步速与住院老年高血压患者全因死亡的相关性

摘要:

目的 探讨步速与住院老年高血压患者全因死亡的相关性。方法 选择 2015 6 月至 2017 12 月在首都医科大学附属复兴医院综合科住院的年龄≥65 岁的高血压患者 515 例,记录患者基本 资料、共病及多重用药情况,采用6 m 步行试验计算步速,并评估患者日常生活能力、认知状况及营养状 态,出院随访 2 年以上并记录患者全因死亡的发生情况,分析步速与全因死亡的相关性。结果 共入选患 515 例,平均年龄(84.1 ± 5.9)岁,其中男 299 例(58.1%),中位随访 3.3(2.7,3.9)年,随访期间共 106 20.6%)发生全因死亡。Spearman 相关分析显示,步速与年龄、Charlson 共病指数、多重用药呈负相关, 24 h 舒张压水平、微营养评定法简表、简易智能评估量表及 Katz 日常生活能力量表评分呈正相关 P < 0.05)。将步速进行三分位数分组,Kaplan⁃Meier 曲线显示随着步速下降,全因死亡风险逐渐升高 log⁃rank:P < 0.001),Cox 回归模型结果显示,在调整混杂因素后,与最高步速组相比,最低步速组全因死 亡风险更高(HR = 2.325,95%CI:1.113 ~ 4.855,P = 0.025)。结论 在住院老年高血压患者中,低步速与出 院后全因死亡风险相关,步速可作为一项简便易行的死亡预测指标。

Abstract:

Objective To investigate the correlation between gait speed and mortality in elderly inpatients with hypertension. Methods From June 2015 to December 2017,we conducted to examine elderly patients aged ≥ 65 years at our hospital. Patients’basic information,biochemical indicators,comorbidities and polypharmacy were recorded at admission. We used 6m walking test to measure gait speed. Activities of daily living was assessed by Katz⁃ADL,cognitive function by mini⁃mental state Examination(MMSE)and nutrition level by micronutrient assess⁃ ment method summary(MNA⁃SF). After discharge,all⁃cause mortality was followed up for more than 2 year and recorded. We analyzed the correlation between gait speed and mortality. Results A total of 515 patients[(84.1 ± 5.9)years,58.1% men]were enrolled. During a median follow ⁃up of 3.3 years(IQR 2.7,3.9),106 participants (20.6%)died. Spearman correlation analysis revealed that gait speed was negatively correlated with age,CCI and polypharmacy,and positively correlated with 24hDBP levels,Katz⁃ADL,MMSE,and MNA⁃SF scores(P < 0.05). Gait speed was divided into tertile groups. The Kaplan Meier curve showed that mortality increased with the decrease of gait speed(log rank:P < 0.001). Cox regression analysis with adjustment for potential confounding variables showed that compared with the highest group,the lowest group had a higher risk of mortality(HR = 2.325,95%CI 1.113 ~ 4.855,P = 0.025).
