阿尔茨海默病海马与全脑功能连接的时间变异性研究

The temporal variability of functional connectivity between hippocampi and the whole brain in Alzheimer′s disease

摘要: 目的 探讨阿尔茨海默病(Alzheimer′s disease,AD)海马与全脑功能连接的时间变异性及其与认知功能的相关性。方法 共纳入37名被试者,包括18例AD患者(AD组)和19例正常者(对照组)。使用简明精神状态量表(mini-mental state examination,MMSE)评估认知功能。同时收集三维结构MRI和静息态功能MRI数据。影像专业人员利用滑动时间窗分析图像并处理数据,以双侧海马为感兴趣区,分别计算每个兴趣区与全脑各区功能连接的时间变异性;比较两组海马与全脑动态功能连接差异。结果 两组相比,AD组的左侧海马与右侧尾状核的动态功能连接时间变异性显著高于对照组,差异有统计学意义(GRF多重比较,体素水平P < 0.005并且团块水平P < 0.05),其变异值与认知评分差异无统计学意义(P > 0.05)。右侧海马与全脑动态功能连接时变性在两组间差异无统计学意义(P > 0.05)。结论 AD患者左侧海马与右侧尾状核的功能连接具有增强的时间变异性,为理解AD的病理机制提供了新的视野。

Abstract: Objective To investigate the temporal variability of functional connectivity between hippocampi and the whole brain and to examine the correlation of the variability with cognitive performance in Alzheimer′s disease (AD). Methods A total of 37 subjects were enrolled, including 18 AD patients and 19 normal controls (NC). Their cognitive abilities were evaluated with mini-mental state examination (MMSE). The data of 3D structural MRI and resting-state functional MRI werecollected at the same time. Using sliding time window analysis, the image professionals analyzed the images, processed the data and calculated the temporal variability of functional connectivity between each regions of interest (e.g., the bilateral hippocampi) and the whole brain. Then, the differences between the two groups of hippocampi and whole-brain dynamic functional connectivity were compared. Results Compared the temporal variability of functional connectivity of both groups, there was statistically significant differences between the left hippocampus and the right caudate nucleus in AD groups (GRF multiple comparison, voxel P < 0.005, cluster P < 0.05), however, no statistically significant differences were found between the temporal variability of functional connectivity and the cognitive scores (P > 0.05). Also, there was no statistically significant differences (P > 0.05) between the right hippocampus and the whole brain. Conclusion sEnhanced temporal variability of dynamic functional connectivity is existed between the left hippocampus and the right caudate nucleus in AD, whichprovides new horizons for understanding the pathological mechanism of AD.
