Analysis of cervical high⁃risk human papillomavirus infection status and subtypes of women in rural areasof Linzhi city,Tibet

摘要:

目的 本研究数据拟对西藏林芝市农村地区女性高危型人乳头状瘤病毒(high⁃risk humanpapillomavirus,HR⁃HPV)感染状况和 HR⁃HPV 亚型分布情况进行分析,为完善国内 HR⁃HPV 流行病学数据和为国内 HPV 疫苗的研发提供数据。方法 收集 2019 年至 2021 年前三季度西藏林芝市农村妇女两癌筛查结果,应用PCR荧光法对筛查的标本进行HR⁃HPV亚型基因检测,统计分析西藏林芝市农村妇女HPV感染率、不同HR⁃HPV亚型感染率以及不同年龄组HR⁃HPV亚型感染率,并与青藏高原以外农村地区妇女 HPV感染率、不同 HR⁃HPV 亚型感染率进行比较分析。结果 共有13 741 例样本纳入研究,西藏林芝市农村地区妇女 HR⁃HPV 总体感染率为 12.81%(1 760/13 741),高于青藏高原以外农村地区妇女 HPV 感染率 7.82%(9 249/118 237)(χ2 = 532.2,P < 0.001)。35 ~ 39 岁年龄组 HPV 感染率最高,为 14.99%(441/2 942),高于青藏高原以外农村地区同年龄组妇女 7.22%(1 827/25 322)(χ2 = 215.9,P < 0.001)。50 ~ 54 岁年龄组 HPV 感染率最低,为 9.47%(209/2 208),与青藏高原以外农村地区同年龄组妇女 HPV 感染率 8.14%(1 604/19 698)差异无统计学意义(χ2 = 4.575,P = 0.032)。西藏林芝市农村地区妇女 HR⁃HPV 感染亚型前三位为 HPV52型(19.94%,351/1 760)、HPV16 型(12.33%,217/1 760)和 HPV58 型(11.88%,209/1 760);青藏高原以外农村地区妇女前三位 HR⁃HPV 感染亚型与西藏林芝市农村地区妇女 HR⁃HPV 感染亚型一致。西藏林芝市农村地区妇女 35 ~ 39 岁年龄组、40 ~ 44 岁年龄组和 45 ~ 49 岁年龄组前三位的 HR⁃HPV 感染亚型分别为HPV52 型、HPV16 型和 HPV58 型,与青藏高原以外农村地区一致。 结论 西藏林芝市农村地区妇女 HR⁃HPV 感染率明显高于青藏高原以外农村地区妇女,两地区前三位 HR⁃HPV 感染亚型一致。

Abstract:

Objective The existing research data lacks the HR ⁃HPV infection status and HR ⁃HP Vsubtype distribution of females in rural areas of Linzhi City,Tibet. This study intended to improve the domesticHR ⁃ HPV epidemiological data,inspire domestic HPV vaccine research and solve the above problem. Methods The results of two⁃cancer screening in rural women in Linzhi City,Tibet from 2019 to the first quarters of 2021were collected. PCR fluorescence were applied to detect HR ⁃HPV subtype genes in the screened specimens. Westatistically analyze HPV infection in rural women in Linzhi City,Tibet. The infection rate of different HR ⁃HPV subtypes and the infection rate of HR⁃HPV subtypes in different age groups were compared with others in ruralareas outside the Qinghai ⁃Tibet Plateau. The infection rate of different HR ⁃HPV subtypes were also analysed.Results A total of 13 741 cases were included in the study. The overall HR⁃HPV infection rate of women in ruralareas of Linzhi City,Tibet was 12.81%(1 760/13 741),which was higher than that of women in rural areas out⁃side the Qinghai⁃Tibet Plateau(7.82%,9 249/118 237)(χ2 = 532.2,P < 0.001). The 35 ~ 39 years old group had the highest HPV infection rate(14.99%,441/2 942)which was higher than that of women in the same agegroup in rural areas outside the Qinghai⁃Tibet Plateau(7.22%,1 827/25 322)(χ2 = 215.9,P < 0.001). The 50 ~54 years old group has the lowest HPV infection rate,9.47%(209/2 208),and 8.14%(1 604/19 698)of HPV infec⁃tion rate of women in the same age group outside the Qinghai ⁃ Tibet Plateau,which was no statistical difference(χ2 = 4.575,P = 0.032). The top three subtypes of HR⁃HPV infection among women in rural areas of Linzhi City,Tibet are HPV52(19.94%,351/1 760),HPV16(12.33%,217/1 760)and HPV58(11.88%,209/1 760). Thetop three HR⁃HPV infection subtypes of women in rural areas outside the plateau were consistent with the HR⁃HPVinfection subtypes of rural women in Linzhi City,Tibet. The top three HR⁃HPV subtypes of women in the 35 ~ 39,40 ~ 44,and 45 ~ 49 age groups in rural areas of Linzhi City,Tibet are HPV52,HPV16,and HPV58 respectively,which were similar to those on the rural areas outside Qinghai⁃Tibet Plateau. Conclusion The HR⁃HPV infectionrate of women in rural areas of Linzhi City,Tibet was significantly higher than that of women in rural areas outsidethe Qinghai⁃Tibet Plateau. The top three HR⁃HPV infection subtypes in two regions were the same.
