摘要：

目的初步探讨联合心理、康复的多学科临床护理路径在老年踝关节骨折治疗中的作用。方法选取 2018 年 6 月至 2019 年 12 月在我院手术治疗的 65 例老年创伤性踝关节骨折，随机分为常规护理组和护理路径组。常规护理组 30 例，护理路径组 35 例。所有患者按照骨折护理常规进行护理，护理路径组根据设计的老年踝关节骨折护理路径进行流程化护理。结果路径护理组总住院日、外伤距离手术时间、平均住院费用均低于常规护理组，差异有统计学意义（P < 0.05）。路径护理组护理干预后 SAS 评分优于常规护理组，差异有统计学意义（P < 0.05）。住院患者护理满意度调查中，路径护理组在健康教育、服务主动性与及时性和服务人性化 3 个项目优于常规护理组，差异有统计学意义（P < 0.05）。末次随访时，路径护理组功能评分优于常规护理组，但差异无统计学意义（P > 0.05）。结论多学科整合护理路径模式，配合外科技术的发展，可以提高老年创伤性踝关节骨折患者的治疗效果，值得临床推广。

Abstract:

Objective To explore the effects of multidisciplinary clinical nursing pathway combined with psychology and rehabilitation for the management of ankle fractures in the elderly. Methods From June 2018 to December 2019，65 cases of geriatric ankle fractures who accepted surgery in our hospital，were randomly divided， with 30 cases in the conventional nursing group and 35 cases in the nursing pathway group.All patients received routine nursing care，as to the nursing pathway group，procedural nursing program carried out according to the newly designed nursing pathway for ankle fractures in the elderly. Results The nursing pathway group proved significantly superior to the conventional group with regard to the average length of stay，interval between trauma and surgery， as well as the average hospital expenses（P < 0.05）. The nursing pathway group had better SAS scores than the conventional nursing group，with significant difference（P < 0.05）. With respect to nursing satisfaction survey of inpatients，the nursing pathway group demonstrated significantly better scores than the conventional nursing group in terms of health education，service initiative and timeliness，as well as service humanization（P < 0.05）. At the last follow ⁃up，the function scores of the nursing pathway group were better than that of the conventional nursing group，which did not reach significant difference（P > 0.05）. Conclusion The multi⁃disciplinary clinical nursing pathway model combined with psychology and rehabilitation，along with the development of surgical techniques， may improve the treatment effect of traumatic ankle fractures in the elderly，which is worthy of clinical promotion.
