摘要

目的 探讨基于激痛点理论的针刀联合中药膏摩对胸背肌筋膜炎(TDMS)患者的影响及机制。 方法 收集2022年3月至2023年3月安徽中医药大学第二附属医院收治的100例TDMS患者为研究对象,数字随机法分为对照组50例和研究组50例,对照组接受中药膏摩治疗,研究组接受针刀联合中药膏摩治疗。每组每天治疗1次,连续治疗3周。比较两组患者的治疗总有效率、治疗前后Oswestry功能障碍指数问卷(ODI)、简式McGill疼痛问卷量表(SF-MPQ)评分、贝克焦虑量表(BAI)评分及不良反应。 结果 研究组治疗总有效率(94.00%)高于对照组(78.00%)(χ 2 = 5.316,P = 0.021)。治疗后两组ODI、BAI评分、疼痛分级指数(PRI)、视觉模拟评分(VAS)、现时疼痛强度(PPI)及SF-MPQ总分均较治疗前降低,且研究组低于对照组(P < 0.05)。研究组总不良反应发生率(6.00%)与对照组(4.00%)比较,差异无统计学意义(P > 0.05)。 结论 基于激痛点理论的针刀联合中药膏摩治疗可有效改善TDMS患者的功能障碍、焦虑情绪及疼痛程度,且不良反应较少,安全性高。

Abstract

Objective To investigate the effects and mechanisms of acupuncture combined with Chinese herbal cream mo based on the agonistic point theory on patients withthoracic and dorsal myofascitis syndrome (TDMS). Methods One hundred patients with TDMS admitted to the second affiliated Hospital of anhui University of traditional Chinese medicine from March 2022 to March 2023 were collected as the study subjects, and were divided into 50 cases in the control group and 50 cases in the study group by digital randomization method, with the control group receiving Chinese herbal paste and moisturizing treatment and the study group receiving acupuncture combined with Chinese herbal paste and moisturizing treatment. Each group was treated once a day for 3 weeks. The total treatment efficiency of the two groups was compared, and the Oswestry Dysfunction Index Questionnaire (ODI), Short Form McGill Pain Questionnaire (SF-MPQ) scores, Beck Anxiety Inventory (BAI) scores and adverse effects were analyzed before and after treatment. Results Regarding the total treatment efficiency, the total treatment efficiency of the study group was 94.00%, which was statistically significant when compared with 78.00% in the control group (χ 2 = 5.316, P = 0.021). After treatment, ODI, BAI score, PRI, VAS, PPI and SF-MPQ total scores were lower in both groups than before treatment, and the study group was lower than the control group (P < 0.05). For adverse reactions, the overall incidence of 6.00% in the study group was not statistically significant when compared with 4.00% in the control group (P > 0.05). Conclusions Acupuncture combined with Chinese herbal cream mo based on the agonistic point theory has a positive therapeutic effect on patients with TDMS, and can effectively improve patients' functional impairment, anxiety and pain level with fewer adverse effects and high safety.
