肺泡灌洗液宏基因组测序在儿童重症社区获得性肺炎的诊断价值

摘要:

目的 探讨肺泡灌洗液(BALF)宏基因组测序技术对儿童重症社区获得性肺炎的诊断价值。方法 选取 2019 3 月至 2020 2 月期间因重症社区获得性肺炎需肺泡灌洗治疗的患儿,所得 BALF 时进行传统病原学检测与宏基因组测序(metagenomics next⁃generation sequencing,mNGS)病原检测,分析两种检测方法对重症获得性肺炎病原学检测的诊断效能。结果 29例患儿的BALF同时进行传统病原检测 mNGS检测,细菌检出率mNGS明显高于传统检测方法(58.6% vs. 20.7%),差异有统计学意义(P = 0.003)。病原菌阳性率与 2 种以上病原体感染上,mNGS 明显高于传统检测方法(93.1% vs. 72.4%,34.5% vs. 0%),差异有统计学意义(P<0.05)。在 mNGS 检测与传统检测结果一致性上,在不一致的病原体、细菌与真菌中,培养阴性多于 mNGS 阴性结果。在复合感染中,mNGS 明显高于传统检测方法(58.6% vs. 27.6%),差异有统计学意义(P = 0.017)。结论 在儿童重症社区获得性肺炎中,BALF mNGS 对病原菌的检出率高于传统检测方法,尤其是对细菌与复合感染时诊断价值更高,能早期对重症社区获得性肺炎提供诊治依据。

Abstract:

Objective To investigate the value of bronchoalveolar lavage fluids(BALF)metagenome next⁃ generation sequencing in the diagnosis of severe community ⁃ acquired pneumonia in children. Methods From March 2019 to February 2020,29 children with severe community⁃acquired pneumonia treated with bronchoalveolar lavage were included in the study. The BALFs obtained during lavage were detected by both traditional etiology and metagenomics next generation sequencing(mNGS)simultaneously,and the diagnostic efficacy of the two detection methods for etiology of severe community⁃acquired pneumonia was analyzed. Results The detection rate of bacteria in the BALFs of all 29 children by mNGS was significantly higher than that by the traditional detection methods (58.6% vs. 20.7%)(P = 0.003). The positive rate of pathogens and the infection detection rate of more than two pathogens by mNGS were significantly higher than by the traditional detection method(93.1% vs. 72.4%,34.5% vs. 0%;P = 0.041,P = 0.001). In terms of the consistency between MNGS and traditional detection results,the negative results of culture were more than the negative results of MNGS in the pathogens with inconsistent results of the two detection methods. The detection rate of compound infections by mNGS was significantly higher than by the traditional detection methods(58.6% vs. 27.6%,P = 0.017). Conclusion In children with severe community⁃ acquired pneumonia,the detection rate of bacterial pathogens from BALF by way of mNGS is higher than by tradi⁃ tional detection methods,especially for bacterial and compound infections. So mNGS can provide early diagnosis and treatment basis for severe community⁃acquired pneumonia.
