新冠病毒感染者前白蛋白对淋巴细胞的影响:焦虑和超敏C反应蛋白的中介作用

Effects of prealbumin on lymphocytes in COVID⁃19 patients:mediating role of anxiety and high⁃sensitivity C⁃reactive protein

摘要:

目的 探讨前白蛋白对新冠病毒感染者的焦虑、超敏 C 反应蛋白(hs⁃CRP)和淋巴细胞的影 响及中介作用,为预防及改善新冠肺炎愈后提供依据。方法 采用医院焦虑抑郁量表(Hospital Anxiety and Depression Scale,HADS)测量 212 例新冠病毒感染者的焦虑情况,血液指标如前白蛋白、hs⁃CRP 量和淋巴细胞数量等由医院检验科统一检测和提供结果 本次共纳入 212 例,年龄 19 ~ 59 岁,其中男 139 例,女 73 例。年龄(34.67 ± 9.97)岁,住院时长 2 ~ 27 d。新型冠状病毒感染 108 例(50.9%),新型冠状 病毒肺炎(轻型)71 例(33.5%),新型冠状病毒肺炎(普通型)33 例(15.6%)。血清前白蛋白与焦虑程 度、hs⁃CRP 水平呈负相关(P < 0.001),与淋巴细胞数量呈正相关(P < 0.001);焦虑与淋巴细胞数量呈负相 关(P < 0.01);hs⁃CRP 与淋巴细胞数量呈负相关(P < 0.001)。中介效应由两条路径组成:前白蛋白→ →淋巴细胞的间接效应1,前白蛋白→hs⁃CRP→淋巴细胞的间接效应2。研究结果显示,前白蛋白可以通 过焦虑、hs⁃CRP 的中介作用对淋巴细胞产生影响(P < 0.05)。结论 焦虑、hs⁃CRP 是新冠病毒感染者前白 蛋白对淋巴细胞影响的重要中介机制。

Abstract:

Objective To investigate the effects of prealbumin on anxiety,hypersensitive C ⁃ reactive protein and lymphocytes in patients with COVID⁃19,and to provide reference for preventing COVID⁃19 and im⁃ proving its recovery. Methods The anxiety of 212 patients with COVID⁃19 was measured by Hospital Anxiety and Depression Scale(HADS),and serum prealbumin,high⁃sensitivity C⁃reactive protein and lymphocyte cells were detected by the hospital laboratory. Results A total of 212 people,aged 19⁃59 years,were enrolled in this study including 139 men,with an average age of 34.67 ± 9.97 years. The length of hospitalization was 2 ~ 27 days. One hundred and eight people(50.9%)were infected with COVID⁃19,71 of which(33.5%)was with novel coronavirus pneumonia(mild),and 33 people(15.6%)with novel coronavirus pneumonia(common). Serum prealbumin was negatively correlated with anxiety(P < 0.001)and the level of high⁃sensitivity C⁃reactive protein,and positively with the number of lymphocytes(P < 0.001). Anxiety was negatively correlated with lymphocytes(P < 0.01),and hypersensitive C⁃reactive protein was negatively correlated with lymphocytes as well(P < 0.001). The mediating effect consisted of two paths:the indirect effect 1 of prealbumin→anxiety→lymphocytes and the indirect effect 2 of prealbumin→hs ⁃ CRP→lymphocytes. The results showed that prealbumin could affect lymphocytes through the mediation of anxiety and hs ⁃CRP(P < 0.05). Conclusion Prealbumin exerts influence on lymphocytes through the importantly mediating role of anxiety and hypersensitive C⁃reactive protein in patients with COVID⁃19 infection.
