Fulminant myocarditis(FM)is a relatively rare but serious clinical syndrome. It usually accom⁃ panied by severe hemodynamic changes.It presents with as rapid onset hypotension,cardiogenic shock,or even death. currently,there is no specific drug treatment. Mechanical circulatory support(MCS)has become a key mea⁃ sure to help patients with explosive myocarditis through the acute phase,as it can replace part of the heart and/or lung functions,maintain stable circulation. As a treatment method for explosive myocarditis,Mechanical circulatory support has become a mature technology in abroad,but it has been developed relatively late and experience is rela⁃ tively lackingin china. This article reviews the research progress of mechanical circulatory support in the application of fulminant myocarditis.

Objective To investigate the impact and potential regulatory mechanisms of Umbilical Cord Mesenchymal Stem Cells(UCMSCs)on the immune microenvironment of autoimmune hepatitis(AIH). Methods Peripheral blood mononuclear cells(PBMCs)from AIH patients co⁃cultured with UCMSCs at a ratio of 10∶1 (Co⁃culture 1)or 5∶1(Co⁃culture 2),respectively. Flow cytometry was used to detect Th17,Treg and Th17/Treg ratio;ELISA was used to detect the levels of transforming growth factor β1(TGF ⁃β1),interleukin ⁃17(IL ⁃17) and IL ⁃ 6 in the supernatant. RNA ⁃ seq was used to screen the differential genes before and after co ⁃ culture of PBMCs,and the top 15 most highly expressed mRNAs were enriched for subsequent bioinformatic analysis. western blot and RT⁃qPCR were used to determine the expression levels of Foxp3 and RORγt. Results UCMSCs significantly decreased CD3+ CD8⁃IL⁃17A+ Th17 ratio and increased CD25+ Foxp3+ CD4+ Treg ratio. Compared with the PBMCs group,TGF⁃β1 was significantly upregulated(P < 0.01)and IL⁃17 and IL⁃6 were significantly downregu⁃ lated(P < 0.05)in the Co ⁃culture 2 group. Transcriptome sequencing analysis revealed that the transcription factors were mainly focused on immune response and T cell activation;among them,SMAD3 and STAT3 occupied a more central position. Protein and mRNA assays showed that the expression levels of Foxp3 and SMAD3 were significantly upregulated in the Co⁃culture 2 group(P < 0.01),and the expression levels of RORγt and STAT3 were significantly downregulated(P < 0.01). Conclusion UCMSCs promote the differentiation of Th17 to Treg cells through the TGF⁃β pathway,suppress inflammatory responses to improve the hepatic immune microenvironment.

Objective To investigate the effect of electroacupuncture on articular cartilage and subchon⁃ dral polarization ⁃ related protein expression in aged rats. Methods Sixteen 24⁃month ⁃old SD male rats were randomly divided into old group and electroacupuncture group,with 8 rats in each group. Eight SD male rats aged 6 months were youth group. Electroacupuncture group received electroacupuncture treatment,and the two sides of the“Taixi”,“Zusanli”,“Yanglingquan”,“xuehai”were selected. Electroacupuncture parameters:density wave, sparse wave frequency 3 Hz,dense wave 15 Hz,current intensity 1 mA,once a day,30 minutes each time,5 days a week,consecutive 8 weeks. The other two groups fixed sterile needles with adhesive tape,connected the elec⁃ troacupuncture instrument but did not turn on. After 8 weeks,the content of C⁃terminal peptide(CTX⁃Ⅱ)in serum of type Ⅱ collagen was determined by ELISA. Microscopic CT(Micro⁃CT)was used to detect the microstruc⁃ ture of the left tibia subchondral bone. The left tibial plateau cartilage was observed under light microscope with saffron⁃solid green staining,and the degree of articular cartilage degeneration was evaluated by modified Mankin′s score. RT⁃qPCR and Westernblot were used to detect matrix metalloproteinase 1(MMP⁃1),matrix metallopro⁃ teinase 3(MMP⁃3),matrix metalloproteinase 13(MMP⁃13),tumor necrosis factor α(TNF⁃α),interleukin 1β (IL⁃1β),and macrophage mannose⁃receptor(MMR). Expression levels of arginase 1(Arg1)mRNA and protein. Results (1)Compared with the young group,the CTX⁃Ⅱin the blood of the old group increased(P < 0.05),Saf⁃ fron O⁃fast green staining showed that the structure of rat cartilage was seriously damaged,cartilage and subchon⁃ dral bone decomposed indistinctly,calcification was severe,multiple tidal lines appeared,the number of cells decreased significantly,split layer appeared,cell morphology and structure was abnormal,and staining was uneven. and the modified Mankin′s score increased(P < 0.05). Bone mineral density,bone volume fraction and number of bone trabeculae decreased(P < 0.05),trabecular thickness and trabecular spacing increased(P < 0.05),MMP1, MMP3,MMP13 mRNA and protein expression increased(P < 0.05),TNF⁃α,IL⁃1 β mRNA and protein expres⁃ sion increased(P < 0.05),MMR,Arg1mRNA and protein decreased(P < 0.05).(2)Compared with the senile group,the CTX⁃Ⅱdecreased in the electroacupuncture group(P < 0.05),The saffron O⁃fast green staining showed that the cartilage of the rats was slightly smooth and there was no obvious cleavage layer. The number of chondro⁃ cytes in the aged group was higher than that in the aged group,and the staining was more uniform;and the modi⁃ fied Mankin′s score was decreased(P < 0.05). Bone mineral density,bone volume fraction and trabecular number increased(P < 0.05),trabecular spacing decreased(P < 0.05),trabecular thickness tended to decrease(P > 0.05),MMP1,MMP3,MMP13mRNA and protein expression decreased(P < 0.05),TNF⁃α,IL⁃1 β mRNA and protein expression decreased(P < 0.05),MMR,Arg1mRNA and protein increased(P < 0.05). Conclusion Electroacupuncture may protect joints by promoting OA articular cartilage and subchondral bone M2 polarization, alleviating articular cartilage degradation and inhibiting subchondral bone osteoporosis in aged rats.

Establishment of breast cancer organoid models and its sensitivities to chemotherapeutic drugs

Objective To establish and characterize patient ⁃derived breast cancer organoids ,and to explore their potential role as promising preclinical models for individualized chemotherapeutic drug sensitivity assays. Methods Breast cancer tissues(different molecular subtypes)of 4 female patients who underwent radical mastectomy were obtained. Freshly resected tumors were collected and cells were isolated. Isolated cells were plated in Matrigel drops and overlaid with organoid culture medium for three ⁃dimensional culture. Tissues and organoids were fixed,followed by dehydration,paraffin embedding,and serial sectioning. Tissues and organoids sections were subjected to H&E and immunohis⁃tochemical staining(ER,PR,HER2,and Ki⁃67)in order to investigate whether the histological characteristics of the parental tumors were maintained in the breast cancer organoids. After treatment of breast cancer organoids with different concentrations of chemotherapeutic drugs for 5 days,cell viability was determined using the CellTiter⁃Glo® 3D reagent and drug sensitivities were analyzed. Results Breast cancer organoids recapitulated the histological features and marker expressions of their parental tumors. And the sensitivity of organoid chemotherapeutic drugs of different molecular subtypes in breast cancer was different. Conclusion The in vitro culture system of breast cancer organoids has been successfully established in this study,which can reflect the distinct molecular subtypes and histological features of the parent tumor,and can be used for in vitro chemotherapy drug test. These data suggest that breast cancer organoids may be an excellent preclinical model for studying clinical response to chemotherapeutic drugs in a personalized way,as well as for providing reference for clinic drug use.

Objective To investigate the expression of MicroRNA⁃27a(miR⁃27a)in the placenta of gesta⁃ tional diabetes mellitus(GDM)rats and its effects on oxidative stress and inflammation in GDM rats,as well as to analyze the underlying mechanism. Methods SD normal pregnant rats were injected intraperitoneally with strepto⁃ zotocin(STZ)to induce the establishment of GDM models. After successful modeling,they were divided into four groups:model group,agomir ⁃NC group,miR ⁃27a agomir group,and miR ⁃27a agomir + lipopolysaccharide(LPS) group,eachwith 12 rats;the control group consisted of12 normal pregnant rats. The levels of fasting blood glucose (FBG)and fasting insulin(FINS)in rats were detected,and the insulin resistance index(HOMA⁃IR)was calculated; the pregnancy outcome and placenta quality were analyzed;the levels of interleukin 1β(IL⁃1β)and tumor necrosis factor α(TNF⁃α),the activities of superoxide dismutase(SOD)and catalase(CAT),and the content of malondialde⁃ hyde(MDA)in placenta tissuewere determined;Hematoxylin⁃Eosin(HE)staining and TUNEL method were used to detect pancreatic and placental tissue damage and cell apoptosis;and real ⁃time quantitative PCR(RT ⁃ qPCR)and Western blot were used to detect the levels of miR⁃27a,Toll⁃like receptor 4(TLR4),myeloid differentiation factor 88 (MyD88),nuclear factor⁃κB p65(NF⁃κB p65)mRNAs and proteins in placental tissues. Results Overexpression of miR⁃27a significantly reduced FBG,FINS and HOMA⁃IR in GDM rats,accompanied by a reduction in pro⁃inflam⁃ matory cytokine levels and oxidative stress,improved placental and pancreatic tissue damage and apoptosis,and the pregnancy outcome,and inhibited the activation of the TLR4/MyD88/NF ⁃κB signaling pathway in the placenta (P < 0.05). Conclusion The overexpression of miR ⁃27a can reduce the placental inflammation,oxidative stress, and IRin GDM rats. Its mechanism could be related to the inhibition of the TLR4/MyD88/NF⁃κB signaling pathway

Objective To explore the effects of dimethyl fumarate(DMF)on hydrogen peroxide(H2O2)⁃ induced ferroptosis of human cardiac microvascular endothelial cells (HCMEC) and its potential mechanism. Methods HCMEC were cultured in vitro,incubated with H2O2,and then treated with different concentrations of DMF. Cell proliferation was measured by CCK ⁃ 8 method. The level of cell inflammation was detected by ELISA. Western blot was used to detect the changes of ferroptosis⁃related proteins GPX4 and ACSL4,as well as Nrf2/HO⁃1 and JAK2/STAT1 pathway proteins. Results The results of CCK8 assay showed that compared those in the control group,H2O2 significantly inhibited the proliferation of HCMEC cells,whereas,DMF treatment promoted H2O2 ⁃ induced cell proliferation in a concentration dependent manner(P < 0.05 for all). H2O2⁃induced increased levels of inflammatory cytokines TNF⁃α,IL⁃1β and IL⁃18 in HCMEC cells were reversed by DMF treatment(P < 0.05). Western blot revealed that H2O2 treatment caused the decrease in GPX4 protein and increase in ACSL4 protein; whereas DMF inhibited ferroptosis of HCMEC cells induced by H2O2(P < 0.05). In addition,H2O2 induced a decrease in Nrf2 and HO⁃1 expression,while an increase in JAK2 and STAT1 phosphorylation levels in HCMEC (P < 0.05). Compared with those in the H2O2 group,DMF increased Nrf2 and HO⁃1 proteins and down⁃regulated the phosphorylation levels of JAK2 and STAT1(P < 0.05). Finally,it was found that Nrf2 inhibitor ML385 signifi⁃ cantly weakened the inhibition of ferroptosis and the protective effect of HCMEC damage. Conclusion DMF inhib⁃ its H2O2 ⁃induced inflammation of HCMEC and ferroptosis. Nrf2/HO⁃1 and JAK2/STAT1 pathways may play a key role in DMF⁃mediated protection of HCMEC.

Objective To evaluate the effect of a new type of narrow sterile hemostatic ring on blood loss during upper limb lymphedema aspiration. Methods 150 patients with upper limb lymphedema were treated with four different types of aspiration. In the first group,the operation was performed under the condition of hemostasis ring and autotransfusion. In the second group,the operation was performed without the use of hemostasis ring and autotransfusion. In the third group,the operation was performed with the hemostatic ring and without autotransfusion. In the fourth group,the operation was performed without the use of hemostasis ring and autotransfusion. The blood loss,surgical effect and postoperative complications of patients in different groups were compared. Results In the hemostatic ring aspiration group(Group 1 and Group 3),the hemoglobin decrease was(16.90 ± 8.45)g/L,and the blood loss was(328.43 ± 171.54)mL. In the conventional aspiration group(Group 2 and Group 4),the hemoglobin decrease was(20.95 ± 9.55)g/L,and the blood loss was(442.42 ± 172.08)mL. The hemoglobin decrease and the blood loss index in the hemostatic ring aspiration group and the conventional aspiration group were statistically significant(P < 0.05),Using a narrow sterile hemostatic collar can significantly reduce blood loss. The hemoglobin decrease(18.42 ± 8.75)g/L and blood loss(381.00 ± 194.02)mL were observed in the autologous blood group (Group 1 and Group 4),while the hemoglobin decrease(20.15 ± 9.64)g/L and blood loss(415.00 ± 171.96)mL were observed in the non autologous blood group(Group 2 and Group 3). There was no statistically significant difference in the hemoglobin decrease and blood loss indicators between the two groups(P > 0.05),The use of autologous blood transfusion has no significant impact on blood loss and postoperative hemoglobin decline in patients. The surgical time for group 1 was(97.82 ± 24.90)min,for group 2 was(97.74 ± 22.48 )min,for group 3 was (88.83 ± 21.06)min,and for group 4 was(104.82 ± 16.25)min,with no statistically significant difference(P > 0.05). The use of narrow sterile hemostatic ring and autologous blood transfusion did not increase the surgical time. The amount of fat sucked out in group 1 was(1 380.77 ± 482.81)mL,the amount of fat sucked out in group 2 was (1 122.95 ± 382.43)mL,the amount of fat sucked out in group 3 was(1 275.00 ± 478.87)mL,and the amount of fat sucked out in group 4 was(1 272.73 ± 610.07)mL,with a statistically significant difference(P < 0.05). Using a narrow sterile hemostatic collar and autologous blood transfusion can significantly increase the amount of fat tissue sucked out. After surgery,patients in different groups experienced different degrees of limb sensory abnor⁃ malities and limited range of motion,with no statistically significant difference(P > 0.05). Conclusions The use of narrow sterile hemostatic ring for upper limb lymphedema aspiration can effectively reduce the amount of bleeding. 
Analysis of 85,525 cases of rare autosomal trisomy detected by noninvasive prenatal testing

Objective To provide further data for clinical treatment of rare autosomal trisomy(RATs), through the analysis of the results of RATs screened by non⁃invasive prenatal examination(NIPT). Method A total of 85,525 pregnant women who received NIPT in Maternity and Child Health Hospital Affiliated to Anhui Medical University from January 1,2018 to June 1,2022 were selected. Invasive diagnosis was performed for women with high risk results and pregnancy outcome was followed. Result Among the 85,525 samples,182 cases of high risk of RATs were detected. A total of 136 cases chose for further diagnosis,and 7 cases were confirmed,including 2 cases of T2,1 case of T7,1 case of T9,1 case of T15,and 2 cases of T16. Positive predictive value(PPV) were 50%,3.13%,12.5%,33.3%,and 12.5%,respectively,and the PPV of other chromosomes was 0. The total PPV was 5.15%. Detailed follow⁃up was conducted in 121 high⁃risk pregnant women,and 116(95.87%)had a live birth,the incidence of fetal growth restriction(FGR)and preterm birth(PTB)were 25.62%,20.66% respectively. Conclusion RATs screened by NIPT has a lower PPV ,but the high risk of RATs has important clinical significance,suggesting that pregnant women are at increased risk of FGR and PTB and need enhanced ultrasound follow⁃up.

Objective To explore the application effect of fetal movement perception on fetal intrauterine status monitoring during late pregnancy,and to provide reference for routine management and high⁃risk screening of pregnant women. Methods A total of 3 000 cases of primiparae with intrauterine single live fetus having prenatal examination and planning to be hospitalized for delivery in Nanfang Hospital,Southern Medical University from January 2021 to January 2023 were prospectively selected. They were divided into fetal movement counting group (1 500 cases)and fetal movement perception group(1 500 cases),monitored by fetal movement counting and fetal movement perception respecitvely. The general data of the pregnant women,compliance with monitoring fetal move⁃ ment,time of abnormal fetal movement,and the monitoring effect and improvement of fetal intrauterine condition of two fetal movement monitoring methods were descriptively analyzed in terms of frequency,standard deviation, median and quartile. Results The score of Frankl′s Behavior Rating Scale of the pregnant women in fetal move⁃ ment perception group was higher than that in fetal movement counting group,and the abnormal time of fetal move⁃ ment in fetal movement perception group was shorter than that in the fetal movement counting group(P < 0.05). when in critical condition,the monitoring effect(100.00%)/ the improvement effect of fetal movement perception on fetal intrauterine condition(10.99%)was better than fetal movement count(62.59%)/(-59.78%),showing statistical significance. Conclusion The fetal movement perception to monitor intrauterine condition during late pregnancy can improve the compliance of fetal movement monitoring in pregnant women,shorten the time of abnor⁃ mal fetal movement,improve fetal intrauterine condition and neonatal outcomes,and is worthy of clinical promotion.

Objective To investigate the expression and clinical significance of signal peptide⁃CUB⁃EGF⁃ like domain⁃containing protein 2(SCUBE2)in peripheral blood and placenta of pregnant women with preeclampsia. Methods A total of 58 patients with preeclampsia who were hospitalized for delivery in Hospital affiliated to Xuzhou Medical University from June 2021 to June 2022 were selected,including 30 patients with early⁃onset preeclampsia(weeks of onset < 34 weeks)and 28 patients with late⁃onset preeclampsia(weeks of onset ≥ 34 weeks); another 30 pregnant women with full⁃term normal delivery were selected as the control group. Serum samples were collected from pregnant women before admission and fresh placental tissue samples during delivery. The expression levels of SCUBE2 and VEGFR2 in the serum of the three groups were detected by ELISA. The localization and expression of SCUBE2 in the placental tissues of the three groups were detected by immunohistochemistry. The expression levels of SCUBE2 and VEGFR2 in the placental tissues of the three groups were detected by Western blot assay and real⁃time PCR(RT⁃qPCR)assay. Results Immunohistochemistry showed that SCUBE2 was mainly expressed in placental syncytiotrophoblasts and placental vascular endothelial cells. The expression level of pre⁃ eclampsia group was lower than that of control group,and the expression level of early ⁃onset preeclampsia group was lower than that of late⁃onset preeclampsia group;The results of Western⁃blot and RT⁃qPCR showed that there were significant differences in the relative expression levels of SCUBE2 and VEGFR2 protein and mRNA in the pla⁃centa of the three groups(P < 0.05). ELISA showed that the expression levels of SCUBE2 and VEGFR2 in serum of pregnant women in the three groups were significantly different(P < 0.05). The preeclampsia group was lower than the control group,and the expression level in the early ⁃onset preeclampsia group was lower than that in the late⁃onset preeclampsia group(P < 0.05),And there was a positive correlation between serum SCUBE2 and VEGFR2 expression in preeclampsia group. Conclusion The decreased expression of SCUBE2 and VEGFR2 in maternal serum and placental tissue may be related to the occurrence and development of preeclampsia.

Objective To evaluate the effect of driving pressure⁃guided individualized positive end⁃expira⁃ tory pressure(PEEP)on intracranial pressure and regional cerebral oxygen saturation(rSO2)in elderly patients undergoing robot ⁃ assisted laparoscopic radical prostatectomy(RALP). Methods Eighty patients with elective general anesthesia for RALP,aged 65 to 80 years,with BMI 19 to 28 kg/m2 ,and ASA classification I to Ⅲ,were selected. A randomized number table method was used to divide the patients into conventional pulmonary protective ventilation(group A)and individualized PEEP with driving pressure guidance(group B). In group A,5 cmH20 PEEP was fixed 10 minutes after the Trendelenburg position was adjusted for low tidal volume lung protection venti⁃ lation;In group B,PEEP titration was performed after the establishment of pneumoperitoneum position and supine position. The air was ventilated with the PEEP until the tracheal tube was removed. The mean arterial pressure (MAP)was recorded immediately before anesthesia induction(T0),5 minutes after endotracheal intubation(T1), 5 minutes after CO2 pneumoperitoneum establishment(T2),5 minutes after pneumoperitoneum Trendelenburg position(T3),5 minutes after positive end⁃expiratory pressure titration(T4,10 minutes after PEEP ventilation in group A),60 minutes after positive end⁃expiratory pressure titration(T5),and 5 minutes after surgery(T6). Systolic blood flow velocity(VS),blood flow velocity diastolic period(VD),mean blood flow velocity(VM),and pulsation index(PI)of the middle cerebral artery were measured,and cerebral perfusion pressure(CPP)were calculated; T1⁃6 lung compliance(Cdyn),arterial partial pressure of oxygen(PaO2)and arterial partial pressure of carbon dioxide(PaCO2)and T0 ⁃ 6 rSO2 were recorded;Pulmonary complications were recorded one week after operation. Results Compared with group A,group B had higher Cdyn and PaO2 at T5⁃6 and a significantly lower incidence of pulmonary complications(P < 0.05),with no significant differences in MAP,CPP and SO2 between the two groups (P > 0.05). Conclusion Individualized PEEP guided by driving pressure in RALP is safe and effective. It can improve the pulmonary oxygenation function and compliance during operation,reduce the incidence of postopera⁃ tive pulmonary complications,and have no significant impact on cerebral hemodynamics and rSO2.

Objective To investigate the value of secretory leukocyte protease inhibitor(SLPI)in serum and lower respiratory tract secretion in severe pneumonia. Methods 90 patients with severe pneumonia from the Intensive Care Unit(ICU)of the Third People′s Hospital of Hefei were selected for clinical data. They were respectively divided into a survival group and a death group based on their28⁃day survival status. Baseline data,serum and lower respiratory tract secretions(SLPI)and serum Tumor necrosis factor⁃α(TNF⁃α),Interleukin⁃8(IL⁃8)levels were compared between the two groups at admission to ICU and after 24 hours treatment.Analyzing the correlation between serum SLPI and lower respiratory secretions SLPI,TNF ⁃ αand IL ⁃8. Conduct binary logistic regression analysis on the influencing factors of prognosis for severe pneumonia. And used receiver operating characteristic (ROC)curve to analysis the evaluation value of patient serum and lower respiratory tract secretions(SLPI)for the prognosis of severe pneumonia. Results The mechanical ventilation time in the death group was longer than that in the survival group,and there was no significant difference in the remaining baseline data(P > 0.05). After 24 hours of treatment,the serum and lower respiratory tract secretion SLPI of both groups increased compared to before,and serum TNF⁃α,IL⁃8 levels decreased(P < 0.05). After treatment,the SLPI of serum and lower respira⁃ tory tract secretions in survival group were higher than those in the death group,and the serum TNF⁃α,IL⁃8 levels were lower than those in the death group(P < 0.05). Spearman correlation analysis found that serum SLPI and TNF⁃ α,IL⁃8 showed a negative correlation(r = -0.583,-0.672,P < 0.001);And there was a positive correlation with lower respiratory tract secretion SLPI(r = 0.527,P < 0.001). The binary logistic regression analysis showed that serum and lower respiratory tract secretions(SLPI)were protective factors for 28 day mortality in severe pneumonia (P < 0.05). ROC curve analysis showed that the area under the curve(AUC)of serum and lower respiratory secretion SLPI after treatment were 0.953 and 0.968. Patients with serum SLPI < 87.59 ng/mL and lower respiratory secretion SLPI < 835.68 ng/mL had poor prognosis(P < 0.05);The combined prediction efficiency of the two indicators is higher than that of individual indicators,with an AUC of 0.991(P < 0.05). Conclusion SLPI is a protective factor in severe pneumonia,serum and lower respiratory tract secretions SLPI may serve as effective indicators for the treat⁃ ment and predictive evaluation of severe pneumonia.

Objective To compare the clinical outcomes of direct anterior approach(DAA)in lateral position and modified anterolateral approach in supine position (MAASP)in unilateral total hip replacement. Methods 76 patients who were to undergo unilateral total hip replacement were enrolled,and they were randomly divided into group A and group B(38 cases each). Total hip replacement with DAA in lateral position was performed in group A,while total hip replacement with MAASP was performed in group B. The operative indexes(operation time,intraoperative hemorrhage,incision length,first time out of bed activity time,postoperative hospital stay time),postoperative pain levels at 1,3 d and 7 d after operation,acetabular anteversion and lower limb length difference after operation,hip function scores at 3,6 months and 1 year after operation and complications with in 1 year were compared between the two groups. Results The operation time,incision length,the first time of getting out of bed after operation and the time of hospitalization in group B were shorter than those in group A(P < 0.05), and the amount of intraoperative bleeding was less than that in group A(P < 0.05). The pain scores at 3 d and 7 d after operation were lower than those at 1 d after operation in both groups(P < 0.05),and the pain scores at 7 d after surgery were lower than those at 3 d after operation(P < 0.05),and the pain scores at 1,3 d and 7 d after operation in group B were lower than those in group A(P < 0.05). There were no statistical differences in hip joint function scores at 3 months,6 months and 1 year after operation and complications within 1 year between the two groups (P > 0.05). Conclusion DAA in lateral position and MAASP can both increase hip joint function recovery,but the latter is less traumatic,recovers faster,and has less postoperative pain.

Objective To compare the clinical effect and safety of laparoscopic Lich⁃Gregoir and laparo⁃ scopic Cohen techniques in the treatment of children′s uretero⁃bladder junction malformation. Methods The clinical data of 70 children with ureterovesical conjunctival malformation admitted to the pediatric surgery department of our hospital from February 2018 to September 2022 were retrospectively reviewed and analyzed. The patients treated by laparoscopic Lich⁃Gregoir approach were assigned to group A(n = 35)and those treated by laparoscopic Cohen approach to group B(n = 35). The two groups were compared in terms of surgical indexes,therapeutic effects, complications,oxidative stress including levels of superoxide dismutase(SOD),malondialdehyde(MDA),cortisol (Cor)and epinephrine(E),and inflammatory factors including tumor necrosis factor⁃α(TNF⁃α),interleukin⁃8 (IL ⁃8)and IL ⁃10.  Results The average hospital stay in group B was significantly shorter than that in group A (P < 0.05). The reexamination results of urinary cystourethrography(VCUG)3 months after operation in both groups indicated that hydronephrosis was relieved and no reflux was observed in all the children. There was no significant difference in the incidence of postoperative complications between the two groups(2.86% vs. 5.72%, P > 0.05). After surgery,the serum SOD activity was significantly decreased,and the levels of MDA,Cor,E,TNF⁃α, IL⁃8 and IL⁃10 were significantly increased in both groups(P < 0.05),but there was no significant difference between the two groups(P > 0.05). Conclusion Laparoscopic Lich ⁃Regoir technique and laparoscopic Cohen technique have the same efficacy and safety in the treatment of children′s uretero⁃bladder junction malformation, and have a similar impact on the stress and inflammation in them. Surgeons should optimize their surgical approaches according to the conditions of diseased children and their own proficiency in the surgical operation.

Objective To evaluate the clinical application of the terminal branches portal vein emboliza⁃ tion and multimode imaging technology in precise hepatectomy for patients with huge hepatocellular carcinoma. Methods Clinical data of 52 patients of huge hepatocellular carcinoma underwent precise hepatectomy with the help of multimode imaging technology in Yuebei People′s Hospital between Feb. 2018 and Nov. 2021 were retrospec⁃ tively analyzed. According to the treatment method,the patients were divided into direct surgical resection group (control group,27 cases). And terminal branches portal vein embolization followed by surgical resection group (study group,25 cases). The multimode imaging technology fusing three⁃dimensional reconstruction which convert⁃ ed from CT imaging,three⁃dimensional visualization,three dimensional printing and augmented reality in assess⁃ ing the hepatic anatomy,instructing preoperative planning and navigating operation. The terminal branches portal vein embolization technology blocked the interconnected portal vein of the two sides of the liver,and promoted the future liver remnant before surgery. Perioperative results and postoperative efficacy of the two groups such as the surgical methods,operative time,intraoperative blood loss,and postoperative complications were observed and analyzed. Results The multimode imaging technology were successfully applied in all 52 patients,achieved satis⁃ fying preoperative assessment,surgical planning,and intraoperative navigation. The future liver remnant of patients in the study group were promoted effectively,(93 ~ 322 mL,media183 mL). All patients underwent surgery suc⁃ cessfully,there were none perioperative death,no serious postoperative complications such as bleeding,biliary leakage and liver failure occurred. The hepatic blood flow occlusion time,intraoperative blood loss,postoperative length of stay and the incidence of postoperative complications of the study group were(27.6 ± 11.6)min,236 (100)mL,(10.8 ± 2.4)d,12.0%(3/25),respectively. Significantly less than which in the control group(34.4 ± 10.9 min,292(150)mL,(13.1 ± 4.1)d,37.0%(10/27)(P < 0.05). The operative time were(209.6 ± 36.5)min and(237.0 ± 69.1)min in the control group respectively,there was no significant difference(P > 0.05). There were 5 and 11 cases of recurrence respectively in two groups,but no statistical difference between two groups (χ2 = 0.735,P > 0.05). Conclusion The terminal branches portal vein embolization and multimode imaging tech⁃ nology can improve the safety and effect of precise hepatectomy for patients with huge hepatocellular carcinoma through augmented the future liver remnant,accurate preoperative planning and surgical navigation.

Objective To investigate the effects of indobufen combined with clopidogrel on vascular endo⁃ thelial function,inflammatory factors,and oxidative stress in patients with acute coronary syndrome. Methods This study retrospectively selected 156 patients as the research object who had acute coronary syndrome and underwent PCI in our hospital from June 2020 to March 2022. They were divided into two groups by the number table method, with78 patients in each group. The observation group was treated with indobufen combined with clopidogrel,and the control group was treated with clopidogrel. The efficacy and indicators of patients were compared,and the adverse reactions were recorded. Result The therapeutic effect of the observation group was better than that of the control group(P < 0.05);After treatment,the endothelial function indicators of both groups of patients were signif⁃ icantly improved compared to before treatment(P < 0.05),and the VEGF level in the observation group was higher than that in the control group(P < 0.05),while ET⁃1 was lower than that in the control group(P < 0.05);After treatment,the inflammatory factors in patients decreased compared to those after treatment(P < 0.05),and the observation group showed hs CRP and TNF⁃α levels. These levels were significantly lower than those of the control group(P < 0.05);After treatment,the oxidative stress indicators of patients improved compared to before treat⁃ ment(P < 0.05),and the NO and tNOS indicators in the observation group were significantly higher than those in the control group(P < 0.05),while the malondialdehyde levels were lower than those in the control group(P < 0.05); The incidence of cardiovascular adverse events and bleeding in the observation group after treatment was lower than that in the control group(P < 0.05);There was no statistically significant difference in the incidence of adverse reactions between the two groups(P > 0.05). Conclusion The application of indobufen combined with clopidogrel in the treatment of acute coronary syndrome patients has a definite therapeutic effect that can effectively improve various indicators,reduce the incidence of adverse events,and alleviate inflammation and therefore lead to a low bleeding rate,and good safety,which is worthy of further promotion.

Objective To investigate the clinical effect of iRobot Ⅱ of orthopedic robotic system in per⁃ cutaneous endoscopic nucleus pulposectomy(PTED). Methods From June 2020 to August 2022,32 patients (improved navigation group)who completed the modified two⁃dimensional navigation⁃assisted PTED by iRobot Ⅱ of orthopedic robotic system in our hospital and 35 patients(C⁃arm fluoroscopy group)who completed PTED under the guidance of C ⁃arm in the same period were selected. The number of X ⁃ ray fluoroscopy,radiation dose,insertion time of working channel,total operation time,VAS score,ODI score,technical learning curve,and surgical complications of the two groups were statistically analyzed. Results Less X⁃ray fluoroscopy times,lower radiation dose,reduced working channel insertion time and shorter total operation time were observed in improved navigation group(P < 0.05). There was no significant difference in VAS score and ODI score between the two groups before and after operation(P > 0.05),but the comparison of VAS score and ODI score between the two groups at different time points after operation and intra⁃group comparison before operation showed statistical significance(P < 0.05). There was no significant difference between the two groups in the formation of intervertebral foramen,the length of hospital stay and postoperative complications(P > 0.05). The learning curve of the improved navigation group was lower and flater than that of the C⁃arm fluoroscopy group,and the operation time was constant,which was signifi⁃ cantly shortened with the increase of the number of operations(P < 0.05). There was no significant difference in the incidence of complications between the two groups(P < 0.05). There was no statistical difference in the excellent and good rate of the modified MacNab score between the two groups in the last follow⁃up(P > 0.05). Conclusion There are shortened operation time,reduced radiation and operation risk during the operation and shortened learn⁃ing curve of PTED with the application of iRobot Ⅱ of orthopedic robotic system in PTED.

Pancreatic cancer(PC),as one of the digestive tract tumors with the lowest survival rate and the worst prognosis,is known as the“king of cancers”. Most patients have lost the opportunity for radical resection at diagnosis and have developed resistance to gemcitabine(GEM),the first ⁃line chemotherapy agent,further reducing the effectiveness of treatment. In recent years,ferroptosis a new type of iron⁃dependent cell programmed death,and its regulatory mechanism has been widely studied and applied in the treatment of pancreatic cancer. Ex⁃ perimental studies have shown that inducing ferroptosis in pancreatic cancer cells can effectively reduce their resis⁃ tance to chemotherapy drugs. In this paper,the role of ferroptosis in gemcitabine resistance of pancreatic cancer were reviewed,aiming to provide new ideas and directions for the study of the treatment of pancreatic cancer based on ferroptosis.

Ovarian cancer is a common gynecological malignancy. Most patients with the cancer are already at an advanced stage when visiting for medical consultations and are prone to recurrence and chemoresistance,so ovarian cancer patients have a high mortality rate and poor prognosis. N6 methyladenosine is a common epigenetic modification that dynamically and reversibly regulates the entire process of RNA metabolism. Recent studies have shown that m6A modification is aberrantly expressed in ovarian cancer patients,which is closely related to tumori⁃ genesis,metastasis and chemoresistance. Therefore,m6A can be used as a potential tumor marker for the early diagnosis or assessment of prognosis of ovarian cancer. The recent research of m6A also provides new ideas for the treatment of ovarian cancer. In this paper,we reviewed the role of m6A modification in ovarian cancer.

Diabetes(DM)is one of the most common metabolic diseases in the world. The etiology of dia⁃ betes is complex. Its etiology is complex,and more and more evidence shows that diabetes is regulated by intestinal flora and microbial metabolites,and the imbalance of flora is one of the risk factors for diabetes. As a new treat⁃ ment method,fecal microbiota transplantation(FMT)can regulate the flora. In this review,we focus on the appli⁃ cation of FMT in diabetic population and diabetic animal models,aiming to evaluate the efficacy and safety of FMT targeted therapy for diabetes,and consider FMT as a potential therapy for DM.

Multi⁃disciplinary collaboration(MDT)has now become an international trend in the diagnosis and treatment of obesity for its various causes and complications and complicated mechanism of its occurrence,but at present,without comprehensive understanding of the model and the necessity of its implementation,the MDT model in the diagnosis and treatment of obesity is still in the stage of exploration. No mature operating model has been formed,which,to a great extent,restricts the development and its application. In order to deepen the overall understanding of MDT model in the diagnosis and treatment of obesity,and to further understand the necessity of its implementation,this article reviews the development,necessity of implementation,team structure,operation model,etc. of the multi⁃disciplinary collaborative diagnosis and treatment model of obesity,hoping to providing ref⁃ erence for the diagnosis and treatment of obesity and the promotion of MDT model in the diagnosis and treatment of the obesity in our country.
