不同程度烧伤对患者七氟烷MACBAR的影响

摘要:

目的 探讨不同程度烧伤对患者七氟烷MACBAR影响。方法 选择2017年6月至2019年5 我院收治的严重烧伤患者,根据烧伤程度不同进行分组,A 30%面积烧伤(n = 20 例)、B 50%面积烧伤 n = 20)、C 70%面积烧伤(n = 20),共 60 例。三组均给予七氟烷麻醉诱导,采用序贯法测定七氟烷 MACBAR、比较术前、术中、术后心率(HR)、平均动脉压(MAP)、血糖(BG)、去甲肾上腺素(NE)、术中脑电双频 指数(BIS)、记录自主呼吸消失时间、苏醒时间、拔管时间及镇静-躁动评分。结果 C MACBAR、HR MAP、BG、NE、均高于 B 组及 A 组,而 B 组则高于 A 组,差异有统计学意义(P<0.05);C 组自主呼吸消失时 间长于 B 组及 A 组,且 B 组长于 A 组,差异有统计学意义(P<0.05);C 组苏醒时间、拔管时间短于 B 组及 A 组,且 B 组短于 A 组,差异有统计学意义(P<0.05);三组术后镇静-躁动评分比较,差异无统计学意义 P>0.05)。结论 随着烧伤程度的增加,七氟烷 MACBAR逐渐增加,但麻醉作用时间则逐渐缩短,在相关手术麻醉中应适当提高吸入浓度并延长药物维持时间。

Abstract:

Objective To investigate the effect of MACBAR of sevoflurane in patients with different classi⁃ fications of burn. Methods Sixty severely burned patients admitted to our hospital from June 2017 to May 2019 were divided into group A with second⁃degree burns(30%,n = 20),group B with third⁃degree burn(50%,n = 20 and group C with fourth⁃degree burn(70%,n = 20). Three groups were given sevoflurane anesthesia induction the MACBAR of sevoflurane was determined by sequential method,and the preoperative,intraoperative,postoper⁃ ative heart rate(HR),mean arterial pressure(MAP),blood glucose,norepinephrine,bispectral index(BIS), disappearance time of spontaneous breathing,awakening time,extubation time and sedation ⁃agitation scale were compared. Results The MACBAR,preoperative,intraoperative,postoperative HR,MAP,blood glucos,and nor⁃ epinephrine in group C were higher than those in group B and A,and higher in group B than in group A(P < 0.05). Disappearance time of spontaneous breathing in group C was longer than that in group B and A,and longer in group B than in group A(P < 0.05). Awakening time and extubation time in group C were shorter than those in group B and A,and shorter in group B than in group A(P < 0.05). There was no difference in postoperative seda⁃ tion⁃ agitation scores among the three groups(P > 0.05). Conclusion With the severity of burn,the MACBAR of sevoflurane was increased but the anaesthetic effect time was shortened. The concentration of inhalation should be appropriately increased and drug maintenance time should be extended in the relevant surgical anesthesia
