基于每搏输出量最大化的目标导向液体治疗在断指再植手术中的应用

摘要:

目的 观察采用每搏输出量最大化的目标导向液体治疗策略在断指再植手术中的应用效 果。方法 选择手外科急诊断指再植手术,年龄 18 ~ 60 岁,ASA 分级Ⅰ-Ⅱ级患者 40 例,均在超声引导下 行肌间沟入路臂丛神经阻滞,根据临床试验方法的不同随机分成两组:常规补液组(C 组)、目标导向液 体治疗组(G 组),每组 20 例。术中记录患者入室后(T0)、手术开始前(T1)、手术结束时(T2)三个时刻的平 均动脉压(MAP)、每搏输出量(SV)、心输出量(CO)指标值,并于 T1、T2时刻采集患者桡动脉血检测血 乳酸(Lac)、IL⁃1β、IL⁃10 值。结果 胶体输入量 G 组明显多于 C 组(P < 0.05),晶体液输入量差异无统计 学意义(P > 0.05);两组桡动脉血 MAP、SV、CO、Lac、IL⁃1β IL⁃6 值分别在 T1、T2时刻差异有统计学意义 P < 0.05)。结论 基于每搏输出量最大化的目标导向液体治疗应用于断指再植手术中,利于维持患者血 流动力学稳定,能动态优化微循环灌注,降低动脉血乳酸浓度,抑制 IL⁃1β、IL⁃6 炎症因子的表达,减轻因 使用止血带等因素造成的缺血再灌注损伤。

Abstract:

Objective To observe the effect of goal ⁃directed fluid therapy based on maximum stroke volume in replantation of severed fingers. Methods Forty ASAⅠ-Ⅱ patients,aged 18 to 60,were randomly divided into two groups according to different experimental methods :normal control group(group C)and goal⁃ directed fluid therapy group(group G),20 for each group. Mean arterial pressure(MAP),stroke volume(SV and cardiac output(CO)were recorded at the time points of entering the operating room(T0),before surgery(T1 and at the end of surgery(T2). Radial artery blood was collected at T1 and T2 to detect blood lactic acid(Lac), IL⁃1β and IL⁃10. Results The amount of colloid input in group G was significantly higher than that in group C (P < 0.05),but there was no significant difference in the amount of crystalloid fluid input(P > 0.05);the values of MAP,SV,CO,Lac,IL⁃1β and IL⁃6 in radial artery blood between the two groups at T1 and T2 were statistically significant(P < 0.05). Conclusions Application of goal⁃directed fluid therapy based on maximum stroke volume in replantation of severed fingers can maintain hemodynamic stability,dynamically optimize microcirculation perfu⁃ sion,reduce arterial blood lactic acid concentration,inhibit expressions of IL⁃1β and IL⁃6 inflammatory factors
