煤矿工人高血压患病情况及影响因素分析

摘要: 目的 了解煤矿工人高血压患病情况及其影响因素。方法 选取2017年4-6月山西省某国有企业男性煤矿工人652例作为研究对象,按照2010年《中国高血压防治指南》诊断标准判定高血压;采用高效液相色谱法(HPLC)测定尿中4种多环芳烃代谢物(OH⁃PAHs)的浓度;采用χ2检验、Wilcoxon秩和检验和多因素logistic回归模型对煤矿工人高血压的影响因素进行分析。结果 煤矿工人高血压总体患病率为33.7%。Wilcoxon秩和检验结果显示,尿PAHs代谢物水平在正常血压与高血压组间的差异无统计学意义(P>0.05)。logistic回归分析结果显示,年龄>40岁、饮酒、超重和肥胖均是煤矿工人高血压发生的危险因素。井下一线和井下辅助作业是煤矿工人高血压发生的保护因素。结论 没有发现尿PAHs代谢物水平与高血压之间有关联;年龄>40 岁、BMI ≥ 24 kg/m2及有不良生活行为习惯的煤矿工人罹患高血压的风险较高,应加强对此类人群的健康宣教和日常监测。

Abstract:

Objective To understand the prevalence of hypertension in coal miners and analyze its influ⁃encing factors. Methods From April 2017 to June 2017,we selected 652 male coal miners from a state⁃ownedenterprise in Shanxi Province as research subjects. Hypertension was determined according to the diagnostic criteriaof the 2010 Chinese guidelines for the prevention and treatment of hypertension. The concentrations of 4 PAHsmetabolites in urine were determined by HPLC. The influencing factors of hypertension in coal miners were analyzedby chi⁃square test,Wilcoxon rank⁃sum test and multivariate logistic regression model. Results The overall prevalenceof hypertension among coal miners was 33.7%. Wilcoxon rank⁃sum test showed that there was no statisticallysignificant difference in the level of urinary PAHs metabolites between the groups of normal blood pressure andhypertension(P > 0.05). Logistic regression analysis showed that age > 40,alcohol consumption,overweight andobesity were all risk factors for hypertension in coal miners and the protective factors of hypertension in coal mineworkers were the underground first⁃line work and underground auxiliary job. Conclusion No association is foundbetween urinary PAHs metabolites and hypertension. Coal miners aged over 40 years,with BMI ≥ 24 kg/m2,andpoor living habits have a higher risk of hypertension. Health education and daily monitoring of such people shouldbe strengthened.
