摘要：

目的探讨 HIV 相关神经认知障碍（HAND）患者外周血单个核细胞（PBMCs）中 miR⁃744⁃5p 和 miR⁃543 表达水平及其对 HAND 诊断的临床价值。方法选取 HIV⁃1 感染患者 76 例，分为 HAND 组（45 例）和对照组（31 例）。采用 qRT⁃PCR 法检测患者外周血 PBMCs 中 miR⁃744⁃5p 和 miR⁃543 表达水平； ELISA 法检测血清 IL⁃6、IL⁃10、IL⁃16、IL⁃18 和 TNF⁃α 水平；全自动生化分析仪检测血清 HDL、TG、LDL 和 TC 含量。统计学方法分析 HIV⁃1 感染者发生 HAND 的危险因素以及 miR⁃744⁃5p 和 miR⁃543 表达水平对 HAND的临床诊断价值。结果与对照组比较，HAND 组患者外周血 PBMCs 中 miR⁃744⁃5p 表达水平、IL⁃10 和 IL⁃16 水平升高，miR⁃543 表达水平和 HDL 水平降低，其它炎症及脂代谢指标无显著性变化；统计学分析表明，miR⁃744⁃5p 高表达以及 miR⁃543 低表达均是 HIV⁃1 感染者发生 HAND 的独立危险因素，而 miR⁃744⁃ 5p 和 miR⁃543 均对 HAND 有良好的临床诊断价值，且两者联合诊断效果更佳。结论 miR⁃744⁃5p 和 miR⁃ 543对预测HAND 有良好的临床诊断价值，这可能与慢性炎症以及脂代谢过程有关。

Objective To investigate the expression levels of miR ⁃ 744 ⁃ 5p and miR ⁃ 543 in peripheral blood mononuclear cells（PBMCs）in patients with HIV ⁃ associated neurocognitive disorders（HAND）and their clinical value in the diagnosis of HAND. Methods A total of 76 patients with HIV⁃1 infection were divided into HAND group（n = 45）and control group（n = 31）. qRT⁃PCR was used to detect the expression levels of miR⁃744⁃ 5p and miR⁃543 in PBMCs. ELISA was used to detect the levels of serum IL⁃6，IL⁃10，IL⁃16，IL⁃18 and TNF⁃α； The contents of serum HDL，TG，LDL and TC were detected by automatic biochemical analyzer；The risk factors of HAND in HIV infected patients and the clinical diagnostic value of miR ⁃ 744⁃5p and miR ⁃543 expression in HAND were analyzed by statistical methods. Results Compared with the control group，the expression levels of miR⁃744⁃5p，IL⁃10 and IL⁃16 in PBMCs of HAND group were significantly increased，while the expression levels of miR ⁃543 and HDL were significantly decreased，and the other inflammatory and lipid metabolism indexes had no significant changes；Statistical analysis showed high expression of miR⁃744⁃5p and low expression of miR⁃543 were independent risk factors for HAND in HIV⁃1 infected patients；miR⁃744⁃5p and miR⁃543 had good clinical diagnostic value for HAND，and the combined diagnostic effect was better. Conclusion miR ⁃744⁃5p and miR ⁃ 543 had good clinical diagnostic value for predicting HAND，which may be related to chronic inflammation and lipid metabolism process.
