血管内皮生长因子水平对2型糖尿病患者并发视网膜微血管病变的预测价值

Predictive value of vascular endothelial growth factor level in patients with type 2 diabetes mellitus compli⁃ cated with retinal microangiopathy

摘要:

目的 探讨血管内皮生长因子水平(VEGF)对 2 型糖尿病(T2DM)患者并发视网膜微血管病 变(diabetic retinopathy,DR)的预测价值。方法 选择 2018 7 月至 2021 3 月在我院就诊的 T2DM 患者 146 例作为研究对象,根据是否发生 DR 分为 DR 组(n = 67)和非 DR 组(n = 79)。单因素及多因素 Cox 归分析影响 T2DM 患者并发 DR 的因素;根据独立影响因素构建列线图预测模型,并评价其预测效能。 结果 DR 组患者 T2DM 病程明显久于非 DR 组(P < 0.05),在生化指标方面,DR 组患者 FPG、HbA1c、GA Scr、UA、BUN、UACR以及VEGF明显高于非DR组(P < 0.05),而ALB和RBC明显低于非DR组,差异均有统 计学意义(P < 0.05)。T2DM 病程、HbA1c、UA、VEGF T2DM 患者发生 DR 的独立危险因素(P < 0.05), ALB RBC 是保护因素(P < 0.05)。依据危险因素构建的预测模型的 ROC 曲线下面积(AUC)为 0.874 (95%CI:0.825 ~ 0.928,P < 0.001),具有较高的预测价值。结论 VEGF 水平在 DR 患者血清中显著升高, T2DM 患者并发DR 的独立危险因素,对DR 具有较高的预测价值。

Abstract:

Objective To investigate the predictive value of vascular endothelial growth factor(VEGF level in patients with type 2 diabetes mellitus(T2DM)complicated with retinal microangiopathy. Methods A total of 146 patients with T2DM treated in our hospital from July 2018 to March 2021 were selected and were divided into DR group(n = 67)and non⁃DR group(n = 79). Univariate and multivariate Cox regression analysis of the factors affecting T2DM patients with DR;The nomogram prediction model is constructed according to the independent influencing factors,and its prediction efficiency is evaluated. Results The course of T2DM in DR group was significantly longer than that in non ⁃DR group(P < 0.05). In terms of biochemical indexes,FPG,HbA1c,GA SCR,UA,bun,UACR and VEGF in DR group were significantly higher than those in non DR group(P < 0.05), while ALB and RBC were significantly lower than those in non DR group(P < 0.05). The course of T2DM HbA1c,UA and VEGF were independent risk factors for DR in T2DM patients(P < 0.05),ALB and RBC were protective factors(P < 0.05). The area under ROC curve(AUC)of the prediction model constructed according to risk factors was 0.874(95%CI:0.825 ~ 0.928,P < 0.001),which had high prediction value. Conclusion The level of VEGF was significantly increased in the serum of DR patients,which could be an independent risk factor for T2DM patients with DR and has a high predictive value for DR.
