摘要:

目的 分析内皮特异性分子⁃1(Endocan)、纤维蛋白原(Fib)水平在慢性阻塞性肺疾病急性 加重期(AECOPD)的诊断、病情严重程度评估中的价值。方法 收集苏大附三院 2021 4 月至 2022 4 月期间,在呼吸与危重症医学科进行救治的慢性阻塞性肺疾病(COPD)急性加重期患者 79 例为 AECO⁃ PD 组(根据疾病严重程度分为 3 个亚组),选同期门诊复诊的 30 例慢性阻塞性肺疾病稳定期(SCOPD)患 者为 SCOPD 组,另选取在本院进行健康体检的 30 例为对照组。比较各组血清 Endocan、Fib 水平,分析上 述因子在 AECOPD 诊断中的预测价值。结果 AECOPD 组和 SCOPD 组的 Endocan、Fib 水平均显著高于对 照组(均 P < 0.05),且上述因子水平以 AECOPD 组最高。在 AECOPD 各亚组间的比较,发现 Endocan、Fib 水平差异均有统计学意义(均 P < 0.05),并随 AECOPD 级别增高而增加;受试者工作特征曲线分析结果显 示,Endocan、Fib 的曲线下面积(AUC)分别为 0.832、0.729,二者联合检测的 AUC 更高,为 0.855。结论 血清 Endocan、Fib 的表达水平与 AECOPD 患者的疾病严重程度相关,可作为疾病诊断、病情评估的重要指 标,且Endocan 的诊断效能优于Fib。

Abstract:

Objective To analyze the value of endothelial ⁃specific molecule ⁃ 1(Endocan)and fibrinogen (Fib)levels in the diagnosis and severity assessment of acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods A total of 79 patients with acute exacerbation of chronic obstructive pulmonary disease (COPD)treated in the department of respiratory and critical care medicine from April 2021 to April 2022 were collected as a AECOPD group. Thegroup was then divided into three subgroups based on disease severity. 30 patients with stable chronic obstructive pulmonary disease(SCOPD)who revisited our clinics during the same period were selected as a SCOPD group,and 30 individuals who underwent physical examination in our hospital were selected as acontrol group. Serum levels ofEndocan and Fib were comparedamong the groups,and the predictive value of the above factors in the diagnosis of AECOPD was analyzed. Results Levels of Endocan and Fib in the AECOPD and SCOPD groups were significantly higher than those in the control group(P < 0.05 for all comparisons),demonstra⁃ tingthe highest levelsin the AECOPD group. Levels of Endocan and Fib were significantly different among the AECO⁃ PD subgroups(P < 0.05),and increased with the severitylevel of AECOPD. The receiver operating characteristic curve analysis results showed that the area under the curve(AUC)of Endocan and Fib were 0.832 and 0.729,respec⁃ tively;and the AUC of the combined detection was higher,which was 0.855. Conclusions The expression levels of serum Endocan and Fib are related to the severity of AECOPD,which can be used as an important indicator for the diagnosis and assessment of disease course. The diagnostic efficiency of Endocan is better thanthat of Fib.
