关节镜下改良压配式双排缝合技术对巨大肩袖撕裂患者肩关节功能、活动度及疼痛的影响

Influences of arthroscopic improved press⁃fit double⁃row suture technique on shoulder function,range of motion and pain in patients with large rotator cuff tear

摘要:

目的 探究关节镜下改良压配式双排缝合(Improved⁃press⁃fit DR)技术对巨大肩袖撕裂患者 肩关节功能、活动度及疼痛的影响。方法 选取 65 例巨大肩袖撕裂患者,随机分为对照组(n = 32)和观察组(n = 33)。其中对照组给予关节镜下缝合桥(Suture⁃Bridge)技术治疗,观察组给予 Improved⁃press⁃fit DR 技术治疗。使用美国肩肘外科协会(ASES 评分)评分标准、美国加州大学洛杉矶分校(UCLA 评分) 肩关节评分系统、肩关节 Constant⁃Murley 评分对两组患者进行肩关节功能评定,使用[欧洲五维健康量 表(EQ⁃5D)]进行生活质量评定,进行活动度(体侧外旋、前屈)评定,使用视觉疼痛模拟评分法(VAS) 进行疼痛评定。结果 与治疗前比较,两组治疗后 ASES 评分、UCLA 评分、Constant 评分、EQ⁃5D 评分、 体侧外旋、前屈度数均明显升高(P < 0.05),且与对照组比较,观察组明显升高(P < 0.05);两组治疗后 VAS 评分均明显降低(P < 0.05),且与对照组比较,观察组明显降低(P < 0.05);观察组再撕裂率 (3.03%)明显低于对照组(25.00%)(P < 0.05)。结论 关节镜下 Improved⁃press⁃fit DR 技术治疗巨大肩 袖撕裂患者,可有效提高肩关节功能及活动度,减轻疼痛,改善生活质量,减少再撕裂发生率,具有更好的临床疗效。

Abstract:

Objective To investigate the influences of arthroscopic improved press⁃fit double⁃row suture technique on shoulder function,range of motion and pain in patients with large rotator cuff tear. Methods Sixty⁃ five patients with large rotator cuff tear admitted to our hospital were collected and randomly assigned to control group(n = 32)and observation group(n = 33). The control group was treated with arthroscopic Suture ⁃Bridge technique,and the observation group was treated with Improved⁃press⁃fit DR technique. The shoulder joint function [American Society for Shoulder and Elbow Surgery(ASES score)scoring standard,University of California,Los Angeles(UCLA score)shoulder joint scoring system,shoulder Constant⁃Murley score],quality of life[European five⁃dimensional health scale(EQ⁃5D)],range of motion(lateral external rotation,forward flexion),visual ana⁃ logue scale(VAS)and rotator cuff healing were compared. Results Compared with before treatment,ASES score, UCLA score,Constant score,EQ⁃5D score,lateral external rotation dimension,and forward flexion dimension in both groups were obviously increased after treatment(P < 0.05),and observation group had obviously higher results than those of the control group(P < 0.05);the VAS scores of both groups were obviously decreased after treatment(P < 0.05),and the observation group was obviously decreased compared with the control group(P < 0.05); the re ⁃tear rate of the observation group(3.03%)was obviously lower than that of the control group(25.00%) (P < 0.05). Conclusion Arthroscopic improved press ⁃fit DR technique for patients with large rotator cuff tear could effectively enhance shoulder joint function and range of motion,reduce pain,improve quality of life,and reduce the incidence of re⁃tear,with good results.
