安氏Ⅰ类骨面型Ⅰ类错 正畸后下颌磨牙轴倾度的变化

Inclination angle of mandibular molars after orthodontic treatment in patients with Angle Class Ⅰ and skeletal ClasssⅠ malocclusion

摘要: 目的 通过锥形束 CT(CBCT)技术测量安氏Ⅰ类、骨性Ⅰ类错 正畸前后下颌磨牙轴倾度 及其变化,获得错颌畸形患者下颌磨牙矫治后轴倾度理想范围,提高错 矫治效果。方法 筛选 28 例安 氏Ⅰ类、骨性Ⅰ类患者头颅 CBCT 扫描数据,对影像行校正、重建,测得下颌磨牙近远中向轴倾度,并行统 计学分析。结果 下颌第一和第二磨牙矫治前轴倾度分别为(13.9 ± 6.07)°和(19.0 ± 7.64)°,下颌第一和 第二磨牙矫治后轴倾度分别为(10.8 ± 6.07)°和(15.0 ± 8.38)°。下颌第一与第二磨牙矫治前后轴倾度差 异有统计学意义。成年组与未成年组之间及不同下颌平面角类型患者之间下颌磨牙矫治前轴倾度差异 无统计学意义(P > 0.05)。结论 CBCT 可直观便捷准确地测量下颌磨牙近远中轴倾度,并获得了正畸必 要的下颌第一、第二磨牙接近正常 的理想轴倾度范围。

Abstract:

Objective Cone beam computed tomography(CBCT)technology was used to measure the inclination angle of mandibular molars before and after orthodontic treatment of Angle Class Ⅰand skeletal class malocclusion,in order to obtain the ideal range of inclination angle of mandibular molars after orthodontic treat⁃ ment of malocclusion patients,improve the curative effect. Methods The CBCT scanning data of 28 patients with angle classⅠand skeletal classⅠmalocclusion were screened,and the images were corrected,reconstructed and measured. The mesial and distal inclination angles of mandibular molars were measured and statistically analyzed. Results The initial inclination angles of the mandibular first molar and mandibular second molar before orthodontic treatment were(13.9 ± 6.07)° and(19.0 ± 7.64)°,and the postoperative inclination angles of the mandibular first molar and mandibular second molar were(10.8 ± 6.07)° and(15.0 ± 8.38)° respectively. There is a statistical differ⁃ ence between the inclination angles of the mandibular first molar and second molar before and after treatment. There was no statistical difference in the initial inclination angle of mandibular molar inclination between adults and juveniles and also no statistical difference between patients with different mandibular plane angle patterns. Conclusion CBCT can directly,conveniently and accurately measure the axial inclination of mandibular molars, and obtain the ideal axial inclination range of mandibular first and second molars which is necessary for orthodontics.
