Breast cancer is a common disease,which is harmful for females,and surgery dominated its treatment. However ,after surgical treatment ,the incidence of lymphedema in upper limbs is high due to the injury of axillary lymphatic vessels,which affects the patients′ quality of life and increases their pain. Clinicians especially breast surgeons should attach more importance to it. In this paper,breast cancer⁃related lymphedema is discussed,especially the prevention and treatment of lymphedema. With the development of breast cancer research and the popularization of minimally invasive surgery,the incidence of breast cancer ⁃ related lymphedema will be gradually reduced,which is of positive significance for alleviating the pain of patients and improving the quality of life.

Objective Triple negative breast cancer(TNBC)is an aggressive cancer without effective therapeutic targets. Long non⁃coding RNA(lncRNA)has been shown to play an important role in the development of TNBC. This study is intended to investigate the effects of lncRNA SPINT1⁃AS1 on the proliferation and apoptosis of TNBC cells and find a potential therapeutic target for TNBC. Methods The relative expression of SPINT1⁃AS1 in normal breast epithelial cell lines and TNBC cell lines was detected by real⁃time quantitative PCR(qRT⁃PCR). The cell localization of SPINT1⁃AS1 was detected by RNA fluorescence in situ hybridization. Overexpressed and down⁃regulated SPINT1⁃AS1 of TNBC cell lines were constructed respectively,and the proliferation and apoptosis of TNBC were explored by clone formation assay,CCK⁃8 assay and flow cytometry. Results SPINT1⁃AS1 was expressed mainly in the cytoplasm. The expression of SPINT1⁃AS1 in the TNBC cells was significantly lower than that in normal breast epithelial cells(P < 0.05). SPINT1⁃AS1 downregulated the proliferation of TNBC cells in a significant way(P < 0.05),and promoted their apoptosis(P < 0.05). Conclusion SPINT1⁃AS1 is expressed at a lower level in the cytoplasm of TNBC cells. It can suppress the proliferation of TNBC cells and induce their apopto⁃ sis,which indicates that it can be a potential therapeutic target for treating TNBC.

Objective Molecular staging of breast cancer is an important guide for clinical diagnosis and prognosis assessment. Our study sought to non ⁃invasively predict the biological characteristics of breast cancer by examining the relationship between ultrasound features and molecular subtypes. Method 213 female patients who had been surgically or pathologically diagnosed with breast cancer underwent a retrospective analysis. The molecu⁃ lar subtypes were classified into five types:Luminal A,Luminal B HER⁃2(⁃),Luminal B HER⁃2(+),HER⁃2 overexpression and triple ⁃ negative. Different molecular subtypes′ clinical pathological features and conventional ultrasound characteristics were analyzed and summarized. Results Comparison of size,lymph node metastasis and histologic grading of breast cancers with different molecular staging,as well as comparison of morphology,mar⁃ gins,posterior echogenicity and presence of microcalcifications,and blood flow grading in ultrasound features. The differences were statistically significant(P < 0.05);LA nodes were predominantly ≤ 20 mm,with lower incidence of lymph node metastasis and lower histologic grade;LB HER⁃2(+)and HER⁃2 overexpressing nodes were more often associated with microcalcifications,and HER⁃2 overexpressing nodes had higher blood flow grade. Sharp mar⁃ gins,regular shape,and enhanced posterior echo were more prevalent in breast cancer lesions of the triple⁃nega⁃ tive type. Conclusion Each molecular stage of breast cancer has its own ultrasound performance characteristics. Preoperative ultrasound can provide some reference basis for breast cancer diagnosis,treatment,and prognosis assessment.

Objective Investigate the value of blood cell parameters in breast cancer diagnosis by OPLS⁃DA and artificial neural network algorithm. Methods Blood cell parameters from breast cancer patients and healthy females were collected. The OPLS ⁃DA algorithm was used to calculate the significant differences between breast cancer patients and normal females. ROC was used to assess the diagnostic efficacy of the prominent parameters including sensitivity and specificity. A prediction model for breast cancer diagnosis is established by neural network algorithm based on the prominent different parameters. Results According to the OPLS⁃DA analysis,there are significant differences in blood cell parameters between breast cancer patients and healthy women. MPV,BA# PLT,MCV,RBC and LY# were the main differential parameters between the two groups. The specificity of these parameters for breast cancer diagnosis is 0.564,0.983,0.622,0.674,0.878 and 0.762 respectively;the sensitivity is 0.819,0.614,0.618,0.561,0.393 and 0.393 respectively;and the AUC is 0.773,0.793,0.657,0.649,0.643 and 0.635 respectively. Based on MPV,BA#,PLT,MCV,RBC and LY#,the artificial neural network model was estab⁃ lished by 16 862 iterations,and the sensitivity and specificity of breast cancer prediction were 0.941 2 and 0.795 5 respectively. Conclusion The OPLS⁃DA algorithm is used in this study to investigate the main differential parame⁃ ters of blood cell parameters between breast cancer patients and healthy women. Breast cancer prediction model based on artificial neural network has been successfully established,which is of certain value for breast cancer screening.

Objective To explore conventional ultrasound combined with contrast ⁃ enhanced ultrasound (CEUS)in predicting axillary lymph node metastasis of breast cancer. Methods Conventional ultrasound and CEUS features of breast cancer tumors(n = 119)were retrospectively collected. According to the postoperative pathological results,the patients were divided into the group with and without axillary lymph node metastasis(ALNM). And the methods of univariate and multivariate Logistic regression analysis were used to screen out the risk factors associated with ALNM in breast cancer. Logistic regression prediction model was established ,and the receiver operating characteristic(ROC)curve was drawn. Results Univariate analysis showed that there were significant differences between the two groups in maximum diameter of breast cancer,alder blood flow grade,edge burr sign radial convergence of enhanced edge and range change after enhancement(P < 0.05). Multivariate analysis showed that the maximum diameter of breast cancer > 2 cm,enhancement of perfusion defect and enhancement of the larger rangewere independent risk factors for ALNM(P < 0.05). The AUC of these three ultrasound features predicting the ALNM were 0.699,0.616 and 0.664 respectively. The prediction model of the above indicators combined with AUC was 0.791,whichwas greater than that of breast cancer > 2 cm,enhancement of perfusion defect and enhance⁃ ment of the larger range alone(P < 0.05). The prediction model had a better prediction efficacy through the consis⁃ tency test(Kappa value = 0.607). Conclusion The maximum diameter of breast cancer > 2 cm,enhancement of perfusion defect and enhancement of the larger range were independent risk factors for ALNM in breast cancer,andthe value of combined use of them had a higher predictive value for ALNM. 

Predict the Luminal type of invasive breast cancer using machine learning models based on ultrasono⁃ graphic features

Objective To build the machine learning models based on ultrasonographic features to predict the Luminal type of invasive breast cancer,and to screen out the important ultrasonographic characteristics that influence the Luminal type. Methods The ultrasonographic and immunohistochemical features of 529 patients with postoperative pathology⁃diagnosed invasive breast cancer were studied retrospectively. The patients were divided into two groups based on immunohistochemical features:luminal and non⁃luminal. Using SPSS Modeler 18.0 statis⁃ tical software,patients were randomly divided into training cohort and validation cohort,and five classifiers were used to build the models,including Logistic regression analysis,support vector machine (SVM),Bayesian network,random forest,and decision tree. The ROC curves were drawn according to the original propensity score of the models. The AUCs were used to evaluate the predictive ability of different models ,and DeLong test was used to compare the AUCs of the five models. The best model was used to screen out the most important variables for predicting the Luminal type. Results Logistic regression analysis and SVM outperformed other models in both the training and validation cohorts,and the difference was statistically significant(P < 0.05). The three most important variables for predicting the Luminal type,according to the SVM model,were diameter,margin,and calcification. Conclusion Machine learning models based on ultrasonographic features can accurately predict the Luminal type of invasive breast cancer,and SVM outperforms Logistic regression analysis in terms of predictive ability. The three most important ultrasonographic features for predicting Luminal type are diameter,margin,and calcification.

Infection of Helicobacter pyloriH.pylori)has become one of the intractable problems in the field of public health. It was identified asan association with many gastrointestinal diseases. Specially,its associa⁃ tion with gastric cancer that has been confirmed by substantial evidence.At present ,the studies that attend to explore the pathogenic mechanism of Hp mainly focus on the protein of virulence factor. As a newly identified virulence factor,the blood group antigen ⁃binding adhesion(BabA)has obtained a great attention on its roles for Hp to induce the occurrence and development of gastric cancer. This paper will comprehensively review the structure and function of BabA,its expression regulation,gastric cancer⁃promoting roles and clinical application,aiming to provide direction for developing new strategies in preventing and treating gastric cancer.

Objective To investigate the effect of Buyiganshen decoction on a mouse model of osteoarthritis and its mechanism. Methods A mouse model of knee osteoarthritis(KOA)was established. The study groups were designed as follows:(1)blank control group;(2)osteoarthritis model group;(3)model group with high⁃dose Buyiganshen decoction;(4)model group with administration of high⁃doseBuyiganshen decoction plus TGF⁃β Inhib⁃ itor sb⁃431542. After the administration,the mice in each group were killed,and the articular cartilage tissues were obtained for HE staining. Expressions of TGF⁃β1,ALK5,Smad2,Smad3,Sox9,P70S6K,PI3K,Akt and mTOR in these tissues were detected by immunohistochemistry. Peripheral blood expressions of TNF⁃ α,IL⁃6 and IL⁃8 were measured by ELISA. Results As compared with those in the control group,expression levels of TGF ⁃β 1,ALK5,Smad2,Smad3,Sox9,P70S6K,PI3K,Akt and mTOR were decreased in the model group. In compari⁃ son with those in the model group,expressions of the above genes were significantly up⁃regulated in the group with Buyiganshendecoction. As compared with those in the control group,expressions of TNF⁃α,IL⁃6 and IL⁃8 were significantly up⁃regulated;while as compared with those in the model group,expressions of TNF⁃α,IL⁃6 and IL⁃8 were significantly down⁃regulated. Additional TGF⁃β1 inhibitor sb⁃431542 could reverse the effect of Buyiganshen⁃ decoction. Conclusions Chinese herbal prescription of Buyibanshenplays a certain role in the treatment of osteoar⁃ thritisby activating the TGF⁃β1⁃sox9/p70S6 extracellular matrix signaling pathway.

Objective To investigate the expression of microRNA⁃9⁃5p(miR⁃9⁃5p)in the hippocampus and plasma of a mouse model of Alzheimer′s disease(AD),and the effect of its overexpression on Tau pathology. Methods Male APPswe/PS1dE9(APP/PS1)transgenic mice and WT C57BL/6(WT)mice were selected as research subjects,and then they were randomly divided into 3 groups:Lv⁃WT group,Lv⁃con group and Lv⁃miR⁃ 9⁃5p group,with 18 mice in each group. Lv⁃miR⁃9⁃5p(4 × 105 TU)or Lv⁃con(4 × 105 TU)was slowly injected into bilateral hippocampus of the mice in the Lv⁃miR⁃9⁃5p group and Lv⁃con group. The behavior tests or electro⁃ physiology experiments were performed 30 days after the injection. miRNA expression and AT8 protein expression were analyzed by microarray and quantitative real⁃time PCR and immunoblotting and immunofluorescence,respec⁃ tively. Results Compared with the Lv⁃con group,the discrimination index,contextual fear cessation,cue condi⁃ tioned fear cessation,cross⁃platform times and target quadrant time were significantly increased(P < 0.05),and cross ⁃ platform time was significantly decreased in the Lv ⁃miR ⁃ 9⁃5p group(P < 0.05). The density of synaptic spines and the percentage of mushroom spines in the hippocampus of mice in the Lv⁃miR⁃9⁃5p group were signifi⁃ cantly higher than those in the Lv⁃con group(P < 0.05),and the mice in the Lv⁃miR⁃9⁃5p group showed higher long⁃term enhancement. Compared with the Lv⁃WT group,the level of AT8 and the co⁃localization amount of AT8 and γH2AX in the hippocampus of mice in the Lv⁃con group were significantly increased(P < 0.05). The amount of AT8 in the Lv⁃miR⁃9⁃5p group was significantly reduced than that in the Lv⁃con group(P < 0.05). Conclusion miR⁃9⁃5p plays a beneficial role in AD pathogenesis by reducing aberrant Tau phosphorylation and DNA damage.

Objective To explore whether miRNA⁃216a(mir⁃216a)inhibits the metastasis and epithelial⁃ mesenchymal transition(EMT)of ovarian cancer(OC)cells by regulating the Janus kinase 2(JAK2)/signal trans⁃ ducer and activator of transcription 3(STAT3)pathway. Methods Detection of miR ⁃ 216a expression levels in OC tissues and cells by qRT⁃PCR. Transwell were used to detect the ability of migration and invasion. Western blot and immunofluorescence were used to detect the expression levels of EMT⁃related proteins. The expression levels of JAK2/STAT3 pathway related proteins were analyzed by Western blot. Results The expression of miR⁃216a was down⁃regulated in OC tissues and cells(P < 0.05). Up⁃regulated miR⁃216a impaired OC cell migration,invasion and EMT and simultaneously inhibited JAK2/STAT3 pathway activation(P < 0.05). Overexpressed JAK2 reversed the effects of up⁃regulated miR⁃216a on the above indicators(P < 0.05). Conclusion miR⁃216a may inhibit the metastasis and EMT of OC cells by suppressing JAK2/STAT3 pathway activation. 

Mechanism of CSE ⁃induced up ⁃regulation of MUC5AC in 16HBE cells by transcriptomics

Objective To explore the signaling pathway and molecular mechanism involved in up⁃regulation of MUC5AC in 16HBE cells induced by CSE using the method of transcriptome sequencing,and to provide theoreti⁃ cal basis for finding new targets of COPD. Methods Differently expressed genes(DEGs)were detected by tran⁃ scriptome sequencing in 16HBE cells induced by CSE. Excavating out which potential signal pathway involved in up⁃ regulation of MUC5AC in 16HBE cells induced by CSE was excavated through bioinformatics analysis. The transcript levels of relevant genes in the screened signaling pathways were verified by RT ⁃ qPCR,and whether the signaling pathway and related genes were involved in up⁃regulation of MUC5AC in 16HBE cells induced by CSE was verified using the method of signaling pathway agonists and siRNA interference. Results A total of 2 567 DEGs were detected by transcriptome sequencing,and the bioinformatics analysis showed that MAPK signaling,WNT signaling and PI3K⁃Akt in 16HBE cells were abnormally expressed after CSE stimulation. RT⁃qPCR showed that the tran⁃ script levels of WNT4,WNT5B,WNT6 and WNT10A genes were down⁃regulated when compared with control group (0.885 times,0.893 times,0.705 times,0.618 times,P < 0.05),and WNT9A were up ⁃ regulated(3.727 times P < 0.05),which were consistent with transcriptome sequencing. After the activation of the classical WNT signaling pathway,the MUC5AC transcript level of 16HBE cells was up⁃regulated by 3.197 times when compared with the control group(P = 0.031 3). However,in the small molecule interference experiment,the transcript level of MUC5AC in WNT5B siRNA + CSE group was 2.092 times higher than that in negative control + CSE group(P = 0.016 2). Conclusion WNT5B in the non ⁃classical WNT signaling pathway is down ⁃ regulated and participates in CSE induced up⁃regulation of MUC5AC in 16HBE cells.

Efficacy of a new minimally invasive lumbar fusion device for lumbar spondylolysis

Objective To investigate the safety and feasibility of a new lumbar fusion technique in the treat⁃ ment of lumbar spondylolysis. Methods Twenty patients with lumbar spondylolysis diagnosed in the First Affiliated Hospital of Guangxi University of Chinese Medicine from January 2021 to August 2021 were prospectively selected and randomly divided into group A(new type of lumbar fusion device group)and group B(PLIF group),with 10 patients in each group. Age,sex,body mass index(BMI),operative segment,operative time,intraoperative blood loss,ambulation time,wound length,intraoperative X ⁃ ray fluoroscopy times,postoperative complications,VAS score,JOA score and ODI score were recorded and compared in the two groups. Results There was no significant difference in terms of age,sex,BMI,surgical segment,preoperative VAS score,preoperative JOA score and preop⁃ erative ODI score between group A and B(P > 0.05). Intraoperative blood loss,ambulation time,wound length intraoperative X ⁃ ray fluoroscopy frequency and operation time in group A were significantly different from those in group B(P < 0.05). There was statistically significant difference in VAS score of postoperative pain on the 1st and 7th day between group A and B(P < 0.05). There was significant difference in JOA score and ODI score between group A and B on the 1st,and 14th day and 1 month after operation(P < 0.05). Conclusion In the treatment of lumbar spondylolysis,the new type of lumbar fusion device has the advantages of less intraoperative blood loss small wound and rapid recovery. At the same time,the symptoms are relieved obviously and the curative effect is sat⁃ isfactory. However,intraoperative X⁃ray fluoroscopy is more frequently needed when compared with PLIF technique. 

Clinical and pathological characteristics of idiopathic non⁃cirrhotic portal hypertension:A 115⁃case study

Objective To summarize and analyze the clinical and pathological characteristics of patients with idiopathic non⁃cirrhotic portal hypertension(INCPH),and to provide basis for clinicians to identify the disease in the early stage. Methods The data of 115 patients with INCPH diagnosed in China⁃Japan Friendship Hospital from January 2016 to December 2021 were analyzed retrospectively,including demographic characteris⁃ tics,clinical manifestations,and laboratory,imaging and pathological manifestations. Results Among 115 patients with INCPH,there were 51 males(44.35%)and 64 females(55.65%),with a male⁃female ratio of about 1∶1.25. Patients aged from 12 to 79 years,with an average age of(47.89 ± 21.14)years. The majority Child⁃Pugh score of liver function was A(88.89%). The onset clinical symptoms were splenomegaly(34/38,89.47%),hemocytopenia (38/45,84.44%)and upper gastrointestinal bleeding(25/41,60.97%). The median values of WBC,NEUT,PLT and protein C activities were lower than the lower limit of normal,and the abnormal rate was more than 50%. The median values of ALT,AST,GGT,ALP,ALB,TBIL,CHE,TG and TC were all in the normal range,with low abnormal rate. Gastroscope found that the detection rate of esophageal and gastric fundus varices was as high as 77.42%;the detection rates of splenomegaly and portal hypertension in imaging were higher than 50%,and TE ⁃ LSM ≥ 7.3 kpa accounted for 19.0%. In addition to the main pathological changes of the liver,CD34(+)(25/27 92.59%)of hepatic sinuses,dilatation of hepatic sinuses(33/91,36.26%),and irregular fibrous septum(23/91 25.27%)were observed. No cirrhosis was found in all specimens. Conclusion INCPH patients has severe portal hypertension,mild liver function injury and good liver reserve function. The diagnosis of this disease requires closecombination of clinical and pathological findings.

Objective To investigate the efficacy of MAP washed red blood cells(MAP WBC)and normal saline mixed with red blood cells(WBC)on direct antiglobulin test(DAT)of blood transfusion therapy in DAT⁃ positive patients. Methods From January 2020 to December 2021,207 DAT ⁃ positive patients in our hospital were selected and divided into MAP group(n = 93)and saline group(n = 114). MAP group was infused with MAP washed red blood cells for treatment,while saline group was treated by infusion of saline and washed red blood cells mixture. The blood transfusion efficacy as well as side effects of the two groups was observed via detecting the laboratory indexes of hemoglobin(HGB)and total bilirubin(TBIL). Results Comparing the changes of each index before and after blood transfusion in the two groups,inner⁃group HGB and TBIL significantly increased,and the difference was statistically significant(P < 0.05),while there was no obvious change in HGB and TBIL between the two groups and the difference was not statistically significant(P > 0.05). There were no adverse reactions of blood transfusion in both groups. Conclusion Both types of washed red blood cells are safe and effective for DAT⁃ positive patients,but MAP washed red blood cells can better meet the needs of clinical emergency with the capabil⁃ ity to be stored for longer period.

Objective To observe the effect of nalbuphine combined with hydromorphone at different com⁃ patible dosages postoperative analgesia in gynecological laparoscopic patients. Methods The patients hospitalized in the hospital for selective laparoscopy were randomly divided into five groups according to the compatible doses nalbuphine 0.75 mg/mL + hydromorphone 25 μg/mL(Group A),nalbuphine 0.67 mg/mL + hydromorphone 33 μg/mL (Group B),nalbuphine 0.50 mg/mL + hydromorphone 50 μg/mL(Group C),nalbuphine 0.33 mg/mL + hydromor⁃ phone 67 μg/mL(Group D),nalbuphine 0.25 mg/mL + hydromorphone 75 μg/mL(Group E). VAS Pain Scores were recorded at PACU and at 4 h,8 h,12 h,24 h after operation. The total pressing times and effective pressing times of PCIA,adverse reactions and analgesic satisfaction were recorded. Results The VAS scores during activity at 8 h,12 h,24 h after operation,total pressing times and effective pressing times of PCIA in Group A were signif⁃ icantly higher than those of other groups(P < 0.05). The incidence of nausea and vomiting in groups D and group E was significantly higher than that in the other three groups(P < 0.05). The analgesic satisfaction was higher in Group B and Group C than that in the other three groups(P < 0.05). Conclusion Nalbuphine 0.67 mg/mL combined with hydromorphone 33 μg/mL and nalbuphine 0.50 mg/mL combined with hydromorphone 50 μg/mL for PCIA can obtain satisfactory analgesic effect after gynecological laparoscopic operation. The incidence of adverse reactions is the lowest and the patient′s analgesia satisfaction is the highest and therefore they are worthy of clinical application.

Objective To investigate the prognostic value of Neutrophil⁃lymphocyte ratio(NLR)and its dynamic changes in the treatment of hepatitis B associated acute⁃on⁃chronic liver failure(HBV⁃ACLF)with artifi⁃ cial liver. Methods Patients were selected from the Department of Gastroenterology,Western Theater Command General Hospital from January 2018 to December 2021,and received artificial liver support System. Clinical data and relevant laboratory data were retrospectively collected. Patients were divided into survival group and death group according to 90⁃ day disease outcome. Differences between groups were compared to analyze the evaluation value of NLR and its dynamic change level on patients' prognosis. Results A total of 106 patients,male 88 cases 83.02%)and female 18 cases(16.98%),with a median age of 48 years(41.75,56),were enrolled in this study. Binary logistic regression showed that NLR was an independent risk factor for death of HBV ⁃ACLF patients after ALSS(P < 0.05),the level of NLR in survival group[3.94(2.90 ~ 5.47)]was significantly lower than that in death group[5.35(4.12 ~ 7.59),P < 0.05]Baseline NLR level was positively correlated with MELD score,MELD⁃Na score,and iMELD score(P < 0.05)The area under ROC curve to determine the 90⁃day mortality rate of HBV ⁃ ACLF patients by NLR level 1 day before and 3 days after the baseline was 0.72,0.712,0.72,respectively. Survival analysis showed that the 90⁃day survival rate in the low NLR group was significantly higher than that in the high NLR group(P < 0.05). NLR change rate(ΔNLR)was significantly different between survival and death groups. Conclusion Serum NLR level can be used as a potential prognostic indicator for patients with acute⁃on⁃chronic liver failure treated with artificial liver therapy,and dynamic monitoring has higher clinical value.

Objective To study the clinical effects of personalized interventions on patients with type 2 diabetes mellitus with holistic integrative medicine(HIM). Methods Thirty middle ⁃ aged and elderly patients with type 2 diabetes from communities in Guangzhou who volunteered to participate in the project were randomly selected. A multidisciplinary approach of health education and personalized intervention was used. The changes in patients' blood glucose biochemical indexes,blood pressure,anxiety,depression,physiological feelings and blood pressure levels before and after the intervention were compared. Results After 4~12 months of intervention,all glucose⁃lowering drugs were stopped in 13 patients;partially stopped in 15 patients,and 2 patients remained the same medication. The mean levels of biochemical indicators including fasting glucose(9.2 mmol/L and 6.9 mmol/L respectively),2⁃hour postprandial glucose(12.3 mmol/L and 7.7 mmol/L,respectively)and glycated hemoglobin (7.7% and 6.4%,respectively)before and after the intervention were significantly decreased(P < 0.05). After the intervention,36.7% of the patients reached the remission level of diabetes,20% reached the normal level of fasting glucose,2⁃hour postprandial glucose and glycated hemoglobin after stopping the hypoglycemic drugs for 3 months,and the glucose metabolism of the patients who did not reach the remission level also improved signif⁃ icantly(P < 0.05). Seventy⁃five percent(9/12)of those with combined high blood pressure had normal blood pres⁃ sure after 3 months of discontinuation of antihypertensive drugs. Significant improvements in anxiety,depression and physiological perception indicators were also found(P < 0.05). Conclusion Personalized intervention in the mode of HIM can significantly improve glucose metabolism,psychological status,physiological function and high blood pressure in middle⁃aged and elderly patients with type 2 diabetes. Remission or even reversal of diabetes can be expected within a short period of time in mild or moderate patients and significant improvement in glucose metabolism in severe patients is also found. It provide ideas for further optimization of diabetes prevention and control programs.

Objective To investigate the consistency of S⁃Thyroid and sonographer in the evaluating char⁃ acteristics of thyroid nodules according to American College of Radiology Thyroid Imaging Report and Data System (ACR TI⁃RADS)and compare their diagnostic efficiency. Methods A total of 253 patients with 276 thyroid nod⁃ ules were enrolled in the study. The ultrasonic characteristics of thyroid nodules were evaluated by S⁃Thyroid and sonographers respectively. Cohen's kappa test was used to evaluate the consistency between S⁃Thyroid and sonogra⁃ phers in the diagnosis. Taking surgical pathology as gold standard,ROC curves were drawn and diagnostic efficacy was analyzed. Results In terms of ultrasonic characteristics,there were high consistency in morphology and inter⁃ nal structure(Kappa values were 0.973,0.886,0.805,respectively)and medium consistency in hyperechoic foci and edges(Kappa values were 0.613 and 0.597,respectively). Meanwhile,the sensitivity and specificity of S⁃Thy⁃ roid and sonographers were 82.01% vs. 86.24% and 86.21% vs. 83.91%,respectively. There were no significant dif⁃ ference(P > 0.05). The area under the curve was 0.835(95%CI:0.801 ~ 0.926)and 0.891(95%CI:0.846 ~ 0.936),respectively,and there was no significant difference between them(Z = 0.913,P > 0.05). Conclusion According to the criteria of ACR TI⁃RADS,there are good consistency and high diagnostic efficiency in the evalua⁃ tion of thyroid nodules between S⁃Thyroid and sonographers in evaluating thyroid nodules. Therefore,S⁃Thyroid is worthy of further popularization and application in clinic.

Objective To explore the effect of Huoyuan Recipe combined with quadruple therapy on serum pepsinogen(PG),tumor⁃specific growth factor(TSGF)and gastric cancer monoclonal antibody(MG7⁃Ag levels in patients with Hp⁃positive chronic atrophic gastritis with spleen⁃stomach qi deficiency type. Methods A total of 96 patients with Hp⁃positive chronic atrophic gastritis of spleen⁃stomach⁃qi deficiency type who were admitted to Wuhan hospital of traditional Chinese medicine from October 2020 to September 2021 were selected and divided into the control group and the observation group by random number table method,48 cases in each group. Blind experiments were performed on condition the patients knew nothing about their respective therapy. The control group was treated with quadruple therapy + placebo,and the observation group was treated with quadruple therapy + Huoyuan Recipe. The TCM symptom scores of the two groups were evaluated,and the gastrointestinal function detector was used to detect pepsinogen Ⅰ(PGⅠ),pepsinogen Ⅱ(PGⅡ),and gastrin⁃17(G⁃17)level. Enzyme⁃ linked immunosorbent assay was used to detect tumor necrosis factor⁃α(TNF⁃α),interleukin⁃6(IL⁃6),C⁃reactive protein(CRP),TSGF,MG7⁃Ag levels. The clinicopathological conditions of the patients were assessed by patho⁃ logical scoring. The Hp conversion was recorded,and the therapeutic effects of the two groups were compared. Results After treatment,the scores of stomach pain,bloating,fatigue,TSGF,MG7⁃Ag,TNF⁃α,IL⁃6,CRP levels,chronic inflammation,inflammatory activity,gland atrophy,and intestinal metaplasia scores in the observation group were significantly lower than those in the control group(P < 0.05). After treatment,the levels of PGⅠ,PGⅡ and G⁃17 in the observation group were significantly higher than those in the control group(P < 0.05). The negative rate and effective rate of Hp in the observation group were significantly higher than those in the control group (P < 0.05),and the incidence of adverse reactions in the observation group was lower than that in the control group (P > 0.05). Conclusion Huoyuan Recipe combined with quadruple therapy can effectively treat Hp ⁃ positive chronic atrophic gastritis of spleen⁃stomach⁃qi deficiency type by decreasing the TCM syndrome score,improving the levels of PGⅠ,PGⅡ,TSGF,MG7⁃Ag and G⁃17,alleviating the inflammatory response,and improving the patient′s pathology score,and increasing the Hp eradication rate.

The biological clock can widely regulate the behavior and physiological function of human body.Studies have found that the intestinal tract itself also has biological clock rhythm and is regulated by the central biological clock rhythm. Circadian rhythm disorders widely relates to inflammatory bowel disease,irritable bowel syndrome,colorectal cancer and other intestinal diseases. Otherwise,biological clock rhythm disorders will also have an important impact on gastrointestinal motility,intestinal mucosal barrier permeability and intestinal flora,resulting in a variety of intestinal dysfunction or intestinal diseases. This paper mainly reviews the relationship of intestinal biological clock rhythm with the diseases and its mechanism.

Acute kidney injury is a common complication in hospitalized patients,and it has both short-and long⁃term consequences. To improve patient outcomes,methods for identifying patients at risk for acute kidney injury and diagnosing subclinical acute kidney injury must be developed. Rapid advances in risk estimation in acute kidney injury have been enabled by advances in clinical informatics and the growing popularity of electronic medical records. The development of central nervous system (CNS)is one of the cores of neurobiological research. Noncoding RNAs can regulate CNS development in various ways. Studies have shown that the regulation of lncRNAs and miRNAs is crucial for brain development and cognitive ability,and they not only participate in the proliferation and differentiation of CNS neural stem cells,but also affect multiple processes such as neurite out⁃growth,synaptic stability and myelination. This article reviews the regulatory roles of lncRNA⁃miRNA interaction on the development of the CNS.
