Heterogeneous Nuclear Ribonucleoprotein U expression in patients with severe oligoasthenospermia and its effect on clinical outcome of intracytoplasmic sperm injection

摘要:

目的 探讨严重少、弱精子症患者精液精子中 HNRNPU 的表达及其与精子质量参数的相关 性,探讨其对卵泡浆内单精子显微注射(ICSI)治疗临床结局的影响。方法 选取严重少、弱精子症需行 ICSI 治疗患者和正常生育男性各 500 例,收集精液,采用 RT⁃PCR 检测精子 HNRNPU mRNA;计算机辅助精 液分析系统(CASA)分析精液常规参数;Diff⁃Quik 染色法分析精子形态;采用钙离子载体 A23187 诱导精子 发生顶体反应(ARIC);SCD 法检测精子 DNA 碎片化指数(DFI);苯胺蓝染色法检测精子核成熟度;记录 ICSI 组的治疗结局。结果 ICSI HNRNPU mRNA 相对量显著高于正常组精子[(0.38 ± 0.04)vs.(0.14 ± 0.03),P < 0.05],ICSI 组正常形态精子百分率、ARIC%和核成熟度均低于正常组,差异均有统计学意义 P < 0.001);HNRNPU mRNA 与前向运动精子总数、正常形态精子百分率、ARIC%和核成熟度呈负相关, DFI 呈正相关(P < 0.001);选择研究时间内,女方采用长效长方案促排卵,并行新鲜胚胎移植的 104 ICSI 治疗周期,男方精子 HNRNPU 基因 mRNA 与前向运动精子总数和正常受精(2PN)率呈负相关(P < 0.05),但与卵裂率、优胚率和临床妊娠率无显著相关性。结论 精子 HNRNPU mRNA 表达上调可能是严 重少、弱精子症发病的原因之一;对ICSI 受精率有一定的预测作用。

Abstract:

Objective To explore the correlation between heterogeneous nuclear ribonucleoprotein U (HNRNPU)expression and sperm quality parameters in patients with severe and weak sperm disease,and to explore its effect on the clinical outcome of intracytoplasmic sperm injection(ICSI). Methods Totally 500 semen specimens from men with severe oligoasthenozoospermia requiring ICSI treatment (ICSI group)and 500 from normal men(control group)were collected at our clinic. The sperm HNRNPU mRNA,volume,concentration morphology,total progressive motile sperm count,Ca2 + ionophore(A23187)⁃induced arcrosome reaction(ARIC rate,sperm DNA fragmentation index(DFI)and sperm nucleation were detected and outcomes in the ICSI group were recorded. Results The ICSI group(0.38 ± 0.04)showed significantly higher expression of HNRNPU mRNA than the control group(0.14 ± 0.03)(P < 0.05),and the normal morphology sperm,ARIC% and maturity in ICSI group were significant lower than those in the control group(P < 0.001). HNRNPU mRNA was negatively related with total progressive motitle sperm count,normal morphology sperm,ARIC% and maturity,but positively with DFI (P < 0.001). HNRNPU mRNA was negatively related with normal fertilization(2PN)rate(P < 0.05),but not with cleavage rate or clinical pregnancy rate of ICSI. Conclusion The increase of sperm HNRNPU mRNA expression may be one of the reasons for the onset of severe oligoasthenozoospermia. It can predict the fertility rate of ICSI to a certain extent.
