Effect of Qi Chang Xue Xing Tang retention enema combined with heat sensitive moxibustion on gastrointestinal function and immune function of patients after radical gastrectomy

目的 观察气畅血行汤保留灌肠配合热敏灸在胃癌根治术后的应用效果。 方法 将我院2019年1月至2022年1月收治的83例胃癌采用信封随机法分为两组。对照组胃癌术后给予生理盐水灌肠治疗，观察组胃癌术后给予气畅血行汤保留灌肠配合热敏灸治疗，比较两组效果。 结果 观察组肠胃功能恢复指标均优于对照组（P < 0.05）。观察组除术后3 d肠球菌与对照组比较差异无统计学意义（P > 0.05）外，术后3 d、7 d其他肠道菌群均优于对照组（P < 0.05）。观察组术后3 d、7 d时Shannon指数、Chaol指数、胃泌素（GAS）、胃动素（MTL）高于对照组，血管活性肽（VIP）、中医症候积分低于对照组（P < 0.05）。观察组除术后7 d时CD8+与对照组比较差异无统计学意义（P > 0.05）外，术后3 d、7 d时免疫功能指标均优于对照组（P < 0.05）。 结论 气畅血行汤保留灌肠配合热敏灸有利于胃癌根治术后患者胃肠功能的恢复，促进免疫功能的恢复，可能与调节肠道菌群多样性有关。

Objective To observe the effect of Qichang Xuexheng decoction retention enema combined with heat-sensitive moxibustion after radical gastrectomy. Methods Eighty-three cases of gastric cancer admitted to our hospital from January 2019 to January 2022 were divided into two groups by envelope random method. The control group was given normal saline enema after operation for gastric cancer， and the observation group was given Qichang Xuexheng Decoction retention enema combined with heat-sensitive moxibustion after operation for gastric cancer. Results The indexes of gastrointestinal function recovery in observation group were better than those in control group （P < 0.05）. There was no significant difference between the observation group and the control group （P > 0.05）， and the other intestinal flora in the observation group was better than that in the control group （P < 0.05） at 3 and 7 days after surgery. Shannon index， Chaol index， gastrin （GAS） and motilin （MTL） in observation group were higher than those in control group at 3 and 7 days after operation， vasoactive peptide （VIP） and TCM syndrome score were lower than those in control group （P < 0.05）. There was no significant difference in CD8+ between the observation group and the control group at 7 days after surgery （P > 0.05）， and the immune function indexes of the observation group were better than those of the control group at 3 and 7 days after surgery （P < 0.05）. Conclusion Qi Chang Xue Xing Tang retention enema combined with heat sensitive moxibustion is beneficial to the recovery of gastrointestinal function and immune function in patients after radical gastrectomy， which may be related to the regulation of intestinal flora diversity.
