超声在ICU危重症新型冠状病毒肺炎救治中的应用价值

摘要:

目的 重型、危重型新型冠状病毒肺炎(COVID⁃19)的救治是目前阶段疫情防控工作的重点,分析ICU 内重症COVID⁃19 患者超声监测资料,探讨超声的应用价值与特点,期望为日后超声更有效地应用于ICU 重症患者救治起到更好作用。方法 选取于广州医科大学附属第一医院重型及危重型COVID⁃19 患者资料,记录分析胸腔、心脏及血管等各脏器超声资料,统计分析治疗过程中患者对超声的需求度及阳性率,并分析其监控价值。结果 本研究共纳入16例患者,共检查318例次超声,其中依次以心脏、静脉、胸水及消化系统超声检查的临床需求为明显。心脏超声阳性率74%,下肢静脉彩超阳性率60%,颈内静脉彩超阳性率59%;消化彩超阳性率77%,胸水超声阳性率66%。应用ECMO 状态下患者心血管及腹部超声检查阳性率明显高于非ECMO 状态下患者检查阳性率(P<0.05)。结论 超声在ICU 重型、危重型COVID⁃19 患者治疗过程中需求量及检查阳性率较高。并且超声可以良好的综合反映出患者的疾病严重程度。

Abstract:

Objective To analyze ultrasonic monitoring data of severe COVID⁃19 patients in ICU,and toexplore the application value and characteristics of ultrasound. Methods The ultrasonic data of chest,heartand blood vesselsof severe and critical COVID⁃19 patients in the First Affiliated Hospital of Guangzhou MedicalUniversity were recorded;the ultrasound demand and positive rate of patients during the treatment were statisticallyanalyzed. The monitoring value of ultrasound was analyzed. Results Sixteenpatients with 318 times of ultrasoundwere included in this studyand the clinical needs of ultrasound examination in an ascending order were heart,vein,pleural effusion and digestive system. The positive rate of ultrasound was 74% in heart,60% in lower limbvein,59% in internal jugular vein,77% in digestive tract and 66% in hydrothorax.The positive rate of cardiovascu⁃lar and abdominal ultrasound examination in patients with ECMO was significantly higher than that in those without(P < 0.05). Conclusion The demand and positive rate of ultrasound in the treatment of severe and critical COV⁃ID⁃19 patients in ICU are relatively high. Moreover,ultrasound can reflect the severity of the disease.
