肉芽肿性乳腺炎病理组织中NLRP3、caspase-1、IL-17表达及临床意义

Expressions and clinical significance of NLRP3, caspase-1 and IL-17 in pathological tissues of granulomatous mastitis

摘要:

目的 探讨NOD样受体热蛋白结构域相关蛋白3(NLRP3)、半胱天冬酶-1(caspase-1)及白细胞介素-17(IL-17)在肉芽肿性乳腺炎(GM)病理组织中的表达水平,并分析其临床意义。 方法 选取2021年1月至2025年12月湖南省人民医院收治的60例GM患者手术切除的病理组织作为GM组,另选取同期60例乳腺良性病变周围正常乳腺组织作为对照组。采用免疫组织化学SP法检测两组组织中NLRP3、caspase-1、IL-17的表达水平,分别根据上述3个指标的检测结果各自分为阳性组和阴性组。记录患者临床病理学特征,采用 χ2 检验比较NLRP3、caspase-1、IL-17阳性和阴性表达组间差异。分析不同疾病活动期的GM患者NLRP3、caspase-1、IL-17表达差异。以P < 0.05为差异有统计学意义。 结果 GM组织中NLRP3、caspase-1及IL-17的阳性表达率均显著高于对照组(P < 0.05)。不同临床分期GM中,脓肿期、溃后期患者NLRP3、caspase-1、IL-17阳性表达率显著高于肿块期(P < 0.05),且溃后期阳性表达率略高于脓肿期,但组间差异无统计学意义(P > 0.05)。肿块最大径≥ 5 cm者NLRP3、caspase-1、IL-17阳性表达率均显著高于肿块最大径< 5 cm者(P < 0.05)。 结论 NLRP3、caspase-1、IL-17在GM病理组织中均呈高表达状态,三者表达水平与GM临床分期、肿块大小有关,NLRP3、caspase-1、IL-17可能参与GM的炎症发生与进展过程。

Abstract:

Objective To explore the expressions of NOD-like receptor thermal protein domain associated protein 3 (NLRP3), cysteinyl aspartate specific proteinase-1 (caspase-1), and interleukin-17 (IL-17) in the pathological tissues of granulomatous mastitis (GM), and to analyze their clinical significance. Methods Between January 2021 and December 2025, pathological tissues were obtained from 60 patients diagnosed with granulomatous mastitis (GM) at Hunan Provincial People's Hospital, while normal breast tissues were collected from 60 patients with benign breast lesions at the same hospital. These samples were respectively designated as the GM group and the control group. The expression levels of NLRP3, caspase-1, and IL-17 in the two groups were detected by means of the immunohistochemical streptavidin-peroxidase (SP) method. Based on the detection results of the above three indicators, the samples were classified into a positive group and a negative group. The clinicopathological characteristics of the patients were recorded, and the differences between the positive and negative groups (NLRP3, caspase-1, IL-17) were compared using the χ2 test. The differences in NLRP3, caspase-1, and IL-17 among GM patients at different disease activity stages were analyzed. A P-value less than 0.05 indicated that the difference was statistically significant. Results The positive expression rates of NLRP3, caspase-1, and IL-17 in the GM group were significantly higher compared to those in the control group (P < 0.05). In different clinical stages of GM, the positive expression rates of NLRP3, caspase-1, and IL-17 in the abscess stage and the ulcerative stage were significantly higher than those in the lump stage (P < 0.05). Additionally, the positive expression rates in the ulcerative stage were slightly higher than those in the abscess stage, yet there was no significant difference among different groups (P > 0.05). The positive expression rates of NLRP3, caspase-1, and IL-17 in patients with a maximum tumor diameter of ≥ 5 cm were significantly higher than those in patients with a maximum tumor diameter of < 5 cm (P < 0.05). Conclusions The expressions of NLRP3, caspase-1, and IL-17 are up-regulated in GM tissues. All three of these molecules are associated with the clinical staging of GM and lesion size. NLRP3, caspase-1, and IL-17 may be involved in the inflammation and progression of GM.
