Study on the relationship between bone mineral density and serum 25(OH)D and Irisin in elderly men with different degrees of sarcopenia

目的 研究不同程度肌少症(sarcopenia,SP)老年男性人群骨密度(bone mineral density, BMD)及血清 25 羟维生素 D[25⁃hydroxy vitamin D,25(OH)D]、鸢尾素(Irisin)水平表达及在肌量、骨量流失 中的作用及意义。方法 筛选遵义医科大学附属医院内分泌科门诊及住院老年肌少症男性 225 例,根据 肌少症不同程度分为 3 组:肌少症前期(presarcopenia)组(A 组,75 例)、肌少症(sarcopenia)组(B 组 75 例)、 严重肌少症(severe sarcopenia)组(C 组,75 例)。同时选取来自体检中心的健康体检者,匹配其年龄后纳入 正常对照组(NC 组,75 例),采用酶联免疫吸附法(enzyme linked immunosorbent assay,ELISA)测定所有受 试者血清 Irisin 水平。结果 C 组 BMD 明显低于 A 组、B 组及 NC 组(均 P < 0.05);A 组 BMD 明显低于 NC 组 差异有统计学意义,与 B 组间差异无统计学意义;不同 SP 严重程度分级患者血清 Irisin、25(OH)D 水平之 间差异均有统计学意义(均 P < 0.05)。Pearson 结果显示:血清 Irisin 与 BMD(L1⁃4、Neck)、SMI、握力、步 速、25(OH)D、HDL⁃C 呈正相关,与 BMI、LDL⁃C 呈负相关;血清 25(OH)D 水平与 BMD(L1⁃4、Neck)、SMI、握 力、步速、HDL⁃C、Irisin 呈正相关,与 BMI、TG 呈负相关。结论 老年男性人群肌量、骨量流失与 25(OH)D 与Irisin 低水平表达相关,联合检测有助于SP的诊断及治疗。

Objective To investigate the role and significance of Bone Mineral Density(BMD)and serum 25⁃hydroxy vitamin D(25(OH)D)and Irisin levels expression in muscle mass and bone loss in the elderly male population with different degrees of sarcopenia(SP). Methods 225 male elderly patients with sarcopenia were selected from the outpatient and inpatient departments of endocrinology in the Affiliated Hospital of Zunyi Medical University,and then divided into three groups according to the different degrees of sarcopenia:the pre⁃ sarcopenia group(75 cases in group A),the sarcopenia group(75 cases in group B),and the severe sarcopenia group(75 cases in group C). Healthy check⁃ups from medical examination centers were also selected and matched for age and included in the normal control group(75 cases in the NC group),and serum Irisin levels were measured by Enzyme⁃Linked Immunosorbent Assay(Elisa)in all subjects. Results BMD was significantly lower in group C than in groups A,B,and NC(all P < 0.05);the difference between BMD was significantly lower in group A than in group NC was statistically significant,and there was no statistical difference between group A and group B. The differences between serum Irisin and 25(OH)D levels in patients with different SP severity grades were statistically significant(all P < 0.05). Pearson results showed that serum Irisin was positively correlated with BMD (L1⁃4,Neck),SMI,grip strength,gait speed,25(OH)D,HDL⁃C,and negatively correlated with BMI and LDL⁃C; serum 25(OH)D level was positively correlated with BMD(L1⁃4,Neck),SMI,grip strength,gait speed,HDL⁃C, Irisin,and negatively correlated with BMI and TG. Conclusion Muscle mass and bone loss are associated withlow levels of 25(OH)D and Irisin expression in the elderly male population,and their combined testing can help in the diagnosis and treatment of sarcopenia.
