摘要:

急性肺损伤(ALI)是一种病情危重的临床综合征,临床特征是进行性呼吸衰竭和顽固性低 氧血症,可发展为高致命性急性呼吸窘迫综合征(ARDS)。尽管对 ALI 的病理生理学和治疗进行了广泛的 研究,但临床上尚缺乏有效的 ALI 治疗防治方法。间充质干细胞(MSCs)因其分化潜力和低免疫原特性, 逐渐被研究用于治疗 ALI。MSCs 的治疗作用部分是通过释放细胞外囊泡(EVs)发挥作用的,MSC⁃EVs 含多种 mRNA、miRNA、细胞因子和蛋白质等物质,可以促进受损组织的修复和再生,且 MSC⁃EVs 免疫排斥 性低、易于储存和安全性较高,在 ALI 的治疗和防治中具有潜在的应用前景。本文主要对 MSC⁃EVs 生物学特性、治疗 ALI 的作用机制和临床试验的最新进展予以综述,以望 MSC⁃EVs 成为 ALI 未来治疗的 新策略。

Abstract:

Acute lung injury(ALI)is a critical clinical syndrome,characterized by progressive respiratory failure and stubborn hypoxemia,which can develop into highly fatal acute respiratory distress syndrome(ARDS). Although the pathophysiology and treatment of ALI have been extensively studied,there is still a lack of effective therapies and prevention methods for ALI. Mesenchymal stem cells(MSCs)have been increasingly studied for the treatment of ALI because of their differentiation potential and low immunogenicity. The therapeutic effect of MSCs is partially exerted in a paracrine manner by releasing extracellular vesicles(EVs). MSC⁃EVs contain a variety of mRNA,miRNA,cytokines,proteins and other substances,which can promote the repair and regeneration of damaged tissues. This article reviewed the biological characteristics of MSC⁃EVs,mechanism of action in treating ALI and latest progress in clinical trials,with the aspiration that MSC⁃EVs will become a new treatment strategy for ALI in the future.
