食管鳞癌中TOP2A蛋白的表达与临床预后的关系

Correlation studies between TOP2A expression and prognosis in esophageal carcinoma

摘要: 目的 通过检测食管癌组织中TOP2A蛋白的表达,探讨其与食管癌患者预后之间的关系。方法 采用免疫组织化学方法研究TOP2A蛋白在98例食管癌标本中的表达水平,并分析其临床病理特征与食管鳞癌患者5年生存率之间的关系。结果 TOP2A蛋白在98例食管癌组织及55例癌旁组织中的阳性率分别为47.9%和23.6%;采用Kaplan-Meie(K-M)法进行单因素分析并计算生存率,COX风险比例模型进行多因素分析,结果显示分化程度、淋巴结转移、临床分期及TOP2A表达是临床预后的独立危险因素,TOP2A表达阳性患者的5年生存率为19%,而TOP2A表达阴性患者的5年生存率为25%。结论 TOP2A在食管鳞癌中的表达增高,且与食管癌的发生发展、侵袭转移关系密切,TOP2A表达、分化程度、淋巴结转移及临床分期是影响食管鳞癌患者远期预后的主要因素。

Abstract: Objective Investigation of the correlation between expression of TOP2A and prognosis in human esophageal carcinoma by immunohistochemical staining analysis with the aim of evaluating the feasibility for TOP2A as an prognosis marker of esophageal carcinoma. Methods The expression levels of TOP2A in 98 samples of patients with esophageal carcinoma were detected using immunohistochemical method and its correlation with clinical pathological characteristics and prognosis was analyzed. Results The positive rates of TOP2A in 98 cases of esophageal carcinoma tissue and 55 cases of tissue adjacent to carcinoma were 47.9% and 23.6% respectively. The Kaplan-Meier(K-M) method was applied for univariate analysis and COX regression model was applied for multivariate analysis. The yield Results showed that TOP2A expression, differentiation degree, lymph node metastasis and clinical stage were independent factors of prognosis(P < 0.05). The five-year survival rate of patients with TOP2A positive was 19%; The five-year survival rate of patients with TOP2A negative was 25%. Conclusion The elevated TOP2A expression level in esophageal carcinoma has a potential correlation with invasion of the esophageal carcinoma, development and transfer. TOP2A expression, differentiation degree, lymph node metastasis and clinical stage are the main factors that contributes to the prognosis of patients with esophageal carcinoma.
