精子DNA早期复发性流产的关系碎片率与男性不育及其配偶

摘要:

目的 探讨精子DNA碎片率(DFI)与男性不育及其配偶早期复发性流产的关系。方法 回顾 性分析 2018 12 月⁃2020 12 月在本中心进行精子 DFI 检测的 330 名男性,依据其配偶生育情况分为不育组(A 组,n = 96)、配偶早期复发性流产组(B 组,n = 86)及正常生育组(C 组,n = 148),比较三组精子 DFI 情况及精子参数。结果 三组年龄比较差异无统计学意义(P>0.05);A 组和 B 组患者精子浓度、精子活 力及前向运动精子均低于 C 组(P<0.01),畸形率高于 C 组(P<0.01)。A 组、B 组和 C DFI ≥ 30%分别为 52.1%、46.5% 6.8%;A 组和 B 组与 C 组比较,DFI 显著升高(P<0.01)。结论 精子 DFI 升高对男性精子 产生负面影响,导致男性不育及其配偶早期复发性流产,应受到重点关注;可将精子 DFI 检测运用于男性 不育评估中,值得临床推广使用。

Abstract:

Objective To investigate the correlation between sperm DNA fragmentation index(DFI)and male infertility or early recurrent abortion occurred in the subjects′ spouses. Methods A total of 330 men who had received sperm DFI test in our center from December 2018 to December 2020 were retrospectively analyzed. According to the fertility status of their spouses,they were divided into infertility group(group A,n = 96),early recurrent abortion group(group B,n = 86),and normal fertility group(group C,n = 148). The sperm DFI index and sperm parameters were compared among the three groups. Results There was no significant difference in age among groups A,B and C(P > 0.05). Concentration of sperms,sperm activity,and rate of sperms with progres⁃ sive motility were lower,while rate of sperms with abnormal morphology was higher in group A or B than in group C(P < 0.01 for all comparisons). DFI ≥ 30% was 52.1% in group A,46.5% in group B,and 6.8% in group C. DFI in group A or B was significantly higher than that in group C(P < 0.01). Conclusions An increase in sperm DFI has a negative impact on male sperm,leading to male infertility and early recurrent abortion,to which should be paid attention. Sperm DFI index could be used in the evaluation of male infertility,which is worthy of clinical application.
