摘要:

目的 比较计算机辅助诊断(CAD)系统与TI⁃RADS及超声评分法在良恶性甲状腺结节诊断效 能上的研究。方法 回顾性分析经手术及细针穿刺病理证实的 672 个甲状腺结节。以病理结果为金标准 构建ROC曲线,比较CAD系统、TI⁃RADS和超声评分法对甲状腺结节良恶性的诊断价值。结果 CAD系统、 TI⁃RADS 分级和超声评分法诊断甲状腺结节的 ROC 曲线下面积分别为 0.889、0.848 0.844。CAD 诊断的 敏感度、特异度、阳性预测值、阴性预测值和准确性均高于 TI⁃RADS 分级和超声评分法诊断(P < 0.05)。 结论 CAD系统可以提供一种更客观、精准、快速的评估方式,有利于提高甲状腺结节的诊断效能,值得推广。

Abstract:

Objective To compare the diagnostic value of computer aided diagnosis(CAD)system,TI⁃RADS classification and ultrasound scoring in the diagnosis of benign and malignant thyroid nodules. Methods Retro⁃ spective analysis was performed in 672 thyroid nodules confirmed by surgery and fine needle aspiration pathology. The ROC curve was constructed with pathological results as the gold standard,and the diagnostic value of CAD system,TI ⁃RADS classification and ultrasound scoring method for benign and malignant thyroid nodules was compared. Results The area under the ROC curve of thyroid nodules diagnosed by CAD system,TI⁃RADS classi⁃ fication and ultrasound scoring method was 0.889,0.848 and 0.844,respectively. The sensitivity,specificity,posi⁃ tive predictive value,negative predictive value and accuracy of CAD system were all higher than those of TI⁃RADS classification and ultrasound scoring method(P < 0.05). Conclusion CAD system is an objective,accurate and rapid evaluation method,which is beneficial to improving the diagnostic efficiency of thyroid nodule,and is worthy of promotion.
