平衡易位携带者可利用囊胚形成率的影响因素

摘要:

目的 探讨胚胎植入前遗传学检测(preimplantation genetic testing,PGT)周期中平衡易位携 带者可利用囊胚形成的影响因素。方法 回顾性分析 2017 年 1 月至 2021 年 12 月期间在郑州大学第三附 属医院生殖中心行 PGT 治疗的 331 对夫妇的 421 个染色体平衡易位周期临床资料。按照 ISCN 人类染色体 分组标准,根据易位染色体大小分为 A 组和非 A 组;根据其断裂点位置不同分为 pp 组、qq 组和 pq 组。采用多重线性回归分析探讨影响可利用囊胚形成率的独立影响因素。结果 排除各项混杂因素后,多重线 性回归分析显示:(1)相互易位患者 A 组可利用囊胚形成率显著低于非 A 组(B = -0.089,P = 0.001);携带 者性别对可利用囊胚形成率无显著影响(P = 0.713);(2)罗氏易位患者 DD 组与 DG 组间可利用囊胚形成 率差异无统计学意义(P = 0.261);携带者性别不是其独立影响因素(P = 0.510);(3)易位类型不是可利用 囊胚形成率的独立影响因素(P = 0.715)。结论 易位类型不影响平衡易位携带者可利用囊胚的形成;相互易位患者易位染色体长度影响可利用囊胚形成率。罗氏易位携带者的性别及染色体类型对可利用囊 胚形成率均无显著影响。

Abstract:

Objective To investigate the influencing factors of available blastocysts formation in chromo⁃ some balanced translocation carriers. Methods The clinical data of 421 chromosome balanced translocation cycles of 331 couples undergoing preimplantation genetic testing(PGT)in the Reproductive Center of the Third Affiliated Hospital of Zhengzhou University from January 2017 to December 2021 were retrospectively analyzed in this study. According to the ISCN standard of human chromosome grouping,the enrolled subjects were divided into group A and non⁃Group A. According to the location of the breakpoints,the enrolled subjects were sub⁃divided into group qq,group pp and group pq. Multiple linear regression analysis was used to analyze the independent factors affect⁃ ing available blastocyst formation rate. Results After excluding all confounding factors,it was found that:(1)In reciprocal translocation subjects,the available blastocysts formation rate in group A was significantly lower than that in non⁃group A(B = -0.089,P = 0.001),while sex had no significant effect on the rate of available blastocyst formation(P = 0.713).(2)In the subjects with Robertsonian translocation,there was no significant difference in the rate of available blastocyst formation between the DD and DG groups(P = 0.261)and sex was not an indepen⁃ dent factor affecting the rate of available blastocyst formation(P = 0.510).(3)In the balanced translocation popu⁃ lation,translocation type was not an independent factor affecting the formation rate of available blastocysts(P = 0.715). Conclusion The type of translocation does not affect the formation of available blastocysts in the carriers of balanced translocation. In reciprocal translocation patients,the chromosome length affects the rate of available blastocyst formation. Sex and chromosome type of Robertsonian translocation carriers have no significant effect on the formation rate of available blastocysts.
