Measurement of circulation time of head and neck vessels with 4D⁃CTA technology

Abstract:

Objective To explore the feasibility of measuring of arterial and venous circulation time of jugular brain based with 4D⁃CTA data of head and neck range,and to provide reference for CT angiography of head and neck range. Method A retrospective analysis of 298 patients who underwent 4D ⁃CTA imaging of head and neck range from January 2018 to March 2021 was conducted. The CT value and peak time of the arteries(aortic arch,common carotid artery,vertebral artery,middle cerebral artery M1,middle section of basilar artery)and veins(internal jugular vein,sigmoid sinus,superior sagittal sinus)in the head and neck area were measured. The circulation time from aortic arch to intracranial artery,from aortic arch to superior sagittal sinus,from superior sag⁃ ittal sinus to internal jugular vein,the average peak time of arterial system and venous system and the difference between them were calculated. The differences of peak time among patients with different genders,BMI(< 18.5 18.5 ~ 23.9,23.9 kg/m2)and body weight(< 60 kg,60 ~ 80 kg,> 80 kg)were compared. Result In this study the circulation time from aortic arch to intracranial artery ,from aortic arch to superior sagittal sinus and from superior sagittal sinus to internal jugular vein was(1.91 ± 0.78)s,(6.61 ± 1.40)s and(1.52 ± 1.43)s,respectively. The peak time of arterial system and venous system was(21.77 ± 3.09)s and(28.76 ± 3.68)s respectively,and the difference between the peak time of arteriovenous system was(6.99 ± 1.01s). The peak time of aortic arch arterial system,venous system and arteriovenous peak time in male and female patients were(20.65 ± 3.61)s 22.55 ± 2.83)s,(29.90 ± 3.13)s,and(7.36 ± 0.90)s and(19.00 ± 3.05)s,(20.56 ± 3.15)s,(26.99 ± 3.83)s,and(6.44 ± 0.92)s,respectively. The difference was statistically significant(P < 0.05). According to the BMI and body weight,the peak time difference of arteriovenous injection was similar,and the peak time of arteriovenous injection gradually prolonged with the increase of body weight,but no between ⁃group statistical significance was found(P > 0.05). Conclusion Except the earlier peak time and shorter arteriovenous circulation time in women there is no significant difference in peak time and circulation time between patients with different body weights and different BMI. The clinical application of 4D ⁃CTA technology to measure the circulation time of head and neck vessels was feasible,and the interval between the optimal arterial period and the optimal venous period was 6.99 s.
