癫痫相关穴位电极记录伴中央-颞区棘波的小儿良性癫痫患儿棘波的特点分析

摘要:

目的 选用癫痫相关头部穴位作为脑电图(EEG)电极安放位置观察记录伴中央⁃颞区棘波 的小儿良性癫痫(BECTs)患儿发作间期 EEG 棘波的变化特点,并与传统方法比较。方法 选择 BECTs 30例,先采用国际10⁃20系统电极安放方式记录作为对照组,后采用选定穴位进行电极安放并记录作为 穴位组,记录两组 EEG 变化特点并分析。结果 穴位组 EEG 可记录到 α 节律及睡眠周期变化。两组记录 α 节律、顶区棘波波幅最大值差异无统计学意义(P < 0.05)。清醒期穴位组棘波的波幅较对照组更高 P < 0.05),而睡眠期穴位组棘波波幅均值大于对照组,但差异无统计学意义(P > 0.05)。清醒期棘波波 幅约 78.9%的最高位置为率谷穴,而睡眠期约为 73.3%。结论 选用癫痫相关头部穴位作为电极安放位置 记录BECTs 患儿EEG,可显示其特征性表现;棘波多数以率谷穴波幅最高。

Abstract:

Objective To explore the characteristics of spike waves in children with benign epilepsy with central⁃temporal spikes(BECTS)by comparing the method of using epilepsy⁃related acupoint electrodes with the traditional method to record spike waves in centrotemporal region. Methods Thirty children with BECTs were selected. The spike waves were recorded first by placing the electrodes according to the international 10⁃20 system and then by placing the electrodes on epilepsy ⁃ related acupoints. The two groups of data were analyzed. Results The changes in α rhythm and sleep cycle were recorded in the acupoint group. There was no significant difference between the recorded α rhythm as well as the maximum amplitude of spike waves in the apical area between the two groups(P > 0.05). During awakeness,the amplitude of spike waves in the acupoint group was significantly higher than that in the control group(P < 0.05),while during sleeping the average amplitude of spike waves in the sleep point group was insignificantly higher than that in the control group(P > 0.05). The peak of spike wave amplitude of(about 78.9%)during awakeness was recorded at the acupoint of Shuai Gu,while it was about 73.3% during sleeping. Conclusion The epilepsy⁃related scalp acpoints used as positions for electrode placement to record EEG of BECTs children can show their characteristic features. Most spike waves have the highest amplitude at the acu⁃ pint of Shuai Gu.
