全自动滴片在羊水细胞染色体制备中的应用价值

摘要:

目的 比较羊水细胞染色体全自动滴片与人工滴片的效果差异,探讨全自动滴片在染色体 制备中的临床应用价值。方法 常规全自动收获羊水细胞,在相同环境条件下采用全自动染色体滴片仪 和人工法分别对羊水细胞进行滴片,分析对比两种滴片方法获得的分裂相数量、染色体分散面积比、有效 核型率及染色体条带水平。结果 共纳入 47 份羊水标本,滴片 376 张,有效分析分裂相 1 066 个,其中全自动滴片获得 650 个,人工滴片获得 416 个。全自动滴片获得的分裂相数量多于人工滴片,核型质量更 高,差异有统计学意义(P < 0.000 1);两种方法获得染色体分散程度相当,差异无统计学意义(P > 0.05); 人工滴片用时更少,差异有统计学意义(P < 0.01)。结论 全自动染色体滴片仪制备染色体的质量及稳 定性优于传统人工法,值得临床推广采用。

Abstract:

Objective Discuss the clinical value of the automatic dropping in chromosome preparation by comparing the differences between the automatic and manual dropping of amniotic fluid cell chromosomes. Methods The amniotic fluid cells were harvested routinely and automatically. Under the same settings,the amniotic fluid cells were dropped by the metaphase chromosome auto ⁃ spreader and the manual method,respectively. The two methods were compared in terms of mitotic index,ratio of chromosome dispersion area,effective karyotype rate and chromosome band level. Results A total of 47 amniotic fluid specimens were included and dropped into 376 slides. 1066 mitotic phases were analyzed effectively,of which 650 were obtained by metaphase spreader and 416 were obtained manually. As a result,the mitotic index and the quality of the karyotype harvested by the automatic dropping were statistically significantly higher than that of the manual dropping(P < 0.0001). Meanwhile,the degree of chromosome dispersion obtained by the two methods was equivalent,without statistically significant differ⁃ ences between the methods. As for the efficiency,the time by the manual method was significantly shorter than the automatic dropping(P < 0.01). Conclusion The automatic dropping for chromosome preparation is better in quality and stability than the manual method,and thus it is worthy of clinical application.
