCC⁃97H cells and related protein expression level. Conclusions Linc00858accelerated proliferation,invasion and migration of hepatoma cells through the negative regulation of microR⁃NA3126⁃5p.

Objective To observe the proliferation and apoptosis of Jurkat cells,and to discuss the onco⁃lytic effect of RSV on Jurkat cells and its possible mechanism. Refractory or recurrent acute lymphoblastic leukemiais an important role affecting the overall efficacy of children with leukemia. Oncolytic virotherapy is a new biologi⁃cal therapy against tumors in recent years. Multiple oncolytic viruses,such as measel virus and Newcastle virus,have been widely researched in the treatment of leukemia. However,there are few studies in the treatment of leuke⁃mia with respiratory syncytial virus(RSV). Methods Jurkat cells were infected with RSV. Jurkat cells not infectedwith RSV were set as the blank control group(con). Cells were collected at different time points(3 h,6 h,12 h,24 h and 48 h)after infection. Respiratory syncytial virus nonstructural protein 2(RSV⁃NS2)and TRAF6 weredetected by qRT⁃PCR . The proliferation of Jurkat cells was evaluated by MTT and clone formation assay,and theapoptosis level was evaluated by FCM. The protein expression of Bcl2 and TRAF6 and NF⁃κB and cleaved Caspase⁃3,8,9 was compared by Western blot. Results After RSV infected Jurkat cells,qRT⁃PCR detected that the expres⁃sion of RSV⁃NS2 began to appear within 3 h after infection,increased significantly at 6 h,then increased in a time⁃dependent manner(P < 0.05),while the expression of TRAF6 decreased with the extension of infection time(P <0.05). MTT results showed that the proliferation of Jurkat cells decreased significantly from 6 h to 24 h after RSVinfection(P < 0.05). Clone formation experiment showed that the number of clones of Jurkat cells decreased significantly 12 days after RSV infection(P < 0.05),suggesting that RSV inhibited the proliferation of Jurkatcells. FCM results showed that the apoptosis level of Jurkat cells increased significantly in a time⁃dependent man⁃ner from 12 h to 48 h after RSV infection(P < 0.05). Western blot analysis revealed that the expression of cleavedcaspase⁃3 and cleaved caspase⁃9 increased significantly after 12 hours of RSV infection,and the expression of Bcl2decreased significantly(P < 0.05),but the expression of cleaved caspase⁃8 did not change significantly. The expres⁃sion of TRAF6 and p⁃NF⁃κB decreased with the extension of infection time(P < 0.05). Conclusion RSV in⁃hibits the proliferation of Jurkat cells,promotes cell apoptosis and inhibits TRAF6/NF⁃κB pathway,so as to exertthe inhibitory effect on Jurkat cells,revealing that RSV can produce oncolytic effect on hematological tumors.

Results The relative expression levels of miR⁃425⁃5p,miR⁃137,miR⁃325⁃3p,miR⁃342⁃3p,miR⁃203a⁃3p and miR⁃9⁃3p in the serum of children with severe pneumonia were higher than those of healthy childrenand the relative expression of REG3A mRNA was lower(P < 0.05);Both miR⁃137 and miR⁃342⁃3p and REG3A3′⁃UTR have base complementation,and their expression is regulated by targeting(P < 0.05);Both miR⁃137 over⁃expression and miR⁃342⁃3p overexpression can reduce the proliferation activity of A549 cells,reduce the rate of EdUpositive cells,increase the rate of apoptosis and the expression of Caspase⁃3,Bax and p53 proteins,while Bcl⁃2Protein expression decreased(P < 0.05). Conclusion miR⁃137 and miR⁃342⁃3p may inhibit lung epithelial cellproliferation and promote cell apoptosis by targeting REG3A.

Objective To explore the effects of neural stem cells(NSCs)carrying bovine parvovirus bpV(PIC)combined with neurotrophin 3(NT ⁃ 3)chitosan scaffold on the nerves repair after traumatic brain injury(TBI). Methods 90 rats were randomly divided into control group,TBI group,TBI +NSCs group,TBI + bpV(PIC)⁃NSCs group,TBI+NT⁃3 chitosan group and TBI+bpV(PIC)⁃NSCs+NT⁃3 Group(n = 15). The TBI modelwas constructed by impact,and NSCs,bpV(PIC)⁃NSCs,NT⁃3 chitosan or NT⁃3 chitosan loaded with bpV(PIC)⁃NSCs were implanted after impact. Neural function was evaluated by mNSS score and water maze test. HE stainingand BrdU staining were used to evaluate brain tissue damage and neuron regeneration. Results The mNSS score,the degree of brain damage,and the number of new neurons in the TBI group were significantly higher than thoseof the control group,and the target quadrant residence time and the number of crossing platforms were significantlylower than those of the control group(P < 0.05). The mNSS score and the degree of brain damage in the TBI+NSCsgroup were significantly lower than those in the TBI group,while the target quadrant residence time,the number ofcrossing platforms and the number of new neurons were significantly higher than those in the TBI group(P < 0.05).The mNSS score and the degree of brain damage in the TBI+bpV(PIC)⁃NSCs group were significantly lower thanthose in the TBI+NSCs group,while the target quadrant residence time,the number of crossing platforms,and thenumber of newborn neurons were significantly higher than those in the TBI+NSCs group(P < 0.05 ). Clinical characteristics and prognostic factors of pulmonary large cell neuroendocrine carcinoma

Objective To evaluate the effect of different clinical features,tumor markers,TNM stage andtreatment strategies on pulmonary large cell neuroendocrine carcinoma(pLCNEC). Methods A cohort of 149patients with pLCNEC were retrospectively enrolled. All data were collectedfrom the electronic medical records.The effect of clinical features and factors,including serum tumor markers(NSE,CA125,CA153,CYFRA21⁃1),TNM stage,pathological types(pure or combined),and treatment strategies on prognosis were analyzed by KaplanMeier curve and Cox model. Results Of the 149 patients,the median age was 63 years and 136(91.28%)weremales;100 were pure pLCNEC and 49 were combined pLCNEC. The median survival time was 18(10 ~ 32)months. The overall survival(OS)of patients with increased serum NSE,CA125,CA153 and CYFRA21⁃1 levelswas significantly shorter(HR = 2.959,4.249,5.198,1.84,P < 0.05). The poor prognosis also associated withthese factors including tumor TNM stage(compared with stage I patients,the HR of stageⅢ and Ⅳ patients were4.074 and 9.470,respectively,P < 0.05),N and M(HR = 1.815,4.083,P < 0.05). CA125,CA153,N and Mwere independent risk factors of poor prognosis. The stage Ⅰ ⁃ Ⅲ patients treated with surgery had significantlylonger OS than patients treated without surgery(HR = 0.357,P < 0.05). The OS of combined pLCNEC patientswas similar to pure pLCNEC patients,but the OS of pLCNEC patients combined with small cell lung cancer wassignificantly shorter(HR = 2.051,P = 0.024). Surgy⁃treatment was very efficient in patients with negative CEA,CA125 or CA153(HR = 0.256,0.479,0.388,all P < 0.05),but surgy⁃treatment became less efficient in patientswith positive CEA,CA125 or CA153(HR = 0.548,0.628,0.746,all P > 0.05). Conclusion CA125,CA153,N and M are independent risk factors of poor prognosis of pLCNEC. The prognosis of pLCNEC combined with smallcell lung cancer is worse. Surgical treatment significantly improved the prognosis of stage Ⅰ⁃Ⅲ patients.

Objective To study the relationship between anticoagulant therapy and concentration changesof T⁃tau protein and Aβ1⁃42 in plasma neurogenic exosomes in patients with permanent atrial fibrillation(AF),and then to evaluate the effect of anticoagulant therapy on cognitive function in AF patients. Methods A total of140 permanent AF patients aged 50 ~ 75 without previous cerebrovascular events were enrolled in this study. Theywere divided into two groups according to whether anticoagulant therapy was performed,40 patients in anticoagulantgroup and 100 patients in non⁃anticoagulant group. We used Mini Mental State Examination(MMSE)to assess thecognitive function of all participants. Logistic regression analysis was used to explore the effect of anticoagulanttherapy on cognitive function in patients with AF. We collected fasting venous blood from all subjects and enrichedneuronal⁃derived exosomes by immunoprecipitation,then confirmed exosomes by transmission electron microscopeand western blot. Quantitative analysis of T⁃tau protein and Aβ1⁃42 in exosomes was conducted by ELISA. Thedifference of protein concentration in neurogenic exosomes between the two groups was tested. Results Afteradjustment for demographic characteristics and comorbid conditions,non⁃anticoagulant therapy in patients with AFwas significantly associated with CI(OR = 5.024,95%CI:1.584 ~ 15.936,P < 0.01). The cognitive impairment ofpatients with AF in non⁃anticoagulant group was significantly severer than that in anticoagulant group(P < 0.001).The concentration of T⁃tau protein and Aβ1⁃42 in plasma neuronal⁃derived exosomes in non⁃anticoagulant group(221.75 ± 54.34 pg/mL and 3.97 ± 1.20 pg/mL)was significantly higher than that in anticoagulant group(183.02± 46.84 pg/mL and 3.09 ± 0.79 pg/mL)(P < 0.001). Conclusion Anticoagulant therapy is a protective factor forcognitive function in patients with permanent AF and delays the growth of markers related to cognitive impairment.

Objective To evaluate the sensitivity and specificity of noncontact fundus imaging based onsmartphone for diabetic retinopathy(DR)screening in urban primary medical institution. Methods We conducteda prospective and comparative study. Participants with diabetes were recruited to conduct fundus screening bysmartphone ⁃ based noncontact fundus imaging and binocular indirect ophthalmoscope(BIO)after mydriasis fromApril 1,2021,to June 31st in People′s Hospital of Xinjin District of Chengdu. With the results of BIO as a refer⁃ence standard,the sensitivity and specificity of smartphone ⁃ based noncontact fundus imaging to screen DR wereevaluated. Results A total of 143 patients of 276 eyes were recruited. Based on the diagnostic result of BIO,theoverall sensitivity and specificity of smartphone noncontact fundus imaging in the diagnosis of DR were 84.1% and94.2%(kappa = 0.79),respectively;the sensitivity and specificity for mild/moderate NPDR were 79.2% and 94.9%(kappa = 0.73),respectively;Both the sensitivity and specificity of VTDR were 100%(kappa = 1). Conclusion Noncontact fundus imaging based on smartphone is an effective method for DR screening with high sensitivity andspecificity. As a low ⁃ cost alternative to conventional DR screening,it may help to improve the coverage of DRscreening in areas with relatively poor medical conditions.

Objective The research aimed to investigate the relationship between DNA methylation ofm6A demethylase FTO gene and low/no response level of hepatitis B vaccine(HepB),and to provide scientific basisfor predicting individual response level of HepB and developing new HepB. Methods A total of 263 children aged8 to 9 months(who had completed hepatitis B immunization program)were recruited as subjects in a non⁃matchedcase ⁃control study from 2014 to 2016. According to the level of hepatitis B surface antibody(HBsAb),anti ⁃HBsconcentration < 100 mIU/mL was taken as the case group,and anti⁃HBs concentration ≥ 100 mIU/mL was taken asthe control group. DNA methylation level of FTO gene locus was detected by Methyl Target technique ,and therelationship between DNA methylation level of CpG island at 28 locus of FTO gene and immune response level ofHepB was analyzed. Results The DNA methylation level of FTO gene loci was low. The differences of DNA meth⁃ylation levels at FTO_1 gene locus 222 and FTO_2 gene locus 72 were statistically significant between the casegroup and control group. The level of DNA methylation at 222 locus of FTO_1 gene in case group was higher thanthat in control group(Z = -2.38,P = 0.009). The level of DNA methylation at locus 72 of FTO_2 gene in casegroup was higher than that in control group(Z = -1.71,P = 0.044). Conclusion It was suggested that methylationof FTO gene might occur in human body,and the low /non ⁃immune response level of HepB might be affected byFTO_1 gene locus 222 and FTO_2 gene locus 72.

Objective To investigate the energy efficiency of plasma triggering receptor expressed on my⁃eloid cell 2(TREM⁃2)combined with model for end⁃stage liver disease(MELD)score in predicting the near⁃termprognosis of patients with hepatitis B virus⁃acute⁃on⁃chronic liver failure(HBV⁃ACLF). Methods A total of 142patients with HBV⁃ACLF admitted to our hospital from January 2019 to January 2021 were selected as the HBV⁃ACLF group,and were divided into a death group(n = 52)and a survival group(n = 90)according to patientoutcomes at 90 d follow⁃up,and another 58 physically healthy individuals were selected as the control group duringthe same period. Plasma interleukin ⁃ 6(IL ⁃6),IL8,and TREM ⁃2 levels were measured by ELISA and MELDscores were calculated. Pearson/Spearman correlation coefficient was used to analyze the correlation between plasmaTREM⁃2 and the levels of IL⁃6 and IL⁃8 in HBV⁃ACLF patients. Multivariate logistic regression was used to analyzefactors influencing the poor prognosis of HBV ⁃ACLF patients in the near future. The predictive value of plasmaTREM⁃2 levels combined with the MELD score for the near⁃term prognosis of HBV⁃ACLF patients was analyzedusing ROC curves. Results Plasma IL⁃6,IL⁃8 and TREM⁃2 levels were significantly elevated in the HBV⁃ACLFgroup compared to the control group(P < 0.05). Plasma TREM⁃2 was positively correlated with the levels of IL⁃6and IL⁃8 in HBV⁃ACLF patients(r/rs = 0.762,0.857,all P < 0.001). Neutrophil ratio,total bilirubin,bloodcreatinine,international normalized ratio,MELD score,IL⁃6,IL⁃8 and TREM⁃2 were independent risk factors forpoor prognosis of the HBV⁃ACLF patients in the near future(P < 0.05).The areas under the curve for the TREM⁃2,MELD score,and TREM ⁃ 2 + MELD score predicting the near ⁃term prognosis of the HBV ⁃ACLF patients were 0.771,0.795,and 0.858,respectively,and the area under the curve for the TREM ⁃2 + MELD score predictingthe near ⁃ term prognosis of HBV ⁃ ACLF patients was significantly greater than that predicted by the TREM ⁃ 2,MELD score alone(P < 0.05). Conclusion Plasma TREM⁃2 levels and elevated MELD scores are independentrisk factors for poor prognosis of HBV ⁃ ACLF patients in the near future,and plasma TREM ⁃ 2 combined withMELD scores enhances the predictive value of poor prognosis of the patients in the near future.

Objective To investigate the association of DCE⁃MRI quantitative parameters with the degreeof histopathological differentiation and molecular biological marker p53 in patients with rectal cancer. Methods 63 patients with primary rectal cancer were surgically and pathologically confirmed and the expression of p53 wasdetected by immunohistochemistry. All the patients underwent DCE ⁃MRI scan preoperatively. The quantitativeparameters of ROI of tumor site were obtained by image post⁃processing. The association of the quantitative parame⁃ters of DCE⁃MRI with the pathological differentiation of rectal cancer and the expression of p53 was analyzed.Results Ktrans,Kep and Ve values increased with a decrease in tissue differentiation in 63 patients. The differ⁃ences in DCE⁃MRI quantitative parameters of different tissue differentiation were statistically significant(P < 0.05).Ktrans,Kep,and Ve values were significantly correlated with the degree of pathological differentiation of rectalcancer(P < 0.05). The differences in Ktrans and Ve among different levels of p53 expression in rectal cancer werestatistically significant(P < 0.05);Ktrans and Ve were positively correlated with p53 expression(P < 0.05),butKep was not correlated with p53 expression(P > 0.05). The area under ROC curve(AUC)of Ktrans value was thelargest in predicting p53 expression,with sensitivity of 84.44% and specificity of 83.33% . Conclusions Thequantitative parameters of DCE⁃MRI were correlated with the degree of pathological differentiation and the expres⁃sion level of p53 in rectal cancer.

Objective To investigate the correlation between serum NOD⁃like receptor protein 3(NLRP3)and interleukin ⁃18(IL ⁃18)and the severity of septic acute kidney injury(AKI). Methods In this prospectivestudy,56 patients with sepsis who were hospitalized in the ICU of the General Hospital of Ningxia Medical Universityfrom October 2018 to August 2020 were selected and divided into the sepsis group(26 cases)and sepsis AKI group(20 cases of mild AKI and10 of severe AKI)according to the Kidney Disease Improving Global Outcomes(KDIGO)diagnostic criteria,and 30 healthy subjects in the same period were selected as control group. The general data ofeach group were collected and compared. According to the 28⁃day mortality rate,the patients were divided intosurvival group(n = 30)and death group(n = 26). The level of serum NLRP3 and IL⁃18 was detected and com⁃pared,and the correlation between serum NLRP3,IL⁃18 and urine NGAL and Kim⁃1 was analyzed. Risk factorsaffecting the prognosis of patients with sepsis AKI were analyzed,and receiver operating characteristic curve(ROC)was drawn to evaluate the predictive value of independent risk factors alone and in combination. Results Compared with that in the control group,the level of NLRP3 and IL⁃18 in sepsis group and sepsis AKI group washigher than that in sepsis mild AKI group,and the level of NLRP3 and IL⁃18 in sepsis severe AKI group was higherthan that in sepsis mild AKI group(P < 0.05). Compared with that in the death group,the level of NLRP3 andIL ⁃18 in the survival group was lower,and the level of NLRP3 and IL ⁃18 was positively correlated with urinaryNGAL and Kim⁃1. Multivariate logistic regression analysis showed that serum NLRP3 was an independent factoraffecting the death of sepsis AKI(P < 0.05). ROC curve analysis showed that the area under the ROC curve(AUC) of NLRP3 and APACHEⅡ score for predicting 28⁃day mortality of sepsis was 0.72 and 0.73,and the sensitivitywas 80% and 85% respectively. AUC of the combined detection of the above two indexes was 0.78,and the sensi⁃tivity was 95%,which was better than that of the single indicator(P < 0.05). Conclusion The level of serumNLRP3 and IL⁃18 is closely related to the severity of AKI patients with sepsis.

Objective To investigate the clinical characteristics and prognosis of patients weighing ≤ 10 kgin pediatric intensive care unit treated with continuous renal replacement therapy. Methods The patients weighing≤ 10 kg in pediatric intensive care unit treated from January 2010 to December 2019 were retrospectively analyzed.According to the clinical outcome,the patients were divided into survival group(n = 38)and non⁃survival group(n = 40),clinical characteristics and prognosis of patients were analyzed. Results This study included 78 cases,it was found that the proportion of regional citrate anticoagulation in the survival group was higher(92.1% vs. 70%,P = 0.029);the CRRT initiation time was shorter in the survival group(calculated from the time of PICU admis⁃sion to CRRT initiation),the median time of the survival group was 18.6 hours,while the median time of the non⁃survival group was 66.5 hours(P = 0.002);the fluid overload(FO%)of the survivors was lower when CRRT wasinitiated(1.9% vs. 8.6%,P = 0.001). Multivariate logistic regression analysis showed PRISM Ⅲ score ,FO% andCRRT initiation time > 24 h were risk factors for the prognosis of patients in this group(OR = 1.120,1.109,3.912,P < 0.05). Conclusion In our cohort,the prognosis of patients weighing ≤ 10 kg treated with continuous renalreplacement therapy was associated with FO%,PRISM Ⅲ score and CRRT initiation time.

Objective To study the therapeutic effect of beraprost sodium combined with NiaoduqingGranules on improving the residual renal function of hemodialysis patients. Methods Eighty patients with chronickidney disease from March 2019 to April 2021 who underwent haemodialysis therapy were selected and randomlydivided into study group(n = 40)and control group(n = 40). The patients in the control group were treated withhemodialysis,while the patients in the investigation group were treated with beraprost sodium combined with Nia⁃oduqing Granules. The indexes of residual renal function were compared between the two groups. Results Thehighest level of serum creatinine and BUN in the study group was significantly lower than that in the control group,but the level of eGFRand RRF was significantly higher than that in the control group(P < 0.05). The residualrenal function and KT/V value of dialysis in the study group were significantly better than those in the control group(P < 0.05). Conclusion Beraprost sodium combined with Niaodujing Granules can effectively protect the residualrenal function of hemodialysis patients,improve the adequacy of hemodialysis,and the quality of life of patients,and delay the development of the disease.

Objective To investigate the value of shear wave elastography(SWE)in diagnosing lacrimalgland lesions in patients with dry eyes. Methods Forty⁃six patients diagnosed with dry eyes in the Department ofOphthalmology of our hospital from October 2019 to October 2020 were assigned as the dry eyes group,and fifty⁃nine people with normal physical examinations during the same period as the control group. The size,shape andinternal echo of lacrimal glands were measured by two⁃dimensionalultrasounds. The mean elastic modulus(Emean)of lacrimal glands was obtained by SWE. The receiver operating characteristic curve(ROC)was drawn. Results The shapes of lacrimal glands in the dry eyes group and the control group were both triangular⁃like by the twodimensionalultrasounds. There was no significant difference in the sizes of lacrimal glands between the two groups(P > 0.05). The internal echoes of the lacrimal glands in the dry eyes group was mostly inhomogeneous,and thosein the control group were homogeneous and hypoechoic. The Emean of the lacrimal glands in the dry eyes group wassignificantly larger than that in the control group[(20.02 ± 6.28)kPavs.(10.29 ± 2.50)kPa,P < 0.05]. The areaunder ROC curve was 0.939 and the best cutoff value was 13.17 kPa. Conclusion SWE can quantitatively evaluatethe stiffness of lacrimal glands in patients with dry eyes. It has high diagnostic value in detecting lacrimal glandlesions in patients with dry eyes,and therefore it can provide a new auxiliary examination modality for clinical diag⁃nosis of dry eyes.

Objective The existing research data lacks the HR ⁃HPV infection status and HR ⁃HP Vsubtype distribution of females in rural areas of Linzhi City,Tibet. This study intended to improve the domesticHR ⁃ HPV epidemiological data,inspire domestic HPV vaccine research and solve the above problem. Methods The results of two⁃cancer screening in rural women in Linzhi City,Tibet from 2019 to the first quarters of 2021were collected. PCR fluorescence were applied to detect HR ⁃HPV subtype genes in the screened specimens. Westatistically analyze HPV infection in rural women in Linzhi City,Tibet. The infection rate of different HR ⁃HPV subtypes and the infection rate of HR⁃HPV subtypes in different age groups were compared with others in ruralareas outside the Qinghai ⁃Tibet Plateau. The infection rate of different HR ⁃HPV subtypes were also analysed.Results A total of 13 741 cases were included in the study. The overall HR⁃HPV infection rate of women in ruralareas of Linzhi City,Tibet was 12.81%(1 760/13 741),which was higher than that of women in rural areas out⁃side the Qinghai⁃Tibet Plateau(7.82%,9 249/118 237)(χ2 = 532.2,P < 0.001). The 35 ~ 39 years old group had the highest HPV infection rate(14.99%,441/2 942)which was higher than that of women in the same agegroup in rural areas outside the Qinghai⁃Tibet Plateau(7.22%,1 827/25 322)(χ2 = 215.9,P < 0.001). The 50 ~54 years old group has the lowest HPV infection rate,9.47%(209/2 208),and 8.14%(1 604/19 698)of HPV infec⁃tion rate of women in the same age group outside the Qinghai ⁃ Tibet Plateau,which was no statistical difference(χ2 = 4.575,P = 0.032). The top three subtypes of HR⁃HPV infection among women in rural areas of Linzhi City,Tibet are HPV52(19.94%,351/1 760),HPV16(12.33%,217/1 760)and HPV58(11.88%,209/1 760). Thetop three HR⁃HPV infection subtypes of women in rural areas outside the plateau were consistent with the HR⁃HPVinfection subtypes of rural women in Linzhi City,Tibet. The top three HR⁃HPV subtypes of women in the 35 ~ 39,40 ~ 44,and 45 ~ 49 age groups in rural areas of Linzhi City,Tibet are HPV52,HPV16,and HPV58 respectively,which were similar to those on the rural areas outside Qinghai⁃Tibet Plateau. Conclusion The HR⁃HPV infectionrate of women in rural areas of Linzhi City,Tibet was significantly higher than that of women in rural areas outsidethe Qinghai⁃Tibet Plateau. The top three HR⁃HPV infection subtypes in two regions were the same.

Objective The study aimed to investigate the application value of 3D free breathing pinwheeltechnology(Multi Vane XD)combined with chemical shift technology hydrolipid separation imaging (Dixon)sequence in cervical magnetic resonance(MR)enhancement. Methods A total of 61 patients underwent neckMRI scans using 3D MultiVane XD Dixon sequence and conventional T1WI frequency attenuated inversion recovery(SPAIR)enhanced sequence. The image quality of enhanced T1WI sequences in the two groups were scored andstatistically analyzed. Results In terms of fat suppression and image artifacts,the image quality score of 3DMultiVane XD Dixon sequence was higher than that of conventional T1W SPAIR sequence,and the difference wasstatistically significant(P < 0.05). It was higher than 3D MultiVane XD Dixon sequence and the difference wasstatistically significant(P < 0.05). Conclusion 3D MultiVane XD Dixon sequence could significantly reduceneck MR ⁃ enhanced swallowing and breathing,vascular pulsation and metal susceptibility artifacts,which has agood fat suppression effect.

Interleukin(IL)⁃38,a newly discovered anti⁃inflammatory cytokine in the IL⁃1 family,isexpressed in a variety of tissues and secreted by a variety of cells. By binding to multiple receptors such as IL⁃36R,IL⁃1RAPL1 and IL⁃1R1,IL⁃38 plays an essential role in the physiology and pathogenesis of the central nervoussystem(CNS). IL⁃38 mitigates pathological damage by inhibiting the expression of Th17 in neuromyelitisoptica.IL⁃38 improves the symptoms of stroke by inhibiting SIRT1/HIF⁃1α signaling pathway. However,the role of IL⁃38in neurological diseases and its potential mechanism need to be further studied. This review systematically and com⁃prehensively summarizes the biological characteristics,pathological effects and potential mechanisms of IL⁃38 un⁃der physiological and pathological conditions,aiming to explore potential molecular targets and new therapeuticstrategies for CNS diseases.
