高甘油三酯血症性急性胰腺炎的诊治

摘要:

高脂血症是我国急性胰腺炎(AP)的第二大病因,其中高甘油三酯血症性急性胰腺炎 HTG⁃AP)是主要类型。由于 HTG⁃AP 的病因、发病机制特殊,因此其临床特征与其他病因所致 AP 并不相 同,致使该病易被漏诊且易于重症化,导致患者病情进展迅速、病死率高,即使治愈后也容易复发。目前 缺少针对 HTG⁃AP 的可操作性高的专家共识/指南,本文围绕笔者等近期执笔的《高甘油三酯血症性急性 胰腺炎诊治急诊专家共识》,并结合最新研究进展以及 AP 诊治的相关专家共识/指南,就 HTG⁃AP 的诊断、 病因治疗、常规治疗及预防等方面进行探讨。

Abstract:

Hyperlipidemia is the second leading cause of acute pancreatitis(AP)in China,and hypertri⁃ glyceridemic acute pancreatitis(HTG⁃AP)is the main type. Due to the special etiology and pathogenesis of HTG⁃ AP,its clinical characteristics are different from those of AP caused by other etiologies,which makes the disease easy to be missed and become severe,resulting in rapid progress of the disease and high mortality,and easy to relapse even after cure. At present,there is a lack of highly operable expert consensus/guidelines for HTG⁃AP. This paper focuses on the diagnosis,etiological treatment,routine treatment and prevention of HTG ⁃AP based on the “Emergency expert consensus on diagnosis and treatment of hypertriglyceridemic acute pancreatitis ”written by us recently and in combination with the latest research progress and relevant expert consensus/guidelines for the diag⁃ nosis and treatment of AP.
